Pharyngeal involvement is rare. Case summary: 24 years old man consulted for chronic bilateral nasal obstruction, associated with upper dysphagia to solids. Nasal endoscopy found multiples masses involving the the pharynx. A cervical computed tomography (CT) scan of the neck had shown tissular heterogeneous process infiltrating the walls of the nasopharynx, oropharynx: soft palate, and the anterior wall of the hypopharynx. A direct laryngoscopy under general anesthesia was realized. The anatomopathological examination of the hypopharyngeal biopsy sample found amorphous acellular interstitial deposits, marked by Congo red, showing yellow-green apple birefringence under polarized light. Immunohistochemistry diagnosed type AL amyloidosis. The amyloidosis assessment to rule out systemic involvement showed no abnormalities. The patient did well over the next several months, but recurrence of the pharyngeal amyloid tumor necessitated additional conservative endoscopic excisions in the 7 months follow up. Discussion: Amyloidosis in the head and neck is a rare and benign condition that usually takes the form of localized amyloidosis, Pharyngeal involvement is rare. Clinical symptomatology and imaging are poorly specific for pharyngeal involvement, macroscopically, the diagnosis is anatomopathological. Given the rarity of this disease, it is essential to rule out systemic involvement. A deviated septum is usually asymptomatic, but sometimes may be present with difficulty breathing through the nose, nasal congestion, sinus infections, nosebleeds, sleep problems, headache, or postnasal drip. Many methods and guidelines have been scheduled to treat and manage acute nasal trauma; however, re-establishment of anatomical deformities and function is still a challenge. Septoplasty is the required surgical intervention for patients to relieve their nasal obstruction. Aim: To detect the incidence of post-septoplasty complications and their relationship with patients’ age and gender among cases with nasal septum deviation. Methodology: A retrospective Electronic record-based study was conducted. This study included patients diagnosed clinically with a deviated nasal septum (DNS) and have undergone surgical intervention (Septoplasty) at Khamis Mushait General Hospital. Data extracted included patients’ demographics and postoperative recorded complications. Results: The study included a total sample of 93 patients. Nasal obstruction was the commonest symptom among patients. As for postoperative complications, nasal obstruction represents nearly one-third of the cases followed by external nasal deformity, and smell disturbance. Conclusion: In conclusion, our study revealed that the only significant post-operative complication incidence in relation with both gender and age groups is a nasal deformity. Nasal obstruction is the most common postoperative complications recorded in general and among male and female patients as well as in all age groups although there is no significant relation to both gender and age groups. Patient oral examination was difficult due to severe trismus but no signs of tonsillitis. Naso-endoscopic examination showed severe swelling of the base of the tongue without any significant signs of recent trauma or other signs of the acute inflammatory process. Imaging showed large posterior tongue base abscess. Drainage of the abscess was achieved through the external approach. The patient recovered well and was discharged home on oral antibiotics. It is most common in children and those with learning difficulties [1,2]. Intranasal foreign bodies are often benign but can cause nasal obstruction and irritation if not treated. Certain objects, such as button batteries and paired batteries, are locally destructive [3]. Management of foreign bodies generally starts with non-invasive methods such as positive pressure techniques, through instrumental removal and finally as a last resort to removal under a general anaesthetic [1,3]. We report an 11 years old boy with XPA and seizures and history of frequent emergency room (ER) visits to different local hospitals for recurrent inspiratory stridor which was treated as “croup” since the age of 10. The child was brought into our ER for the first time with recurrent, severe stridor and was admitted as a severe case of “croup”. He was treated for approximately two weeks with only minimal improvement. Laryngoscopy showed normal vocal cord appearance with no signs of inflammation. Shortly after discharge, he was re-admitted with stridor, which was noticed to be episodic, exacerbated by irritation, excitation and post oral feeding. Stridor subsides completely during sleep. Repeated laryngoscopy was performed to investigate other potential causes of stridor. Flexible rhino-laryngoscopy showed paradoxical vocal folds movement with glottic closure during inspiration and limited glottic opening upon expiration which is consistent with laryngeal dystonia. This case report represents the history, physical examination and the follow up findings in this XPA young boy with unusual presentation with stridor and rare association with vocal cord dystonia. Endoscopic approach under good Illumination and magnification lessens the chance of injuring the vital structures and hence lessen the chance of complication. This study is done to compare the conventional method with endoscopic method. Method: Total 60 cases of deviated nasal septum, refractory to conservative medical treatment were randomly selected &divided into 2 equal groups. 30 patients underwent endoscopic Septoplasty and 30 patients by conventional method. All of them were followed up for 6 months. Results: The age distribution was ranging from 16 to 60. Out of them 28 patients (93.33%) of endoscopic group and 25 patients (83.33%) on conventional group were relieved from symptom. In endoscopic Septoplasty group, along with Septoplasty functional endoscopic sinus surgery was carried out in 4 patients (13.33%). In conventional group 4 patients (16.67%) had intra and postoperative complications like excessive hemorrhage and septal perforation etc.7 patients (23.3%) of conventional Septoplasty group presented with recurrence or persistence of symptom within 6 months of surgery. Recurrence when occurs locally are particularly difficult to treat due to the close proximity of this region to the infratemporal fossa and the base of skull. There is also a debate when to perform elective neck dissection in these patients. Objectives: The aim of this study was to identify cases with recurrence and correlate its association with various prognostic factors and disease free survival in these patients. Methodology: Overall, 15 cases were identified with maxillary alveolar carcinomas that were treated between 2011 to 2015. Eight of these cases were Squamous cell carcinomas, 5 Adeno carcinomas and 2 Adenocystic carcinomas. T stages, N stage, grade of tumor, surgical margin involvement by the tumor, presence of perineural and lymphovascular invasion were identified as prognostic indicators. The chi-square and Pearson correlation tests were applied to analyze the association between recurrence and these prognostic factors. Results: Recurrence developed in 3 SCC (37.5%) and 1 ADC (20%) patient during the three years period following the treatment. Among the SCC patients, 2 recurrences were local and 1 regional. Recurrence was found to have a strong positive correlation with T stage (r=0.330, p=0.271), surgical margin involvement (r=0.575, p=0.566) and three years disease free survival (r=0.959, p=0.000). Studies have shown that vestibular symptoms are quite common in patients with SLE. The aim of this study was to evaluate the relationship between cervical vestibular evoked myogenic potential (cVEMP) test and saccular dysfunction and the possible relationship between SLE and vertigo in patients with SLE. Materials and methods: Forty-six SLE patients (aged 18-40 years) and forty healthy volunteers (aged 20-40 years) were included in the study. Patients with SLE were questioned about their duration of the disease, hearing status and balance. Pure-tone audiometry, tympanometry and cVEMP tests were performed to all patients in SLE and control groups. Results: When P1 and N1 latencies were compared by the cVEMP in SLE and control groups, P1 and N1 latencies of the left and right ears of the SLE group were found to be significantly longer than the control group (p <0.05). In twenty-six SLE patients with vertigo, P1 and N1 latencies were significantly longer than patients with SLE without vertigo. There was also a significant relationship between vertigo and duration of the disease (p <0.05). It was found that the rates of saccular dysfunction were higher in patients with SLE. Conclusion: This study showed a strong association between the balance disorders and SLE which is an autoimmune disease.
