A systematic search was conducted in the different databases of systematic reviews, cohort studies, case series and case reports of patients of any age diagnosed with HWWS. Clinical presentation characteristics and the most frequent diagnostic technologies used were obtained. A narrative summary of the findings is presented.</p><p><strong>Results: </strong>Overall, 77 publications with a total of 676 patients were included. The most frequent symptom was dysmenorrhea (63.9%), followed by pelvic pain (35.2%). The most frequently used diagnostic technologies were pelvic ultrasound in 92.1% of cases and nuclear magnetic resonance in 74.6%. Hysteroscopy and laparoscopy are seldom used for diagnosing this condition.</p><p><strong>Conclusion: </strong>HWWS is an infrequent disease condition. It must be part of the work-up for renal agenesis in pediatric patients and of the differential diagnosis in primary dysmenorrhea in adolescence. {"title":"Ectopic mammary tissue in vulva: case report and systematic literature review","authors":"Sandra Marcela Buitrago-Flechas, Sandra Johana Barrera-Latorre, Carolina Morante-Caicedo","doi":"10.18597/rcog.3593","DOIUrl":"10.18597/rcog.3593","url":null,"abstract":"<p><strong>Objective: </strong>To report the case of a patient diagnosed with ectopic mammary tissue in the vulva, and to conduct a literature review of the diagnosis, treatment and prognosis of this condition in that location.</p><p><strong>Materials and methods: </strong>A 49-year-old patient who presented with a painful vulvar mass to a private intermediate complexity center in Bogotá, Colombia. The lesion was assessed on ultrasound and then surgically excised; histopathology showed ectopic mammary tissue with absence of malignancy. A search was conducted in the PubMed, Embase, Cochrane, LILACS and Scielo databases using the keywords “Vulva,” “Breast” and “Ectopic.” Case reports and case series of women with histopathology-confirmed mammary tissue in the vulva were included.</p><p><strong>Results: </strong>Overall, 184 titles were identified and, of these, 94 were ultimately included, for a total of 126 cases, with 57.9% being benign tumors, 95% in women under 50 years of age, and 42.06% being malignant tumors, 92% in women over 50 years of age. Diagnosis was made on the basis of the clinical findings, with ancillary diagnostic imaging, tumor markers and immunohistochemistry in some cases. Local excision was performed in 91% of cases with benign pathology and in 43% of cases with malignant pathology, with the diagnostic method being therapeutic.</p><p><strong>Conclusions: </strong>Ectopic mammary tissue in the vulva must be considered as part of the differential diagnosis of vulvar masses, prognosis being different in pre and postmenopausal women. {"title":"Revisiting the indication for prophylactic contralateral mastectomy in patients with Li-Fraumeni syndrome and breast cancer. Case report","authors":"Ximena Briceño-Morales, Clara Briceño-Morales, Silvia Inés Guerrero-Macías, Ana María Pedroza-Durán, Raúl Alexis Súarez-Rodríguez","doi":"10.18597/rcog.3690","DOIUrl":"10.18597/rcog.3690","url":null,"abstract":"<p><strong>Objective: </strong>To describe the case of a patient with Li-Fraumeni syndrome (LFS) and breast cancer in whom the benefit of contralateral prophylactic mastectomy (CPM) was challenged; and to offer a critical discussion regarding the evidence supporting this procedure in this patient population.</p><p><strong>Case presentation: </strong>A 37-year-old woman with breast cancer and a family history of multiple early onset cancer of the LFS spectrum in whom a pathogenic variant of the TP53 gene was confirmed during adjuvant hormonal therapy. The case was presented during the multidisciplinary meeting of the Breast Service of a referral oncology center in Colombia, in order to discuss the benefit of CPM. The decision of the board meeting was not to perform CPM. After 30 months of follow-up, the patient is disease-free.</p><p><strong>Conclusion: </strong>There is no evidence on the impact of CPM on survival of patients with LFS and breast cancer in particular. However, in light of the current knowledge, it is not possible to generalize the approach of withholding this prophylactic surgery. Retrospective cohort study","authors":"Luis Felipe Laverde-Martínez, Laura Marcela Zamudio-Castilla, Akemi Arango-Sakamoto, Natalia Satizábal-Padridin, Leidy Johanna López-Erazo, Einar Sten Billefals-Vallejo, Yuliana Angélica Orozco-Peláez","doi":"10.18597/rcog.3644","DOIUrl":"https://doi.org/10.18597/rcog.3644","url":null,"abstract":"<p><strong>Objective: </strong>To describe the clinical characteristics and the frequency of maternal, fetal and neonatal complications in accordance with the neuraxial anesthesia (NA) technique in women with twin-totwin transfusion syndrome (TTTS) treated with laser photocoagulation.</p><p><strong>Materials and methods: </strong>Descriptive retrospective cohort study of pregnant patients with TTTS treated with laser photocoagulation under NA at Fundación Valle del Lili, Cali (Colombia), between 2013-2017. Patients with Quintero stage V TTTS were excluded. The protocol was approved by the institutional ethics committee.</p><p><strong>Results: </strong>Of the participants, 32 met the inclusion and exclusion criteria. The study population consisted of young, multiparous women. Urgent interventions were performed in 87.5% of cases; 43.7% were Quintero stage III and epidural anesthesia was used in 56.2% of the women. Maternal hemodynamic variables were similar, in accordance with the timing of surgery and the neuraxial technique used. Sustained hypotension occurred in 65.6% of the pregnant women and 9.3% developed pulmonary edema. Pre-term delivery occurred in 65.6% of the patients and 18.7% had premature rupture of membranes. There were 14 fetal demises and five neonatal deaths. There were no cases of maternal mortality.</p><p><strong>Conclusions: </strong>In patients with TTTS requiring laser photocoagulation, the use of epidural, spinal or combined anesthesia is likely associated with similar maternal hemodynamic variables at the time of surgery. Practitioners providing care to these pregnant women must be aware of the frequent occurrence of maternal, fetal and neonatal complications. {"title":"Good prenatal care practices in teenage pregnancy from the perspective of healthcare professionals","authors":"Odette Del Risco-Sánchez, Erika Zambrano-Tanaka, Natividad Guerrero-Borrego, Fernanda Surita","doi":"10.18597/rcog.3695","DOIUrl":"10.18597/rcog.3695","url":null,"abstract":"<p><strong>Objective: </strong>To systematize good prenatal care practices in a referral center for pregnant teenagers in Campinas, São Paulo, Brazil, from the perspective of healthcare professionals.</p><p><strong>Materials and methods: </strong>Qualitative study conducted between June and July 2017 using semi-structured interviews and a focus group with 10 healthcare professionals providing care to pregnant teenagers in a public hospital in the region of Campinas, São Paulo, Brazil. Data were examined using the thematic content analysis technique, which gave rise to new analytical categories. Triangulation of the results was used for validation. The theoretical and practical applicability of the main findings was identified.</p><p><strong>Results: </strong>The study looked into three categories arising from the discourse of the professionals and the evidence found in the literature: 1) comprehensive analysis of pregnant teenagers; 2) professional experiences related to the care of pregnant teenagers reflecting service quality; and 3) strengths and improvement opportunities potentially replicable as a care model. Case report","authors":"Jesús Arnulfo Velásquez-Penagos, Ana María Flórez-Ríos, Edison Muñoz-Ortiz, Jairo Alfonso Gándara-Ricardo, Juan Pablo Flórez-Muñoz, Erika Holguín-González","doi":"10.18597/rcog.3659","DOIUrl":"https://doi.org/10.18597/rcog.3659","url":null,"abstract":"<p><strong>Objective: </strong>To report the case of a pregnant woman with mirror syndrome associated with non-compaction cardiomyopathy in the mother and the fetus, in which antenatal medical treatment provided to the mother resulted in a favorable perinatal maternal outcome.</p><p><strong>Case presentation: </strong>A 16-year old primigravida with 33 weeks of gestation referred from a Level I institution to a private Level IV center in Medellín, Colombia, because of a finding of fetal hydrops on obstetric ultrasound. During hospitalization, the patient showed clinical and ultrasonographic signs of heart failure (dyspnea, edema and hypoxemia), with the diagnosis of hydrops fetalis (mirror syndrome) also confirmed. Diuretic treatment with furosemide was initiated in the mother, with subsequent improvement of the maternal condition as well as of the fetal edema. During the subacute postpartum period in the hospital, the presence of non-compaction cardiomyopathy was confirmed on cardiac nuclear magnetic resonance imaging in both the mother and the newborn. After discharge in adequated condition, they were included in the cardiovascular follow-up program for heart failure and congenital heart disease, respectively.</p><p><strong>Conclusion: </strong>A case of mirror syndrome associated with maternal and fetal non-compaction cardiomyopathy is presented. There is a limited number of reports on mirror syndrome due to cardiac anomalies (maternal and fetal), with weak treatment descriptions, pointing to the need for research in this area. The baby was born with congenital erythroderma complicated with skin infection, and later developed septic shock and died. The genetic and pathologic workup led to the conclusion of Omenn syndrome.</p><p><strong>Conclusion: </strong>Omenn syndrome must be considered as part of the differential diagnoses when prenatal ultrasound shows findings of altered tegument system. {"title":"Congenital syphilis with hydrops fetalis: report of four cases in a general referral hospital in Bogota, Colombia between 2016- 2020","authors":"Andrés Mauricio Camacho-Montaño, Reinaldo Niño-Alba, Edgar Páez-Castellanos","doi":"10.18597/rcog.3591","DOIUrl":"10.18597/rcog.3591","url":null,"abstract":"<p><strong>Objective: </strong>To report four cases of hydrops fetalis secondary to congenital syphilis and carry out a review of the literature to answer the question, What is the antibiotic regimen used in cases of gestational syphilis with hydrops fetalis as a complication?</p><p><strong>Materials and methods: </strong>Four cases of congenital syphilis with hydrops fetalis are presented. Maternal age ranged between 17 and 28 years, gestational age at the time of diagnosis varied between 25 and 30 weeks, and two of the mothers had not initiated prenatal care at that time. Treatment with crystalline penicillin for gestational syphilis was given immediately 6 to 12 weeks before delivery in three cases and partners were prescribed treatment with benzathine penicillin. As for the neonates, two had no active infection or sequelae and one of them was considered to have congenital syphilis based on non-treponemal test titers. In one case, the patient was unable to receive syphilis treatment before delivery and her newborn had signs of active infection. A review of the literature was conducted in the Medline, LILACS and Google Scholar databases using the search terms “hydrops fetalis,” “Lues”, “syphilis – prenatal diagnosis - ultrasound - penicillin – treatment”. The search included case reports and case series or cohorts of newborns with gestational syphilis and hydrops fetalis. Information regarding treatment in the mothers and in the newborns was extracted.</p><p><strong>Results: </strong>Overall, 119 articles were identified. Of these, 13 met the inclusion criteria, three were discarded because the full text could not be accessed. Ten studies with a total of 16 reported cases of hydrops fetalis secondary to congenital infection were reported. Of these, three presented with severe fetal anemia and required intrauterine transfusion; 5 cases received intrauterine penicillin treatment. In four cases the mother received weekly intramuscular injections of benzathine penicillin for 3 weeks, one received additional intravenous crystalline penicillin for 13 days, while another one received intravenous crystalline penicillin for 14 days. Treatment during gestation was not given in a total of 11 cases; and 6 of the 16 cases (37.5%) resulted in perinatal death.</p><p><strong>Conclusion: </strong>Delays in prenatal care and late diagnosis and treatment of gestational syphilis are important causes of persistent congenital syphilis. {"title":"Sexual dysfunction and drug use in women: A reciprocal etiologic relationship","authors":"Andrés Felipe Calvo-Abaunza, Amanecer Rueda-Monsalbe, Diego Andrés Castañeda-Martínez","doi":"10.18597/rcog.3627","DOIUrl":"https://doi.org/10.18597/rcog.3627","url":null,"abstract":"<p><strong>Objective: </strong>To reflect on the reciprocal etiologic relationship between female sexual dysfunction and drug abuse, and its implications for practice and research.</p><p><strong>Materials and methods: </strong>A description of the effects and short-term and long-term consequences of drug use in women is presented together with an analysis of whether drug use is the cause of sexual dysfunction or on the contrary, if sexual dysfunction leads to drug abuse.
