Previous and current chemotherapy, radiation therapy, and anti-cancer therapies need to be considered when developing a management strategy. Here we review strategies for reviewing patient medical histories, the incorporation trichoscopic examinations to aid with diagnosis, and special treatment considerations that do not increase risk of cancer reoccurrence. As novel anti-cancer therapeutics continue to be approved, it is imperative that dermatologists serve in support networks of patients experiencing dermatologic side effects. This article aims to aid dermatologists in the safe and effective treatment of hair-related side effects that negatively impact the survivorship of patients with cancer. In rare cases, PNGD has been attributed to systemic medications. Here we report one case of a cutaneous injection-site reaction that occurred after intradermal administration of an experimental Wilms Tumor 1 (WT1) protein-derived peptide vaccine in a woman being treated for recurrent ovarian cancer. This is the first reported case of a PNGD-like reaction associated with immunotherapy injections. In this article, we discuss this case and summarize the literature related to the diagnosis and management of PNGD, as well as other injection site reactions related to immunotherapies and experimental vaccines.","PeriodicalId":376900,"journal":{"name":"Academic Dermatology","volume":"54 4","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-04-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140729389","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A number of benign conditions may mimic the cutaneous and radiographic features of IBC. We present a case of a patient with hidradenitis suppurativa of the breast complicated by abscess formation with cutaneous findings mimicking IBC, including abrupt erythema, edema, nipple retraction, and peau d’orange changes. This case highlights dermatologic findings that should prompt an urgent evaluation for IBC, features that differentiate clinical mimickers from IBC, and potential pitfalls in diagnosing IBC if an improper or incomplete workup is pursued. We also present a review of the literature regarding mimickers of IBC.","PeriodicalId":376900,"journal":{"name":"Academic Dermatology","volume":"127 S193","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-04-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140731534","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

While on a trial of loading dose dupilumab, a 25-year-old white male with a past medical history significant for anaphylaxis after ingesting peanuts and tree nuts did not experience anaphylaxis after accidentally consuming almonds. Similarly to anti-IgE, the large-scale inhibition of IL-4 and IL-13 signaling to various immune cells involved with type II hypersensitivity reactions using loading dose dupilumab may have played a role in preventing anaphylaxis in this patient. The role of loading dose dupilumab as a potential prophylactic agent against food-induced anaphylaxis could be explored cautiously as a novel way to reduce both morbidity and mortality in patients with these allergies.","PeriodicalId":376900,"journal":{"name":"Academic Dermatology","volume":"183 S486","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-04-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140731140","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose of this study was to validate primary discharge diagnoses for the ICD code rash or other nonspecific eruptions by finding the positive predictive value (PPV) and to determine how often it was used to indicate an eruption that was eventually determined to be a drug eruption. Two dermatologists independently examined each of 39 hospital patient encounters to assess the validity of the ICD code used by stating whether the code used was appropriate or inappropriate, per table 1 criteria. Since in most patients, a drug eruption was suspected, the Naranjo Adverse Drug Reaction Probability Scale was utilized to externally validate our findings. Our results yielded a PPV for nonspecific rashes of 94.87% (83.11-98.58%) for appropriateness of use. The mean Naranjo score for appropriately versus inappropriately coded cases was 5.7 versus 2.5, respectively, with a mean difference of 3.17568 (0.18833-6.16302), (pooled P-value = 0.0378). Out of the cases analyzed, 29/39 or 74% were confirmed to be drug-related. Our high PPV supports the validity of non-specific eruptions found in national patient databases and the high Naranjo criteria suggest that often, these eruptions are drug related, yet not confirmed at the point of discharge.","PeriodicalId":376900,"journal":{"name":"Academic Dermatology","volume":"215 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-07-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124244333","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Despite patient interest in non-pharmacologic methods to treat psoriasis and manage overall health, there is insufficient data available to guide dietary counseling. OBJECTIVE This study aims to identify the role of a popular anti-inflammatory diet known as intermittent fasting (IF) in psoriasis and psoriatic arthritis (PsA). METHODS A single-blind parallel group randomized control trial will be performed in 60 patients who have been diagnosed with psoriasis or PsA. Patients will be assigned 1:1 to either the IF group or control group. Patients assigned to the IF group will follow a 16:8 fasting IF method for the first 12 weeks, followed by the resumption of their routine diet. Patients assigned to the control group will follow their routine diet for 24 weeks. RESULTS A total of 24 patients have been enrolled in our study, with a final enrollment goal of 60 patients. The final visit is foreseen for July 2023. CONCLUSIONS This study aims to identify the role of a popular anti-inflammatory diet known as intermittent fasting (IF) in the management of psoriasis and PsA. Publishing this data will allow all dermatologists and the National Psoriasis Foundation to provide a consistent, evidence-based recommendation for IF as a potential non-pharmacologic intervention for patients with psoriasis and psoriatic arthritis (PsA).","PeriodicalId":376900,"journal":{"name":"Academic Dermatology","volume":"67 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-07-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124818529","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Using a survey design, we approached patients with psoriasis, rosacea, and hidradenitis suppurativa to evaluate whether the patients had received counseling on drug use. In our sample, half of the patients with hidradenitis suppurativa were active smokers, and most had received smoking cessation counseling. Patients with psoriasis were also aware of an association with smoking yet not at the same rates as the hidradenitis suppurativa population. Future research is needed to corroborate this data in a larger sample, and to establish improved means to support lifestyle changes and interventions for dermatologic patients.","PeriodicalId":376900,"journal":{"name":"Academic Dermatology","volume":"74 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-07-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126887590","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
