Burgess","doi":"10.5172/jamh.5.3.247","DOIUrl":"https://doi.org/10.5172/jamh.5.3.247","url":null,"abstract":"Abstract This paper presents an overview of 156 local suicide prevention projects funded under Australia’s National Suicide Prevention Strategy (NSPS), and describes lessons elicited from a content analysis of the projects’ progress and final reports. The purpose of the analysis was to maximise gains in Australian suicide prevention activities by highlighting promising processes and impacts and minimise repetition of less successful elements. The projects targeted 11 groups (with the most common being young people, Aboriginal and Torres Strait Islander people and people in rural/remote areas); occurred in various settings (with community-based projects being particularly popular); and employed different approaches (with training and support for health/community professionals or carers being favoured, along with public health interventions aimed at enhancing well-being and building resilience). Certain processes augured well for project success, including: understanding contextual factors; investigating participants’ needs; drawing on sound evidence; developing multi-faceted strategies; garnering stakeholder support; and employing capable staff. The projects achieved improvements in knowledge about risk and protective factors for suicide, social connectedness and mental health literacy, and reductions in depressive symptomatology. Projects’ sustainability was constrained by their short-term funding. The carers’ and the treating practitioners’ perceptions of these issues are then compared. The research involved semistructured interviewing and qualitative analysis in an interpretative phenomenological approach. In-depth interviews were conducted with the carers of nine clients with a mental illness, from Italian, Vietnamese, Indian and Spanish-speaking backgrounds. The health professionals of the clients were also interviewed, and included psychiatrists and nurses. A number of difficulties experienced by carers from CALD backgrounds were highlighted. These included a lack of involvement in the treatment process, insufficient communication from health professionals, lack of understanding about mental illness, lack of support, increased stress, and a lack of understanding of explanatory models between carer and practitioner. The development of a preventative intervention strategy that is culturally compatible with the carer’s perception of caregiving is needed. Data were gathered from a baseline sample of 13,961 women and a follow-up study using semi-structured telephone interviews in New South Wales, Australia, between August and December 1997. Logistic regression was used to identify factors associated with medicine use. Qualitative comments regarding women’s experiences of and attitudes to medicine use were analysed thematically. The respondents were 322 women aged 46–50 with low mental health scores who took part in the baseline survey of the Women’s Health Australia study. Thirty-six percent of women who reported a recent period of psychological distress used medicines to help them cope. Taking medicines for distress (either prescribed or non-prescribed) was significantly associated with a lower mental health score on the MHI-5 (OR=0.98, 95% CI 0.96-1.00) and an increasing number of negative life events (OR=1.14, 95% CI 1.05-1.24). Taking medicines prescribed by a general practitioner was associated with being dissatisfied with one’s family relationships (OR=0.52, 95% CI 0.30-0.91), or having gone through menopause in the past year (OR=1.68, 95% CI 1.00-2.81). Qualitative data (n=117) highlighted several concerns about use of prescribed psychotropic medicines, including a belief in natural remedies, unacceptable side-effects, fear of dependency, a belief that medicines cover up problems that need to be solved and that doctors prescribe too readily. While over one third of the midlife women took medicines to help them cope with a period of distress, many expressed negative views about the use of prescribed psychotropic drugs. The study samples were selected from the Outpatient Department of the Departments of Psychiatry attached to the General Hospitals located in Northern Malaysia. Details on background and illness characteristics of the respondents were collected through a questionnaire prepared by the researcher. The Disability Assessment Schedule (WHO/DAS) was used to collect information related to respondents’ social functioning. The Quality of Life Interview (QOLI) was used to collect the information about quality of life (QOL). Results of the present study indicated serious deficits in the areas of social functioning and quality of home environment and these deficits were found to have a significant association with low QOL. Based on these findings, the present research argues that policy makers must focus more on creating rehabilitation resources within communities which will complement the level of success achieved in reducing psychotic symptoms. Results of the study highlight an urgent need for the creation of community-based rehabilitation facilities for the successful care of people with a mental illness in Malaysia. It moves beyond the often unproductive institutional versus community care dichotomy to present a more complex consideration of the residents’ perceptions of supported housing as a site for recovery. This paper contributes to the literature on consumers’ experiences of mental health care in particular places. They experience high levels of unemployment and nonparticipation in the labour force. Unemployment has a number of negative effects including the loss of purpose, structure, roles and status and a sense of identity which employment brings. Employment enables social inclusion in the wider community and is an important way that people with a mental illness can meaningfully participate in the wider community. Australia has a mental health strategy, which guides the ongoing reform of mental health services. However, specific strategies to address the social and economic marginalisation of people with a mental illness have not been addressed. A recovery-oriented approach is recommended, which integrates the key sectors involved. To date there has been little intersectoral collaboration between the various sectors such as mental health services, housing, and vocational services. People require more role-specific assistance to enable them to participate in socially valued roles implicit with citizenship. There is a need to formulate improved pathways to assistance and more evidence-based forms of assistance to re-establish career pathways. This Australian study explores factors that influence sustainability of employment for people with psychiatric disabilities. Participants of a community based vocational rehabilitation program who were successful in gaining open employment were followed up six months after the closure of their programs to investigate factors that helped them stay in employment. This paper contrasts the lived experience of suicidality with the academic and professional discipline of suicidology to show that this gap arises from a scientific commitment to objective knowledge that denies subjective knowledge, therefore excluding spirituality. A brief excursion into some contemporary thinking in Consciousness Studies illustrates that this scientific denial of the subjective, and of spirit, is obsolete, based more on ideological dogma than reason, and no longer tenable. Childcare workers have a vital role in the development of a child’s mental health, and yet are often under-trained in this area. This paper describes the implementation, evaluation and sustainability of the Healthy start program in regional Western Australia, with particular attention to outcomes for childcare workers. Healthy start aimed to build the capacity of the childcare workforce to promote the mental health of children attending childcare, their families and those working in the childcare sector. A range of strategies was developed and implemented, including mental health literacy training and communication skills training which were delivered to over thirtyfive childcare workers. Pre and post-training questionnaires showed that awareness of risk factors, protective factors and referral sources, as well as levels of confidence in discussing mental health issues with parents, increased immediately after training. Baseline and followup telephone surveys showed however that the childcare workers’ awareness of risk and protective factors was not sustained over a twelve month period. The findings suggested that messages need to be reinforced post-training to retain new knowledge and confidence. Each partner in the project shared the perception that children and adolescents with disruptive behaviour disorders were often misunderstood and a source of stress to those with whom they came into contact. This paper describes an innovative response to increasing understanding and enhancing effective responses in school settings towards these young people. The project provided professional education seminars for primary and secondary schools, and a resource book was prepared and given to schools. Written material was prepared for families both at primary and secondary schools. As the project developed, a shared understanding of the issues evolved in the partnership.
