Sleep disorders are now considered to be one of the most common ailments among adults worldwide. There are many factors that can affect sleep disorders, including poor sleep hygiene, chronic stress, the use of stimulants or improper diet. The aim of the study is to investigate the relationship between eating habits and subjective sleep quality assessment in adults. Materials and Method. The study involved 520 randomly selected Polish adults – 388 women and 132 men. A diagnostic survey method was used in the data collection using a questionnaire consisting of 3 parts: metric, Sleep Quality Assessment Questionnaire and Dietary Quality Assessment Questionnaire. Results. Only 29 (8%) of respondents were satisfied with their sleep. The vast majority declared normal sleep quality – 81 (2%). Statistically significant correlations with the sleep quality index occurred only for frequency of consumption of energy drinks (p=0.0051) and sweetened drinks (p=0.0429). As the frequency of consumption of these products increased, the sleep quality index increased, and thus the sleep quality of the subjects deteriorated. Conclusions. Nutritional factors, such as irregular meals, low intake of fruit and vegetables, sugary and caffeine-containing beverages, as well as other lifestyle factors, such as low physical activity and smoking, may also affect sleep quality. Insulin resistance is a state of reduced tissue sensitivity to insulin action despite normal or elevated serum insulin levels. Liraglutide (a GLP-1 receptor agonist) stimulates insulin secretion from the pancreas while decreasing excessive glucagon release. GLP-1 analogues have a unique beneficial effect on weight loss by stimulating insulin secretion without increasing blood sugar levels excessively. The aim of the study was to investigate the effects of liraglutide on body mass index and insulin resistance in overweight and obese non-diabetic individuals. Materials and Method. The study involved 31 overweight and obese women (BMI of ≥25 kg/m2) who received liraglutide treatment between 2021–2022. Liraglutide 3.0 mg was used as an adjunct to behavioural therapy, which included a low-calorie diet and exercise programme. The study collected BMI, glucose and insulin levels, and HOMA index before and during liraglutide treatment. Body weight and blood tests were examined at baseline (BMI1, HOMA1), as well as at 3 (BMI2, HOMA2), and 6 months (BMI3, HOMA3) of treatment to observe any changes. Results. Average age of participants – 38.32±7.41 years. Changes in BMI from baseline (32.36±4.56 kg/m²) to 6 months (27.46±4.45kg/m²) were associated with a significant reduction in weight (p <0.001). Changes in HOMA index from baseline (4.73±1.48) to 3 (3.59±1.26) and 6 months (2.47±0.91) were statistically significant (p <0.03, p < 0.001). Conclusions. Liraglutide represents a promising option for reducing body weight and obesity in non-diabetic patients as an adjunct to behavioural therapy. Agonists GLP-1 receptor, representing a new class of antidiabetic drug, undoubtedly offer unique benefits for overweight or obesity patients. {"title":"Pleural effusion – a case report","authors":"Małgorzata Leciejewska","doi":"10.26444/monz/176748","DOIUrl":"https://doi.org/10.26444/monz/176748","url":null,"abstract":"Pleural effusion is a common abnormality detected in additional tests in patients hospitalized in Internal Medicine Departments. Properly carried out diagnostics allows to determine the cause of the fluid, thanks to which it is possible to initiate appropriate treatment. The paper presents the case of a 63-year-old man admitted to a district hospital due to dyspnea on exertion. Chest X-ray revealed fluid in the right pleura. The patient was admitted to the Department of Internal Medicine for diagnostics. Due to the large amount of fluid in the pleural cavity, the patient required multiple punctures. The article discusses the most important diagnostic tests that should be performed to determine the cause of the presence of fluid. A key role was assigned to distinguishing the etiology of the fluid based on Light’s criteria, and other features of the fluid enabling its differential diagnosis were taken into account. Possible diagnostic points were also presented and an attempt was made to briefly discuss the correct procedure. During the diagnostics, pleural lymphoma was suspected and the patient was referred to a higher-reference center for further diagnostics. The author presents the necessary diagnostic steps necessary to establish the final diagnosis. The article is presented in the form of single-choice questions in which the reader can actively participate. The aim of the study was to assess the role and availability of postpartum physiotherapy in women’s opinions. Material and methods. The study included 112 women aged 37 ± 7 years who gave birth at least once. The research tool was an online questionnaire. Results. Eleven of the surveyed women (10%) who had a consultation with a physiotherapist in the hospital after childbirth rated it at 7 ± 3.74, on a scale of 0-10, where 0 meant that they considered the consultation to be definitely unnecessary, and 10 definitely needed. Women who had no contact with a physiotherapist in the hospital after childbirth assessed the validity of such consultation at 5 ± 4.70. After hospitalization, 25 women (22%) consulted a physiotherapist. Among the women who did not (78%), more than a half (57%) did not know that there was such a possibility. Among the women who attended childbirth classes, 34% (n=15) were informed about the possibility of consulting a physiotherapist after childbirth. Conclusions. Women’s awareness of the competence of a urogynaecological physiotherapist should be increased, e.g. during childbirth classes and during hospitalization after childbirth. Solutions should be sought to enable women after childbirth an early access to urogynaecological physiotherapy, at least of an instructional nature.
