{"title":"Masas cardiacas: diagnóstico, pronóstico y tratamiento","authors":"","doi":"10.1016/j.rccl.2024.04.002","DOIUrl":"10.1016/j.rccl.2024.04.002","url":null,"abstract":"<div><p>Cardiac masses encompass a heterogeneous set of pathologies (benign tumors, malignant neoplasms and other entities such as thrombus or vegetations) with a diverse presentation (from asymptomatic cases in which the mass is discovered incidentally to limiting symptoms). An accurate diagnosis is essential in order to guide an individualized therapeutic plan. Evaluation based on multimodal imaging techniques plays a key role in this aspect as it allows a non-invasive and detailed study of each lesion, avoiding the need for a biopsy in many cases. Treatment of cardiac masses is individualized and depends on their nature, clinical impact and patient characteristics. The prognosis of non-tumoral pathology and benign neoplasms is favorable in the majority of patients. Their individual estimate would be useful to carry out the corresponding invasive procedures.</p></div><div><h3>Methods</h3><p>This is an observational, retrospective and unicentral study. Cases diagnosed with tachycardiomyopathy were collected between September 2015 and January 2023. The sample was split into 2 study groups: the first one was used for the construction of the linear regression model by performing a multivariate analysis based on evaluating all possible equations (sample 1, 70%); whereas the second one was intended for the validation of the model (sample 2, 30%).</p></div><div><h3>Results</h3><p>A total of 134 patients were gathered and left ventricular ejection fraction (LVEF) at recovery (the difference between final and original LVEF) was calculated in all of them. Within sample 1, setting parameters of 16,383 models were estimated. The model selected, based on Mallows’ Cp index, was the composed of the following variables: sex, arterial hypertension (HT), LVEF at diagnosis, achievement of rhythm control, and ablation. The selected model explains about half of the individual variability of the recovered LVEF (R<sup>2</sup> 0.493). Afterwards, individual LVEF at recovery predicted by the model was compared with the observed LVEF in sample 2. No overall significant differences were observed in the R<sup>2</sup> coefficient of determination.</p></div><div><h3>Conclusions</h3><p>Sex, arterial hypertension, LVEF at diagnosis, achievement of rhythm control, and ablation were considered as predictors of recovered LVEF in patients with tachycardiomyopathy. We considered ignorance of the lipid profile when there was no previous diagnosis of dyslipidemia, no lipid-lowering treatment was followed, nor was there laboratory determination of LDL-C in the last 5 years.</p></div><div><h3>Results</h3><p>A total of 3122 patients were included, aged 67.1 (13.2) years, 73.4% men and 54% admitted for ST-segment elevation acute coronary syndrome (STEACS); 1325 (42.44%) did not know their cholesterol levels and the associated variables were not having high blood pressure or diabetes, age < 55 years, male sex, low burden of comorbidity and presentation as STEACS. A significant interaction was detected between age and knowledge of dyslipidemia (<em>P</em> = .02). Hospital mortality was 6.5% (201 patients) and was the same in patients who knew or did not know their cholesterol levels; However, it was higher in those who did not know their LDL-C and were < 65 years of age (3.8% vs 1.5%; <em>P</em> < .01). The objective of this study was to analyze the perception of Cardiology resident doctors in Spain regarding the acquisition of competencies in cardiovascular pathology during their university training, and its usefulness in daily clinical practice.</p></div><div><h3>Methods</h3><p>A telematic questionnaire was developed targeting 814 cardiology residents in Spain between January and February 2023. The acquisition of competencies during the medicine degree and the implementation of teaching innovations in the subject of cardiovascular diseases were evaluated.</p></div><div><h3>Results</h3><p>The survey was completed by 69 (8.5%) cardiology residents (mean age of 29, 43% women). The competencies that a larger number of respondents considered acquired during their university training were knowledge of cardiovascular risk factors (91.2%), while the interpretation of major diagnostic tests was considered acquired by a smaller number of respondents (27.9%). The majority of participants stated that university training was only useful for passing the subject. The most commonly implemented teaching innovations were clinical practices (96.2%), and their implementation received positive evaluations from the majority of participants.</p></div><div><h3>Conclusions</h3><p>Cardiology residents participating in the survey reflected that university education in cardiovascular pathology in Spain primarily emphasizes the acquisition of theoretical knowledge.
