Detecting FBs embedded in the tongue's muscle layer is challenging using visual inspection alone. Although preoperative computed tomography (CT) is effective for detecting FBs, two-dimensional images often fail to provide a clear understanding of the shape, position, and angle of the FB. Thus, we used preoperative three-dimensional CT (3D-CT) combined with intraoperative ultrasound to significantly enhance detection and localization.</div><div>A 50-year-old man presented with pain on the right edge of his tongue after eating fried fish. Although palpation revealed a tender area, no FB was detected initially. Plane CT showed a hyperabsorptive area in the tongue, whereas 3D-CT revealed a 24-mm curved fish bone located approximately 10 mm beneath the surface. Preoperative 3D imaging and intraoperative ultrasound before emergency excision played a critical role in confirming the precise location of the FB. The fish bone was successfully removed, matching the predictions made by the 3D-CT, and the patient experienced no postoperative complications.</div></div>","PeriodicalId":37154,"journal":{"name":"Otolaryngology Case Reports","volume":"34 ","pages":"Article 100646"},"PeriodicalIF":0.2,"publicationDate":"2025-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"143183348","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Here we present a case report of an otherwise healthy 39-year-old female who presented with significant sinus pain, nasal congestion, anosmia, rhinorrhea, throat discomfort and odynophagia five months after starting intranasal abuse of 10 tablets of acetaminophen daily. Anterior rhinoscopy revealed a large and inflamed anterior septal perforation with exposed cartilage. Significant debris and acetaminophen tablet residue was present is both nasal cavities. With Suboxone (buprenorphine/naloxone) therapy, she succeeded in cessation of drug use. Due to her ongoing complaints of nasal obstruction, not resolving with medical management, surgery was arranged. This is the first reported case of intranasal acetaminophen abuse in Canada and the first documented case where suboxone was used to help taper intranasal acetaminophen use prior to surgical intervention.</div></div>","PeriodicalId":37154,"journal":{"name":"Otolaryngology Case Reports","volume":"34 ","pages":"Article 100640"},"PeriodicalIF":0.2,"publicationDate":"2025-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"143183338","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

There are few case reports of this tumor developing in the nasal sinuses. An 11-year-old male presented to the ED for epistaxis secondary to minor trauma. The patient denied any history of pain, prior epistaxis, anosmia, or double vision, but endorsed a several year history of nasal obstruction. CT imaging was notable for a nasal mass. He was scheduled for surgical resection of the mass and underwent pre-operative embolization due to concern for intra-operative blood loss. He underwent endoscopic resection and did well post-operatively. Pathology of the tumor showed hypercellular cartilaginous matrix with myxoid stroma. Genetic testing showed loss of expression of the PRKAR1A in the tumor, suggesting the diagnosis of Osteochondromyxoma. He was subsequently referred for genetic evaluation for Carney Complex. By presenting this case report, we add to the limited body of literature on this rare disease entity. Osteochondromyxoma should be kept on the differential for a cartilaginous nasal mass. Treatment is endoscopic resection, and surveillance is necessary to reduce morbidity. It is important to recognize the association between Osteochondromyxoma and Carney Complex.</div></div>","PeriodicalId":37154,"journal":{"name":"Otolaryngology Case Reports","volume":"34 ","pages":"Article 100644"},"PeriodicalIF":0.2,"publicationDate":"2025-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"143183336","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Among these, extramedullary plasmacytoma is a rare tumor of plasma cell primary to bone marrow hematopoietic tissue, accounting for approximately 4 % of all plasmacytomas, and exceptionally rare in the thyroid gland.
