The patient was a 67-year-old female with liver cirrhosis, a huge portal-systemic shunt, hy-perammonemia, and thrombocytopenia. BRTO was performed because hepatic encephalopathy was suspected. The preoperative liver function was Child-Pugh class A, whereas the wedged hepatic venous pressure was 19 mmHg. Six months postoperatively, she had liver failure because of a gastroesophageal varix rupture. Takagi","doi":"10.2957/kanzo.62.703","DOIUrl":"https://doi.org/10.2957/kanzo.62.703","url":null,"abstract":"In total, 428 cases of chronic hepatitis C were treated by direct acting antivirals, and 400 cases achieved a sustained viral response (SVR) (93.5%). We were able to contact 20 cases of SVR in the 44 dropped-out cases from the routine screening of hepatocellular carcinoma (HCC), and the hepatitis medical care coordinators (HMCCs) noticed the need for HCC screening even after achieving SVR. HMCCs in Japan, usually nurses and other medical staff such as medical clerks, should be educated regarding the prevention and examination of hepatitis virus infection. They would be available to consult for hepatitis, its treatment and follow-up for the development of HCC even after achieving SVR. In our study, 3 HCC cases were found from 20 patients who achieved SVR with-out follow-up HCC screening. They were successfully treated and demonstrated a complete remission. She came to our hospital with a chief complaint of a vulvar mass. We suspected syphilis based on the cutaneous findings and a positive RPR test. Se-rum hepatobiliary enzymes were elevated, and biliary tract enzymes were significantly high. Abdominal imaging showed fatty changes around the portal veins and diffuse intrahepatic biliary strictures. Liver biopsy showed lymphocytic infiltration and fibrosis around the Glissonʼs sheath and fatty degeneration in the paren-chyma. After prescribing AMPC, the blood test and imaging findings improved over the course of approxi-mately 11 months. Among the past reports of early syphilitic hepatitis, only a few have shown biliary abnormali-ties on imaging.
