{"title":"Do vitamin D levels affect forced expiratory flow 25-75 among children with asthma?","authors":"Veena Anand, S. Joseph, I. Forced expiratory flow 25-75 (FEF 25-75) is considered a sensitive parameter in children as it is effort independent.Objectives: To assess the correlation between serum vitamin D levels and pulmonary function parameters in children with asthma.Method: This cross-sectional analytic study was conducted at the paediatric respiratory clinic of a tertiary care referral hospital. Children aged 6-12 years, having asthma, on controllers with good compliance and under regular follow-up were enrolled.Results: Mean serum vitamin D level (16.8± 6.69ng/ml) was low and deficiency was seen in 36 (72%) children with asthma. Positive correlation (r=0.41) was seen between serum vitamin D levels and predicted FEF 25-75%. Univariate analysis between vitamin D and FEF 25-75 showed decreased vitamin D levels were associated with significantly low FEF 25-75 (p<0.001). It is rampant even in children of upper and middle classes of the society.Objectives: To study the aetio-pathology of anaemia in children belonging to upper and upper middle socio-economic scale and to correlate it with their nutritional status. Method: It was a cross-sectional descriptive study on 165 preschool children aged 6-59 months who presented with pallor on clinical examination. They were advised to undergo a complete haemogram and further specific tests as required. Results: The most common cause of anaemia was nutritional, including iron deficiency (92%), combined iron and B12 deficiency (2%) and isolated B12 deficiency (0.6%). Thalassaemia was the next common cause (3%) followed by B-acute lymphoblastic leukaemia (1%) and microangiopathic haemolytic anaemia (0.6%). Maternal education and nutritional awareness had positive correlation with anaemia. Majority of children were normally nourished despite being anaemic, However, undernourished children were more severely anaemic. Antibiotic resistance patterns in Salmonella enterica typhi (S. typhi) infections are constantly changing. Continual surveillance of resistance levels of S. typhi is critical for clinicians to update antibiotic options.Objectives: To evaluate the current trends in antibiotic sensitivity and resistance to S. typhi in children in developing countries.Method: A cross sectional observational study was carried out on 102 children aged 6 months to 5 years with culture positive S. typhi infection in a tertiary care hospital in North India. Antibiotic susceptibility information of S. typhi was collected and the pattern of antibiotic resistance was analysed.Results: S. typhi showed a sensitivity of 78.4% to ampicillin and 99% to trimethoprim+ sulfamethoxazole which were the previously used antibiotics. However, it showed 100% resistance to ciprofloxacin and nalidixic acid. Serum phosphorus level abnormalities are common among them and go unnoticed in sick children.Objectives: To determine the prevalence of phosphorus abnormalities in sick children admitted to the paediatric intensive care unit (PICU) and its association with various clinical outcomes.Method: We conducted a prospective, non-interventional study at a PICU in North India. During an 18-month period we investigated the incidence of phosphorus abnormalities in patients admitted to the PICU on admission and followed the course at day 3 and day 7 during their stay.Results: The prevalence of phosphorus level abnormalities was 44.2% on admission to PICU; 34.9% patients had hypophosphataemia while 9.3% had hyperphosphataemia. Hypophosphataemia was significantly associated with severe malnutrition (p=0.041) while hyperphosphataemia was associated with organ failure (p=<0.001). Although the need for mechanical ventilation, duration of PICU stay and mortality were higher in hypophosphataemic patients and PRISM score was higher in hyperphosphataemic patients, these differences were not statistically significant. Changes in the baseline phosphorus levels after PICU stays of 3 days (p=0.346) and of 7 days (p=0.782) were not statistically significant.Conclusions: Phosphorus abnormalities were highly prevalent in ill children admitted in the PICU.
