We report a 4-year-old asymptomatic boy with a right ventricular fibroma detected during routine examination. Multimodality imaging including echocardiography and cardiac computed tomography demonstrated a 24.5 × 33.6 mm well-circumscribed mass beneath the right ventricular outflow tract without hemodynamic compromise. Despite the absence of symptoms, prophylactic complete resection was performed given the tumor's non-regressive nature and potential arrhythmogenic risk. Histopathology confirmed cardiac fibroma. The patient remained recurrence-free with normal ventricular function at 8-year follow-up. This report describes successful management with single-stage combined thoracic and urological surgery. A young female with long-standing nephrolithiasis and recent percutaneous nephrolithotomy, presented with respiratory distress. Initial management included emergent tube thoracostomy for a gross right pyopneumothorax confirmed by computed tomographic (CT) scan of chest. <i>Escherichia coli</i> was cultured from pleural fluid and urine samples. Further contrast enhanced CT findings confirmed Empyema thoracic, transdiaphragmatic nephropleural fistula and lower pole renal cortical defects with emphysematous pyelonephritis. Single-stage right thoracotomy and decortication, repair of the diaphragmatic rent and right nephrectomy achieved excellent outcomes. {"title":"Operative predictors of mortality and neurologic outcomes after modified Bentall for acute aortic dissection: A systematic review and meta-analysis.","authors":"Kristian Kurniawan, Dicky Aligheri Wartono","doi":"10.1177/02184923261439233","DOIUrl":"10.1177/02184923261439233","url":null,"abstract":"<p><p>IntroductionThe modified Bentall procedure is frequently required in acute type A aortic dissection (ATAAD) with root involvement, a high-risk subgroup with substantial early mortality and neurologic complications. Prior studies have examined operative predictors such as cardiopulmonary bypass (CPB) time, cross-clamp duration, circulatory arrest, and conduit type, but findings remain inconsistent. This systematic review and meta-analysis aimed to determine whether specific operative parameters influence early mortality or neurologic outcomes following the Bentall procedure in ATAAD.MethodsA systematic search of PubMed, Embase, CENTRAL, ProQuest, and Google Scholar was performed according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Fifteen observational studies evaluating adult ATAAD patients undergoing the modified Bentall procedure were included. Pooled proportions of early mortality and neurologic complications were calculated using a random-effects model. Heterogeneity, publication bias, sensitivity testing, and meta-regression were conducted to assess associations between operative variables-including CPB time, cross-clamp time, circulatory arrest duration, operative time, conduit type, and concomitant coronary artery bypass grafting (CABG)-and postoperative outcomes.ResultsAcross 1547 patients, pooled early mortality was 13%, and the pooled rate of postoperative neurologic complications was likewise 12%. Sensitivity analyses showed stable results. Meta-regression demonstrated no significant association between any operative parameter and early mortality or neurologic events (all <i>p</i> > 0.05). No substantial publication bias was detected.ConclusionAmong ATAAD patients undergoing the modified Bentall procedure, early mortality and neurologic complication rates remain acceptable. The absence of significant operative predictors suggests that advances in perfusion, myocardial protection, and cerebral management may mitigate time-related risks. The summit brought together leading specialists from across Africa, China, Europe, South America and the USA to address the growing burden of cardiovascular disease, to share their vision for the fight against cardiovascular disease, to raise standards in the diagnosis, treatment and recovery of patients thereby improving procedural safety and clinical outcomes in Africa. The experts agreed on the need for cost effectiveness in cardiac surgery, simulation skills training, an African regional cardiothoracic surgery database, African heart team fellowship programs and specialized working groups to guide cardiovascular diagnostics and treatments focusing on critical areas such as congenital heart surgery, valve surgery and coronary artery bypass surgery (CABG) procedures which are becoming increasingly necessary in Africa due to the rise in cardiovascular emergencies, and finally explore solutions tailored to the continent's unique healthcare challenges. Clinical outcomes, CT-based morphological parameters, and aortic remodeling were compared.ResultsThere were no significant differences between the two groups regarding mortality or major complications. Patients in the Partial-ET group had greater distal stent angulation (34.5 ± 8.9° vs. 24.5 ± 13.7°), and no dSINE was observed during a mean follow-up period of 6.4 months. Aortic remodeling outcomes were comparable between the two groups, while the Partial-ET group demonstrated shorter lower body ischemia time and more proximal distal stent deployment.ConclusionsApplication of Frozenix Partial-ET resulted in outcomes comparable to those obtained using the conventional device, without an increase in mortality or major complications. Despite a greater distal angulation, no early dSINE was observed during the study period. We compared outcomes of off-pump (OPCAB) versus on-pump (ONCAB) coronary artery bypass grafting in diabetic patients using propensity-adjusted observational studies.MethodsWe searched PubMed, Scopus, Web of Science, and the Cochrane Library for studies comparing OPCAB and ONCAB in diabetic patients using propensity-score matching or covariate adjustment. Eligible studies reported short-term or long-term clinical outcomes. Primary endpoints were early (in-hospital or 30-day) mortality and long-term survival. Secondary outcomes included stroke, myocardial infarction, reoperation for bleeding, atrial fibrillation, renal replacement therapy, low cardiac output syndrome, and completeness of revascularization. Pooled odds ratios (ORs) and hazard ratios (HRs) with 95% confidence intervals (CIs) were calculated using random-effects models.ResultsNine studies (observation periods 1996-2021), including >10,000 diabetic patients, met criteria. There was no significant difference in early mortality between OPCAB and ONCAB (OR 0.71, 95% CI 0.44-1.14; <i>P</i> = .15). Long-term mortality was significantly higher with OPCAB (HR 1.16, 95% CI 1.01-1.33; <i>P</i> = .04), while mid-term survival showed no difference. OPCAB was associated with lower risks of stroke (OR 0.47, 95% CI 0.24-0.94; <i>P</i> = .03) and reoperation for bleeding (OR 0.60, 95% CI 0.41-0.88; <i>P</i> = .009), but higher risks of incomplete revascularization (OR 2.07, 95% CI 1.60-2.68; <i>P</i> < .00001) and atrial fibrillation (OR 1.70, 95% CI 1.16-2.47; <i>P</i> = .006).ConclusionsIn diabetic patients undergoing CABG, OPCAB lowers stroke and bleeding risk but increases incomplete revascularization and is associated with significantly higher long-term mortality. We aim to demonstrate the efficacy of extracorporeal circulation (ECC) in extending the ischemia tolerance window and stabilizing metabolic parameters during multidisciplinary replantation.Case PresentationA 23-year-old female presented with bilateral total upper extremity amputations following a high-energy motor vehicle accident. While damage control surgery addressed intra-abdominal injuries, the amputated limbs were preserved using a neonatal oxygenator and a specialized ECC circuit. Perfusion was maintained for 224 minutes, achieving significant metabolic stabilization, with arterial pH improving from 6.72 to 7.57.ResultsAlthough the right limb required secondary amputation due to the initial crush injury, the left limb was successfully salvaged. In our series, transit-time flow measurement often showed systolic reverse flow, which disappeared after proximal right coronary artery ligation. Coronary artery bypass grafting achieved excellent long-term outcomes.
