Mirghafourvand","doi":"10.2174/1573404818666220527095739","DOIUrl":"https://doi.org/10.2174/1573404818666220527095739","url":null,"abstract":"\u0000\u0000Reflexology is a non-invasive, inexpensive, and simple technique that can be employed for pregnant women.\u0000\u0000\u0000\u0000This study aimed to determine the effects of reflexology on anxiety during labor (primary outcome) and its side effects (secondary outcome).\u0000\u0000\u0000\u0000In this systematic review, the Cochrane Library, PubMed, Google Scholar, Scopus, Web of Science, SID, Science Direct, and ProQuest databases were searched systematically and without time constraints until 20 April 2021. The quality of the selected articles was assessed using Cochrane Handbook and the meta-analysis was performed utilizing RevMan 5.3. Heterogeneity was evaluated by the I2 statistic and, as there was heterogeneity between the studies, subgroup analysis was carried out based on the control group. Since the anxiety assessment tools in the included studies were identical, the mean difference (MD) was reported. The quality of the evidence was assessed using the GRADE approach.\u0000\u0000\u0000\u0000Six studies that included 643 pregnant women were investigated in this systematic review. The results of the meta-analysis using five of the studies revealed that reflexology significantly lowered the mean scores for maternal anxiety compared to routine care or simple massage (MD: -11.34; 95% CI: -16.28 to -6.40). None of the studies reported any specific side effects after reflexology treatment.\u0000\u0000\u0000\u0000Reflexology can decrease maternal anxiety during labor. Participants were randomly assigned to the intervention (doula accompaniment) and control groups through block randomization using Random Allocation Software with a block size of 4 and 6. The data were collected using the demographic and obstetric characteristics, Support and Control in Birth Questionnaire, Labor Agentry Scale and the information in the labor progress record. The data were analyzed using SPSS 24. Independent t-test, and ANCOVA were used.\u0000\u0000\u0000\u0000The mean score of support and control in birth was 121. 14.55 in the intervention group and 96.44 ± 15.8 in the control group (p <0.001). Women’s subjective perception of QoL is clinically relevant for assessing the effectiveness of interventions and identifying susceptibility to depression.\u0000\u0000\u0000\u0000This review aims to identify, present, and critically appraise the assessment tools of QoL during pregnancy.\u0000\u0000\u0000\u0000A narrative review of the English-language literature was conducted, through the PubMed, CENTRAL, Scopus, and Google Scholar electronic databases.\u0000\u0000\u0000\u0000Several generic instruments, pregnancy-specific and disease-specific questionnaires concerning common diseases during pregnancy have been proposed. Plasma, salivary and hair cortisol, inflammatory, genetic and epigenetic biomarkers and gut microbiome have also been investigated as potential indicators of maternal anxiety. Their use can quantify the association between pregnancy-specific anxiety and QoL. To sidestep the major intimidations of low blood volume and low blood pressure, the diverse actions of Ang II/ACE/AT1R can be viewed as a useful response in maintaining homeostasis. The deleterious action of Ang II/ACE/AT1R axis is an endogenously counterbalanced by ACE 2/Ang 1–7/MasR. Yet, over activation of the Ang II/ACE/AT1R axis may lead to hypertension. Preeclampsia is characterized by hypertension with proteinuria or end-organ dysfunction after 20 weeks of gestation. The early-onset sort is more genuine and is capable for tall rates of maternal and fetal dismalness and mortality than late-onset sort of preeclampsia. Various as the theories for the pathogenesis of preeclampsia are, the exact underlying molecular mechanisms remain unclear but are likely to be multifactorial. Later examinations of RAS in preeclampsia have highlighted require for a comprehensive survey of this subject. Maternal-fetal morbidity and death due to CS is a serious public health problem worldwide. Our study aimed to assess the impact of multiple cesarean sections (CSs) on mother-fetal morbidity.\u0000\u0000\u0000\u0000This cross-sectional study was performed on 165 women who underwent multiple repeated CSs in our clinic and met the criteria of inclusion by evaluating their records. All women were divided into 3 groups according to the number of CSs: 2nd (n = 111), 3rd (n = 44), and 4th/5th (n = 10). Maternal-neonatal outcomes; blood transfusion needs, adhesions, APGAR scores, and respiratory difficulties were investigated retrospectively.\u0000\u0000\u0000\u0000Our results revealed that adhesion (60.6 %) was the most frequent maternal complication. The prevalence of thick adhesions decreased with the number of cesarean sections performed (70, 25, and 5 cases for the previous 2nd, 3rd, and 4 or more CS, respectively). There was no statistically significant difference between the groups in terms of adhesions, uterine rupture, bladder injury, endometritis, and wound infection. NICU admission (14.5 %), Low birth weight (3.6 %), and IUGR (3.03 %) were among the unfavorable fetal outcomes. There was a significant difference among the three studied groups regarding APGAR score, maternal age, and hospital stay, while a non-significance was found in birth weight and maternal adhesion.\u0000\u0000\u0000\u0000Our findings support our hypothesis that the likelihood of maternal complications rises as the number of CSs rises. The manner and time of delivery have a greater impact on infant outcome than the number of CSs. There is no clear absolute barrier for the number of CSs, however, 4 or more cesarean births were recognized as the essential level for the majority of outcomes. The case group included women with unwanted pregnancies and the control group consisted of women who became pregnant in a preplanned manner. Data were collected from the samples using a checklist. Data were analyzed with SPSS24 at a significance level of 5%.\u0000\u0000\u0000\u0000This case-control study was conducted on 455 mothers in the case group (unwanted pregnancy) and 456 samples in the control group. Hypertensive disorders of pregnancy constitute one of the leading causes of maternal and perinatal mortality worldwide. The incidence of Preeclampsia in Lebanon was estimated at 2.84%.This study aimed primarily to evaluate the knowledge and practices of Lebanese obstetricians, and secondary to identify the factors associated with these scales.\u0000\u0000\u0000\u0000Across-sectional study was conducted between September and December 2020 in obstetricians’ clinics of different healthcare institutions.\u0000\u0000\u0000\u0000Data was collected using a questionnaire based on the recommendations of the National Institute for Health and Care Excellence (NICE) guidelines and American College of Obstetricians and Gynecologists (ACOG) preeclampsia 2020 guidelines. In private clinics, the investigator visited the medical doctor and each doctor agreed to participate in the research, signed the consent form, and filled the distributed questionnaire on his own without any intervention from the investigators. In the hospitals, the questionnaires were distributed to the Obstetrics and Gynecology Department.\u0000\u0000\u0000\u0000In this study, 95 doctors were recruited. The practice score of more than half of the 95 doctors (54.7%) is low. The mean of their knowledge score (14.5) is higher than the mean of their practice score (8.5). Almost all doctors have a high knowledge score (96.8%), but more than half of them have poor practice scores (54.7%). A 'in-ear regression showed that the knowledge score decreased for doctors who live in Bekaa. A second linear regression showed that the practice score decreased for doctors who live in Bekaa (p=0.001). However, training on preeclampsia (p<0.001) increased the practice score. \u0000A third linear regression showed that the total score decreased for doctors who areliving in Bekaa (p<0.001). However, having a board certificate (p=0.03) and training on preeclampsia (p=0.008) increased the total score.\u0000\u0000\u0000\u0000Different continuous medical education activities should be available widely and mandatory for all working doctors to improve their knowledge and practice towards preeclampsia. Mirghafourvand","doi":"10.2174/1573404818666220518154900","DOIUrl":"https://doi.org/10.2174/1573404818666220518154900","url":null,"abstract":"\u0000\u0000Fatigue is a common problem in pregnancy and is associated with negative outcomes.\u0000\u0000\u0000\u0000This study aimed to determine the predictors of fatigue in pregnant women.\u0000\u0000\u0000\u0000This cross-sectional study was conducted on 150 pregnant women with a gestational age of 12-18 weeks visiting the healthcare centers in Mahabad-Iran, 2021. Fatigue was assessed via the modified Multidimensional Assessment of Fatigue (MAF) checklist, depression via the Edinburgh Postnatal Depression Scale (EPDS), stress and anxiety via Depression, Anxiety and Stress Scale-21 (DASS-21), well-being via WHO-5, and positive and negative affect via International Positive and Negative Affect Schedule-Short Form (I-PANAS-SF). The general linear model was used to estimate the degree of the effect of each independent variable (sociodemographic characteristics, depression, stress, anxiety, positive and negative affect, and well-being) on the dependent variable (early pregnancy fatigue).\u0000\u0000\u0000\u0000The mean (SD) fatigue score was 21.91 (7.07) within the 0-50 possible range. The majority of pregnant women (87.3%) suffered from fatigue. Based on the Pearson or Spearman correlation analysis, a significant correlation was observed between fatigue and depression (r= 0.334, P<0.001), anxiety (r= 0.327, P<0.001), stress (r=0.329, P< 0.001), well-being (r= -0.279, P<0.001), and negative affect (r= 0.308, P<0.001). Based on the adjusted general linear model, the education, stress and negative affect variables were predictors of fatigue.\u0000\u0000\u0000\u0000The education, stress and negative affect predict fatigue in pregnant women, and their identification contributes to designation of interventions to mitigate fatigue in this group.
