Leukonychia can be caused by various systemic conditions; however, a fungal infection is also a probable cause. Herein, we report two cases that presented with varying patterns of single-nail leukonychia and were diagnosed with fungal leukonychia based on potassium hydroxide (KOH) examination and fungal culture. A 59-year-old man presented with asymptomatic complete whitish discoloration of the right fourth fingernail. KOH examination revealed many septate hyphae, and Trichophyton rubrum was identified through fungal culture and 18S rRNA sequencing. A 32-year-old man presented with asymptomatic longitudinal ridges and pitting with partial whitish discoloration of the second left fingernail persistent for 5 years. However, only a few studies have reported CAPA in the Republic of Korea.\u0000Objective: To describe clinical characteristics of CAPA in patients at a tertiary care hospital in the Republic of Korea.\u0000Methods: This retrospective, observational consecutive case series study was conducted by reviewing the electronic medical records of patients who developed CAPA at Seoul National University Hospital from January 1, 2020, to August 31, 2021. CAPA was defined by European Confederation of Medical Mycology/International Society for\u0000Human and Animal Mycology (ECMM/ISHAM) consensus criteria. Patient demographics, comorbidities, corticosteroid use, clinical presentation, treatment, and outcomes were investigated.\u0000Results: Eleven patients were diagnosed with probable CAPA according to the ECMM/ISHAM criteria. One patient had classical host factor for invasive pulmonary aspergillosis before admission. All patients received corticosteroid therapy before CAPA diagnosis. The mean total corticosteroid administered before CAPA diagnosis was 220 mg of dexamethasone equivalent dose (range, 80~572 mg), and the mean duration of steroid therapy was 15 days (range, 4~34 days). The median time from intensive care unit admission to CAPA diagnosis was 12 days (range, 5~36 days). All individuals showed aggravation on chest X-rays. Ten patients were diagnosed with positive serum\u0000galactomannan (GM), and one was diagnosed with positive GM in a bronchoalveolar lavage specimen. Of the 11 patients, 8 received voriconazole-based antifungal therapy for a median of 30.5 days. Only two patients survived after antifungal treatment.\u0000Conclusion: These cases illustrate CAPA complicated in critically ill COVID-19 patients. Herein, we report a rare case of cutaneous infection by Fusarium solani in a healthy patient. A 64-year-old patient presented with a tender, erythematous, deep-seated nodule above the left ankle after an intralesional triamcinolone\u0000injection for lipoma was administered at a private clinic 1 month ago. Fungal hyphae were identified in subcutaneous tissue by skin biopsy using Gomori methenamine silver stain. Lactophenol cotton blue staining, fungal culturing, and 28S rRNA sequencing confirmed the presence of F. solani. Other reports have also found the absence of eosinophilia, presumably related to the immunosuppressive state of the patient. This indicates that the normal predictive value of eosinophilia on blood tests is unlikely to be present in patients with immunosuppression cases of multiple sparganosis which has been most frequently reported in immunocompromised patients.
