Anders als Frakturen sind sie oft klinisch schwer fassbar und manifestieren sich nicht selten in Form funktioneller Bewegungseinschränkungen aufgrund unbewusster Schonhaltungen. Die radiologische Diagnostik ist limitiert: Röntgen zeigt häufig keine Auffälligkeiten, während MRT-Bilder durch Artefakte verfälscht sein können. Eine strukturierte klinische Untersuchung ist daher essenziell. Der Beitrag beleuchtet vier relevante Instabilitätsformen: laterale, posterolaterale, mediale sowie longitudinale Instabilitäten. Es werden spezifische klinische Tests wie der Moving Valgus Stress Test, das Milking-Manöver sowie der Sleeper-Stretch zur Unterscheidung medialer Instabilität gegenüber anderen Pathologien wie GIRD (glenohumerales Innenrotationsdefizit) erläutert. Differenzialdiagnosen wie Osteochondrosis dissecans werden berücksichtigt, da sie ähnliche Symptome verursachen, jedoch andere therapeutische Konsequenzen haben. Ziel ist es, ein praxisnahes, differenziertes Vorgehen zur frühzeitigen Erkennung und Behandlung kindlicher Ellenbogeninstabilitäten darzustellen und so Langzeitfolgen zu vermeiden.</div></div><div><div>Elbow instabilities in children present a diagnostic and therapeutic challenge. Unlike fractures, they are often difficult to detect clinically and may manifest as functional movement limitations due to unconscious avoidance behavior. Radiological assessment is limited: conventional X-rays often appear normal, while MRIs are frequently compromised by motion artifacts. Thorough clinical evaluation is thus essential. This article reviews four primary forms of pediatric elbow instability: lateral, posterolateral, medial, and longitudinal. Specific clinical tests such as the moving valgus stress test, the milking maneuver, and the sleeper stretch are discussed to distinguish medial instability from conditions like GIRD (glenohumeral internal rotation deficit). Differential diagnoses, including osteochondritis dissecans, are also addressed, as they can mimic mechanical symptoms without true joint instability. The aim is to provide a practical and structured clinical approach for early identification and management of pediatric elbow instability, ultimately preventing long-term complications and improving functional outcomes.</div></div>","PeriodicalId":38801,"journal":{"name":"Sports Orthopaedics and Traumatology","volume":"42 1","pages":"Pages 81-88"},"PeriodicalIF":0.0,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147682176","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
