Inflamed Meckel’s diverticulum was identified as the cause of hemorrhage due to the reperfusion of this region of the intestine after the invagination was resolved. Case Report – A 16-month-old boy was presented to the emergency department with the signs and symptoms of acute intestinal invagination, which were visually confirmed by ultrasound. The successful reduction of intussusceptions by air enema was performed and confirmed by ultrasound. On physical examination five hours after the gas enema procedure patient had no complaints, and no signs or symptoms of peritoneal irritation. However, 14 hours after the reduction of intussusception patient developed signs of hemorrhage, which suggested the presence of hemorrhaging Meckel’s diverticulum due to a reperfusion of that region of intestine. Conclusion – Despite the successful use of nonsurgical methods of disinvagination, it remains critical for the management team to keep in mind that one of the predisposing factors of acute intestinal invagination such as Meckel’s diverticulum can cause post therapeutic complications, manifesting as acute hemorrhage.","PeriodicalId":36516,"journal":{"name":"Central European Journal of Paediatrics","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-03-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"70851280","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Using voiding cystourethrography (VCUG) IRR is usually detected in high grades of VUR. Our case demonstrates the possibility of visualizing IRR in a child with low-grade VUR using contrast-enhanced voiding urosonography (ceVUS). Case Report − We report the case of a four-month-old girl with Klebsiella pneumoniae febrile urinary tract infection (UTI) who was diagnosed by ceVUS with VUR grade II on the right kidney, with IRR strongest in the lower pole. A 99m Tc 2.3-dimercaptosuccinic acid (DMSA) renal scan, which was performed 8 months after the UTI revealed a small scar on the lower pole of the right kidney, where IRR was found by ceVUS. Conclusion − Our case report showed that not only the grade of reflux but also the presence of IRR is important to predict the severity of the clinical picture and the development of possible scarring. It is important to find IRR because it may be connect with renal scars. VUR with IRR should be managed actively to decrease the chances of renal scarring or the development of new scars.","PeriodicalId":36516,"journal":{"name":"Central European Journal of Paediatrics","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-03-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"43572055","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Research papers related to the consequences of child abuse and neglect and published in leading academic journals were identified and reviewed. We found that most of the available research suggests that there is a variety of negative outcomes among those exposed to physical, sexual and emotional abuse, neglect, witnessing domestic violence and multi-type childhood abuse. Specifically, there was evidence of significant short-term and long-term consequences encompassing physical and mental health difficulties, subsequent perpetrations, social and cognitive difficulties, and attachment. Conclusion  – The results of this review confirm that there is a wide range of short-term and long-term difficulties that are associated with child abuse and neglect. This variety of difficulties should be taken into account in approaching complex and multidisciplinary assessment and treatment.","PeriodicalId":36516,"journal":{"name":"Central European Journal of Paediatrics","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-12-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48962732","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and Methods – We analysed maternal and perinatal history, clinical and laboratory data, therapy and short-term outcomes in neonates who had enterovirus detected in the cerebrospinal fluid (CSF) and compared the group of neonates with early-onset (≤7 day of life (DOL)) meningitis to the group of neonates with late-onset (>7 DOL) meningitis. Results – A total of 30 term neonates were included (63% male). Majority (73%) presented during the summer and autumn months, 57% were in contact with siblings who had signs of viral disease, 10% of mothers had signs of viral disease at delivery or shortly after. Neonates presented with irritability (97%), fever (83%), feeding intolerance (66%), diarrhoea (56%), nonspecific rash (23%), seizures (10%). Both the C-reactive protein (CRP) and pro-calcitonin (PCT) were elevated in 23% of patients; only CRP was elevated in 57%, only PCT in 37%. Twelve (40%) had elevated leukocyte count in the CSF. Antibiotics were initiated in 80% (median treatment 1.5 days). Majority of the neonates with early-onset meningitis had jaundice, higher urea and creatinine serum concentration, and lower thrombocyte numbers. Conclusion – Enteroviruses are an important cause of meningitis in neonates during summer and autumn months. Neonates in close contact with ill family members or carers are particularly at risk. Neonates present with fever, irritability, feeding intolerance, diarrhoea, normal or mildly elevated CRP and/or PCT values. Neonates with early-onset meningitis had a similar grade of infection and outcome as did the neonates with late-onset disease. Our findings confirm that enteroviral meningitis carries a good prognosis in the majority of neonates.","PeriodicalId":36516,"journal":{"name":"Central European Journal of Paediatrics","volume":"16 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46022341","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Material and Methods The study included all women who had vaginal delivery during 2019. Women were classified according to use of the outlet forceps for delivery into the study and control group. We recorded maternal age and parity, duration of delivery, stimulation and epidural analgesia use, neonatal measures and Apgar score, maternal and neonatal complications after delivery. Results Of the 3,570 vaginal births, forceps was applied 23 (0.64%) times. Most women were aged <35 years and were primiparous. The indication for forceps use in all cases was the delay in the expulsion phase. Forceps deliveries lasted on average 6.20+/-2.15 hours. In both groups, birth canal injuries were frequently registered (P=0.001), while uterine atony was rare (P=0.001). Postpartum anemia was more frequent after forceps application, however, not significantly. A similar number of children with and without neonatal complications was registered (P=0.140). Children born using forceps had lower Apgar scores than those from the control group (P=0.046) and spent longer time in the intensive care unit (P=0.038). There were no other significant differences in any of the tested parameters of mothers and children between the study and the control group. Conclusion Use of forceps is remarkably rare in our setting. Children born using forceps have less favorable early perinatal outcomes, but few serious long-term complications. If applied skillfully and according to indications, forceps is safe for both mothers and children.","PeriodicalId":36516,"journal":{"name":"Central European Journal of Paediatrics","volume":"16 1","pages":"123-131"},"PeriodicalIF":0.0,"publicationDate":"2020-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"44088360","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The syndrome is characterized by neonatal respiratory distress, choreoathetosis and hypothyroidism, but the clinical spectrum and severity of symptoms vary widely. To our knowledge, diffuse pulmonary hemorrhage has not been previously associated with the NKX2-1 gene. Case report We report a patient with brain-lung-thyroid syndrome who suffered from pulmonary hemorrhage in the third hour of life, which led to acute respiratory distress and required urgent endotracheal intubation and mechanical ventilation. In addition, our patient has a phenotype that is typical for brain-lung-thyroid syndrome and carries a de novo heterozygous pathogenic a p.T86fs variant in the NKX2-1 gene. Conclusion Pulmonary hemorrhage in our patient could be a primary manifestation of the brain-lung-thyroid syndrome. This report provides evidence of a wide spectrum of NKX2-1-associated pulmonary phenotypes and underscores the importance of considering NKX2-1-related disorder in the differential diagnosis of pulmonary disease of unknown cause.","PeriodicalId":36516,"journal":{"name":"Central European Journal of Paediatrics","volume":"16 1","pages":"155-160"},"PeriodicalIF":0.0,"publicationDate":"2020-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"44391086","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Case Presentation – A 13-year old boy was admitted to hospital in 1981, one and a half month after a dental procedure of tooth extraction. The tooth was presumed to have been ingested during the procedure and the child was sent home. The symptoms presented immediately and were aggravating during the time. Before the admission to the hospital the child was examined by medical professionals on three occasions but only at hospital the FBA was suspected. During the first bronchoscopy, the foreign body had not been extracted, so the child was transferred to the referral centre where the tooth was successfully removed and he recovered. Conclusion – In the setting of dental procedure in paediatric population, the missing instrument, material or extracted tooth must be found or a foreign body aspiration and ingestion should be suspected until proven otherwise.","PeriodicalId":36516,"journal":{"name":"Central European Journal of Paediatrics","volume":"16 1","pages":"150-154"},"PeriodicalIF":0.0,"publicationDate":"2020-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"41496107","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
