{"title":"Two years of the COVID-19 pandemic in the population of Czech patients with inflammatory bowel disease treated with biological therapy – experience of the tertiary IBD center","authors":"K. Lukáš","doi":"10.48095/ccgh202314","DOIUrl":"https://doi.org/10.48095/ccgh202314","url":null,"abstract":"Summary: Background: Inflammatory bowel disease (IBD) may be associated with a more severe course of infections and a different response to vaccination, especially in complicated IBD course and in association with immune-modifying IBD treatment. The aim of this study was to describe COVID-19 pandemic during years 2020–2022 in IBD patients with long-term biological therapy. Methods: A retrospective analysis of SARS-CoV-2 infection incidence in the population of 1,177 IBD (Crohn’s disease or ulcerative colitis) patients with long-term biological therapy (IBD cohort) was performed. The incidence rate, crude incidence rate and standardized incidence ratio of COVID-19 in the IBD cohort, the odds ratio of infection depending on the type of biologic therapy administered, the dynamics of COVID-19 incidence depending on the predominant SARS-CoV-2 variant in the population and the current vaccination coverage of the IBD cohort were calculated. Results: From January 2020 to April 2022, 548 confirmed cases of COVID-19 (46.6%) were reported in the IBD cohort, with 39% share of PCR positivity in vaccinated individuals and with 95% occurrence of infection in unvaccinated part of the IBD cohort. Standardized incidence rate ratio of COVID-19 was 27% higher in the IBD cohort compared to the general Czech population. The dynamics of the development of the number of positive cases of COVID-19 in the IBD cohort was identical to the situation in the entire country. A higher odds ratio of the chances of infection was demonstrated in patients treated with tumor necrosis factor inhibitors, but not in patients treated with anti-integrins or monoclonal antibodies against interleukins. In the IBD cohort, 85.2% of patients were properly vaccinated, which was significantly more than the vaccination rate of the entire Czech population. Discussion and conclusion: During the two pandemic years, the incidence of COVID-19 in patients with severe IBD and long-term biological treatment was higher compared to the general Czech population, despite the favorable vaccination coverage of this high-risk patients’ group. A higher risk was associated with tumor necrosis factor inhibitor therapy. Fojtík","doi":"10.48095/ccgh202321","DOIUrl":"https://doi.org/10.48095/ccgh202321","url":null,"abstract":"Summary: Signet ring cell carcinoma (SRCC) is a rare type of highly malignant adenocarcinoma that produces mucin and predominantly affects the stomach, ovaries, and the small intestine. Primary localisation in the colon is rare, and the prognosis of this type of adenocarcinoma is very poor. We present a case report of a 44-year-old woman who suffered from Crohn‘s disease beginning in the year 2000. During the last three years of her treatment, she was treated using biological therapy with ustekinumab. The treatment led to clinical, laboratory, and endoscopic remission. In February 2021, she had widely metastatic primary signet ring cell carcinoma of the colon, which mimicked the IBD activity in the case of her colon obstruction. Because of the extensive infiltration of her liver and peritoneum at the time of the diagnosis, in addition to the symptoms of the passage disorders she was experiencing, she proceeded with an end ileostomy and palliative treatment was started. The patient‘s prognosis was unfavourable, and she died within 5 months of the diagnosis in hospice care. Among them, there are gastrointestinal complications, which have a significant negative impact on their quality of life. Objective: The purpose of this study was to evaluate the impact of the period since the incidence of the injury, level of spinal cord injury and Body Mass Index on the level of bowel dysfunction and on the quality of life of chronic patients. Method: Proceeding from a questionnaire-based survey, 42 adults with complete and incomplete spinal cord injuries completed the standardized questionnaire Neurogenic bowel dysfunction score (NBD Score), the standardized questionnaire Irritable Bowel Syndrome – Quality of Life survey (IBS-QOL) and non-standardized Czech version questionnaire Burwood Questionnaire. Results: A statistically significant positive linear correlation was evaluated between the period of time since the incidence of the injury and NBD score. The Pearson correlation coefficient was 0.6 (P = 0.02). However, there was no statistically significant linear correlation between the period of time since the incidence of the injury and value of IBS-QOL (–0.47; P = 0.47). Neither was there any statistically significant positive linear correlation with the variables the level of spinal cord injury and Body Mass Index. What most decrease the quality of life of disabled patients with gastrointestinal complications are concerns for their own health, depressive feelings, and diet routines. Conclusion: All medical staff should not neglect the need of diagnostic and therapy of the gastrointestinal problems in the aging subpopulation of people with spinal cord injury. Based on the patient’s medical history, laboratory data, imaging methods and liver biopsy, we diagnosed autoimmune hepatitis. The patient developed acute liver failure, and his liver function did not improve after corticosteroid administration. Therefore, the patient was enrolled in the waiting list and underwent a successful orthotopic liver transplantation. The patient was initially diagnosed with ulcerative colitis and underwent proctocolectomy with ileopouch-anal anastomosis. One year after oostomy closure she got pregnant and, at the same time, she developed perianal abscess and fistula. The diagnosis was thus changed to Crohn’s disease. During pregnancy, she underwent repeated perianal fistula drainages, surgical revision of ileus and treatment with ustekinumab was started. The patient delivered a healthy boy in November 2022 at 38th gestational week.
