{"title":"Emergency department management of syncope.","authors":"James Morris","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Syncope is the transient loss of consciousness and postural tone, with spontaneous recovery. It accounts for approximately 1% of all emergency department visits and $5.6 billion in healthcare costs annually. In a very small subset of patients, syncope may be a warning sign for serious outcomes or death, but identifying these patients is challenging, as the emergency clinician must distinguish between life-threatening causes and the more common, benign etiologies. Low-yield and expensive testing is often performed, even for benign presentations. Much research on syncope is observational, and clinical decision rules frequently perform poorly in validation studies. In patients aged >65 years, the incidence approaches 10%, and the number of patients with atrial fibrillation is expected to almost double in the next 30 years. Atrial fibrillation and its associated comorbidities also carry significant healthcare cost. Electrocardiogram findings may be subtle at times, but prompt diagnosis is needed to maximize good outcomes, especially when patients are cardiovascularly compromised. Many vital structures may be affected in a patient with a traumatic neck injury, including the airway, digestive tract, and carotid and vertebral arteries. Emergency clinicians must also be prepared to diagnose and manage occult and delayed presentations of injury related to neck trauma. This supplement reviews advances and best practices in the evaluation and management of patients with neck trauma, with a focus on evidence-based guidelines. Appropriate use of pre- and postexposure prophylaxis can eliminate the risk of developing rabies if administered according to the United States Centers for Disease Control and Prevention Advisory Committee on Immunization Practices guidelines. Though rabies is very rare, rapid recognition of potential exposures is vital to patient care and protection of public health. This review focuses on the challenges of managing patients who are at risk for or have had a potential rabies exposure, indications and guidelines for administering pre- or postexposure prophylaxis, and requirements for reporting, testing, and monitoring. Patients commonly manifest hyperadrenergic signs and symptoms, necessitating admission to the intensive care unit, administration of intravenous sedatives, and frequently, adjunctive pharmacotherapy. This issue reviews the pathophysiology of alcohol withdrawal syndrome, describes the manifestations of alcohol withdrawal, and examines the available evidence for optimal treatment of alcohol withdrawal.
