Approximately 50 % of cases of infertility in couples are attributable to male factor. One of the most common causes of male infertility is varicose dilation of pampiniform plexus veins in spermatic cord, or varicocele. However, the fact that only in 10-45 % of cases microsurgical subinguinal varicocelectomy leads to a spontaneous pregnancy in a couple urges further search for a diagnostic marker which would be able to predict the success of surgical treatment of varicocele preoperatively. Aim . To study the features of lipid profile of seminal plasma in patients with varicocele and to evaluate the possibility of using lipidomic analysis of seminal plasma for patient selection prior to surgical treatment for varicocele. Materials and methods . Fifty three patients from infertile couples with varicocele and altered seminal parameters aged 18 to 44 years underwent microsurgical varicocelectomy as described by Marmar. All patients were treated in one department. The control group included 24 healthy normozoospermic men without varicocele who had proven fertility. The material for the study was the seminal plasma of men, lipid composition of which was determined by mass spectrometry. Results . No significant differences were found in lipid profiles of seminal plasma from men in control group and varicocele group, as well as in lipid profiles of seminal plasma obtained before and after surgery in same patients. Comparison of lipid profiles of seminal plasma provided by patients before microsurgical varicocelectomy, who had clinical effect ( n = 15) and who did not have clinical effect ( n = 16), revealed differences in level of HexCer-NDS d20:0/18:0 in the positive ion test and in levels of 8 other lipids in the negative ion test. These lipids are related to the oxidation of phospholipids, cardiolipins and phosphatidylethanolamines. Conclusion . According to the contemporary data available contrast-enhanced computed tomography can be used to assess total and split kidney function. The Urology Clinic of the I.M. Sechenov First Moscow State Medical University, Ministry of Health of Russia (Sechenov University) utilizes mathematical processing of computed tomography data for the assessment of split kidney function. Aim . To evaluate the effect of main renal artery clamping (warm ischemia time) during partial nephrectomy for a unilateral tumor on the kidney function. Materials and methods . 33 patients of both sexes who underwent partial nephrectomy for unilateral kidney tumor were enrolled in the study. Prior to and 3–6 months after the surgery, 3D glomerular filtration rate (GFR), 3D perfusion, 3D volume of the renal parenchyma were evaluated by mathematical processing of computed tomography data with contrast enhancement. As well as that the dynamics of the total GFR (CKD-EPI equation) and total volume of the kidneys were assessed. Results . No significant changes were registered for both general functional renal parameters (creatinine level, total GFR, total volume of functioning renal parenchyma) and split 3D GFR. There were significant differences in 3D perfusion and kidney parenchyma volume prior and after partial nephrectomy. Conclusion . The results of the research highlight good reserve capacity the kidneys after partial nephrectomy with main renal artery clamping. The evidence that the non-operated kidney takes over the function of the resected kidney is based on the fact that there is an increase in its perfusion and volume of the kidney in the postoperative period. Materials and methods . Thirty patients with DM were examined (15 patients with type 1 and 15 patients with type 2 respectively). Patients with DM had ED mainly of severe form (IIEF-5 from 6 to 16 points). According to the results of conducted examination the patients were prescribed the course of conservative therapy including hosphodiesterase in hibitors of type V, anticholinesterase drugs, drugs of thioctic acid. Conservative therapy was combined with shock wave therapy in the area of the penis. The group for comparison consisted of 15 healthy men (volunteers) without signs of ED (IIEF-5 21–22 points). Immune-enzyme analysis was used for detection of sex hormone status. Ultrasound, dopplerographic and X-ray methods were used for conduction of angiography of pool vessels of internal pudendal artery (IPA). Electroneuromyography of penis nerves was conducted. Results. According to the conducted research patients with DM were diagnosed with cavernous fibrosis, angiosclerosis of IPA and penis vessels in 100 % cases. Axonopathy of motor and sensory nerves of penis was detected in 100 % cases, stenosis and occlusion changes in IPA were detected in 42.9 % cases causing disorder and insufficiency in arterial perfusion in IPA pool. Conclusion. Denervation changes in sensory and motor nerves of penis and hemodynamicly significant perfusion disorders in IPA pool were principal pathogenetic factors of ED within the patients with DM. Decrease of reaction of IPA to the pharmacological stimulation was noted within patients with DM, which was caused by the angiosclerosis and loss of arterial wall elasticity. A total of 917 men from infertile couples with abnormal ejaculate indicators were examined. Their age was 34.1 ± 6.3 years; the infertility period was 4.6 ± 3.9 years. A retrospective analysis of their case histories, clinical examination, questioning to identify risk factors for infertility, and anthropometric measurements of weight and height were made. Weight was rated normal at a body mass index (BMI) of ≤ 24.9 kg/m2 ; overweight at 25.0–29.9 kg/m2 , and obesity at ≥ 30 kg/m2 . When identifying infertility risk factors, the investigators kept in mind 24 risk factors at the moment of examination or in the patient histories, which were grouped into 3 clusters: 1 – environmental factors and occupational hazards; 2 – evidence of congenital and acquired abnormalities; 3 – social and quality-of-life factors; this cluster also includes history and examination evidence of tuberculosis, sexually transmitted diseases, and other social diseases, such as hepatitis B and C, or human immunodeficiency infection. Then the men who did not show an exacerbation of somatic diseases, genetic anomalies associated with reproductive disorders, or an exacerbation of social diseases at the moment of examination were selected from the total sample. These were divided into 2 groups: normal weight and obese patients. The frequency of the above mentioned infertility risk factors and additionally the proportion of persons engaged in intellectual or manual labor were calculated in each group. Results and discussion. In the total sample, the frequency of infertility risk factors including occupational hazards and environmental factors was 40 %. Conclusion. The sociomedical risk factors of infertility make the most significant contribution to the development of male infecundity. As of now, there is no precise information on the etiology and pathogenesis of CNBP/CPPS and sexual dysfunction in this disease. Despite a considerable body of published work, the disease remains inadequately studied and untreatable. The diagnosis of CNBP/CPPS is most likely to mask a high range of different conditions, including those when the prostate is only indirectly or by no means involved in the pathological process. Some factors, such as pudendal neuropathy with the development of chronic prostatitis, CPPB, vesical obstruction, and neuroautonomic disorders, may be etiological and pathogenetic simultaneously. The problem of CNBP therapy remains unsolved so far in spite of a diversity of used treatments. Therapeutic approaches are generally aimed only at improving quality of life in a patient. All existing treatments for chronic prostatitis imply for reducing the level of sexual dysfunction too as a desirable result.
