{"title":"Formulation and in-vitro Evaluation of Oro-dispersible tablets of Indomethacin","authors":"Gopal Pokhrel, Ganga Kunwar, Jun Devi Rai, Sheela Thapa, Sudip Dhakal, Prashant Basnet","doi":"10.3126/njhs.v2i1.47172","DOIUrl":"https://doi.org/10.3126/njhs.v2i1.47172","url":null,"abstract":"Introduction: Oro-dispersible tablets are rapidly dissolved in saliva without the need for water and are beneficial for renal impaired, bedridden and psychiatric patients.\u0000Objective: The study aimed to formulate oro-dispersible tablets of indomethacin with reduced adverse effects, better patient compliance, faster action, and convenience for patients.\u0000Methods: Oro-dispersible tablets of indomethacin were prepared using three different super disintegrants; crospovidone, croscarmellose sodium and sodium starch glycolate with three different concentrations (2.5%, 5.2%, and 7.7%) by direct compression method. The prepared tablets were evaluated for pre and post-compression parameters including bulk density, tapped density, compressibility index, angle of repose, Hausner's ratio, hardness, friability, wetting time, in vitro disintegration time, and in vitro drug release.\u0000Results: The percentage of drug released in 5 minutes of all formulations of Oro-dispersible tablets of Indomethacin was found to be 74.36% to 80.16% and the percentage of drug released in 10 minutes was 96.18% to 100%. All formulations showed disintegration time in the range of 19-78 seconds. The tablets prepared with 7.7% crospovidone (F6) shows faster disintegration (19 seconds) as compared to tablets prepared with sodium starch glycolate and croscarmellose sodium. Depression among the elderly leads to various physical and psychological problems and consequently poor quality of life.\u0000Objective: This study was aimed at assessing the prevalence of depression among elderly residing in old-age homes in Devghat, Tanahun, Nepal.\u0000Methods: A cross-sectional analytical study design was adopted to assess depression among elderly residing in old-age homes of Devghat town in Tanahun district of Nepal. Non-probability enumeration sampling technique was used to collect the data. The validated Nepali translation of the Geriatric Depression Scale-15 was used to assess depression. Collected data were entered in SPSS version 22 and analyzed by using the Chi-square test.\u0000Results: Out of 155 respondents, 45.8% had mild depression followed by 36.8% who had moderate depression. A statistically significant association was found between the level of depression and marital status, educational status, asthma, and habit of smoking.\u0000Conclusions: This study showed that the prevalence of depression in old-age homes was very high. Dacryocystorhinostomy (DCR) is the treatment of choice and has a high success rate.\u0000Objective: To analyze the success rate of external DCR surgery and its influencing factors at our hospital.\u0000Methods: It is a hospital-based descriptive study of cases that underwent external DCR surgery from June 2016 to February 2017 at Biratnagar eye hospital. The demographic data were recorded and the successful outcome of the surgery along with its influencing factors -tube status, operating surgeons, and gender was analyzed. The main outcome measure was defined as a patent lacrimal passage on syringing at 6 months postoperatively. Chi-square and Fisher’s exact test were used for statistical analysis.\u0000Results: In total 317 patients, the mean age of the patients was 38.46 (SD ± 13.6) years. The male to female ratio was 1:2.5. The commonest presenting symptom was watering with discharge in 224 (70.6%) patients. Of the total, 195(61.5%) had undergone the DCR surgery with intubation and 122 (38.5%) without intubation. Only 114 (35.9%) patients were followed up for 6 months. The success rate was 86.8%. Postoperative wound infection, bleeding or cheese wiring of the punctum was not seen. Fasting blood samples of 5 ml were collected from both the study sample and control sample. Biochemical parameters like glycated hemoglobin, serum total cholesterol, serum triglyceride, serum high-density lipoprotein cholesterol, and serum low-density lipoprotein cholesterol were analyzed with the help of a semi-automated analyzer, Erba Chem5, and Colorimeter-Systronic. Means of glycated hemoglobin, total cholesterol, triglyceride, high-density lipoprotein, and low-density lipoprotein were estimatedamong the cases and controls. The student`s t-test was used to compare the mean values.\u0000Results: A statistically significant difference was observed in mean glycated hemoglobin, total cholesterol, triglyceride, high density lipoprotein and low-density lipoprotein among the cases and controls (p-value<0.05).\u0000Conclusions: This study concluded that there was a significant difference in mean glycated hemoglobin, total cholesterol, triglyceride, high-density cholesterol, and low-density cholesterol among the cases and controls. The clinical evaluation alone is insufficient for proper diagnosis in most cases. So, histopathological examination is the gold standard method for diagnosis and management of patients accordingly.\u0000Objective: The present study was done to evaluate the histopathological spectrum of oral cavity lesions and compare them in relation to age, sex, site, clinical features, risk factors, and clinical diagnoses.\u0000Methods: This prospective cross-sectional study enrolled 127 cases of oral biopsies which were received at the Department of Pathology, Tribhuvan University and Teaching Hospital, Kathmandu Nepal, from May 2018 to April 2019 for histopathological examination. Specimens were fixed in 10% formalin and subjected for tissue processing and Hematoxylin and Eosin stained sections. Data entry and analysis were done by using SPSS 24 version where frequency and percentile were calculated.\u0000Results: Total cases were 127 with slight female predilection and the age group of 50-60 years (mean age of 44.24 years) were commonly affected. The tongue being the most common site, frequently lesions presented as swelling. Most of the lesions were non-neoplastic comprising 45% whereas malignant lesions comprised 23.6%. Smoking increased the risk of malignancy by 2 fold. The most common benign lesions were squamous papilloma & fibroepithelial polyp whereas the malignant lesion was squamous cell carcinoma. Sixty percent of clinical diagnoses didn’t show correlation.\u0000Conclusions: Oral cavity lesions have a wide spectrum of distribution in age, sex, site, and clinical presentation. Initially, oral lesions may present with subtle symptoms which may cause underdiagnosis. Nepal is in the early stages of adopting One Health (OH), but it is continuously expanding. Various initiatives have been taken by the Government of Nepal (GoN) to combat problems such as antimicrobial resistance, highly pathogenic avian influenza and rabies using an integrated health approach, but several challenges remain in their implementation. A few of the major obstacles to implementing OH in Nepal include weak organizational structure to support the initiative, no legal framework required to implement OH, insufficient technical expertise, lack of intersectoral data sharing mechanisms, a limited budget, and poor legislative understanding. To address these gaps and prioritize the health problems where OH will lead to the best outcome, we encourage GoN to address the gaps. To control the ongoing COVID-19 pandemic, the rational application of the One Health strategy should be our utmost priority. Histopathological observation was suggestive of cutaneous tuberculosis, lupus vulgaris, cutaneous leishmaniasis due to overlapping findings during biopsy. The case was diagnosed by the pharmacological intervention observing the effect of drugs i.e., terbinafine. We present a case of a 45-year-old male with a known case of diabetes for five years. He presented with high-grade fever, dysuria, and abdominal pain. Routine tests, ultrasonography along together with high-resolution computed tomography was done. High Resolution Computed Tomography showed subtle ground-glass opacity of the superior segment of lingulae of the left lobe of the lungs. Real time polymerase chain reaction was advised to rule out Covid-19 infection, which was negative. A validated self-administered questionnaire, Likert scale and Observational checklist were used for data collection. The simple descriptive statistics (percentage, mean) and inferential statistics (Chi-square test) were used for the data analysis. \u0000Results: The findings of the study showed that the mean age of the respondents was 26.54 years. Half of the total respondents 70 (50.7%) had adequate knowledge, half 69 (50%) had positive attitude and more than half of the respondents had poor practice 60 (60%) regarding the use of PPE during COVID-19 pandemic. There was significant association of knowledge regarding the use of PPE with training, continue education, profession and attitude at p<0.05 level of significance. \u0000Conclusion: This study concludes that the periodic training program for newly recruited HCWs and refresher training/ continue education for old HCWs to update about the proper use of PPE would enhance the knowledge, attitude and practice of the HCWs.
