{"title":"Preserving Trabecular Bone Structure in Gross Specimens with Paraffin Wax for the Study of Osteoporosis.","authors":"Marc A Pizzimenti, Xiaoliu Zhang, Jessica E Goetz, Punam K Saha","doi":"10.56507/brev5387","DOIUrl":"10.56507/brev5387","url":null,"abstract":"<p><p>Analytical strategies to interpret computed tomography data for the study of osteoporosis require gross anatomical specimens that closely approximate the bone marrow matrix while maintaining the structural integrity of the trabecular network. Here we investigated if paraffin, gelatin-matrix, or silicone polymer impregnated bone specimens maintained the trabecular network and bone density of specimens used for comparative studies in osteoporosis. Distal tibia specimens (n = 12) from cadaveric human legs were used for this study. Multi-row detector computed tomography (MDCT) scans were completed on each specimen, with selected specimens re-scanned using micro-computed tomography (μCT), at different times to evaluate trabecular integrity. Once scanned, bones were de-marrowed using a solvent-based procedure. De-marrowed specimens were then impregnated, under vacuum controlled conditions, with either a silicone polymer, gelatin matrix solution, or paraffin wax, and re-scanned. μCT analysis demonstrated a high Pearson correlation coefficient in trabecular bone network area density between the native and de-marrowed states (r = 0.99). Quantitatively, paraffin impregnated specimens demonstrated the highest congruence in MCDT bone volume fraction (r = 0.92) and trabecular network area (r = 0.94) measures. These results were more robust in the μCT data (bone volume fraction, r = 0.97; and trabecular network area, r = 0.99). Additionally, of the impregnating procedures, paraffin wax provided the best qualitative specimens for handling, storage, and processing time. These data suggest that paraffin demonstrates a space occupying matrix that effectively protects the trabecular network and maintains the ability to quantify important biomechanical measures for assessing bone integrity. Anatomy education is heavily reliant on visuotactile experience and presents a challenge during online instruction. In an effort to provide an effective experience, the Head and Neck block (Fall Semester, 2020) was presented as an online activity that included extended reality anatomy models derived from three-dimensional medical illustrations (artistic), dissections, plastinates, and segmentations, and posted on the Sketchfab platform for student viewing. The purpose of this study was to assess student preference of anatomical models during online anatomy instruction. A photogrammetry workflow was developed to digitize the dissected and plastinated specimens that were posted to the sketchfab.com platform and presented via a secured university-based website hosting service (xrcore.jabsom.hawaii.edu). Segmented models were derived from MRI cadaveric scans, and artistic models were created based on segmented graphics primitives that are defined as nondivisible graphical elements, such as planes or spheres, for input or output within a computer-graphics system. Technical requirements were minimal and relied on several open-source or limited subscription software packages. Accession was recorded and compared using a chi-square analysis. A comparison of the preference of the models was conducted using student surveys (n = 79). When compared to all learning resources, actual dissections were most preferred (34.1%). However, plastinated models were considered most/more preferred (54.3%) compared to other assets suggesting a broader preference as a learning resource. These results suggest that plastinated models are effective and engaging tools for the instruction of gross anatomy for medical students. © 2021, International Society for Plastination. Pirogov has been entombed in the Church of St. Nicolas in Vinnytsia, Ukraine, since 1881, when it was embalmed by Professor Vyvodtsev. During the re-embalming of the body of N.I. Pirogov in 2018, microbiological swabs from the body and its surroundings were taken and, among other things, skin, subcutaneous tissue, and muscle and bone tissues were taken for ultramicroscopic examination. Hyphae of fungi of the genus Penicillium citrinum were found at two sites during this examination.
