S","doi":"10.36611/upjohns/volume10/issue1/7","DOIUrl":"https://doi.org/10.36611/upjohns/volume10/issue1/7","url":null,"abstract":"AIM\u0000To highlight the advantages of median labiomandibulo\u0000glossotomy as a surgical approach for\u0000excision of tongue base malignancy.\u0000MATERIAL & METHODS\u0000It is a case review of 5 cases with tongue base\u0000malignant tumours treated in a tertiary care centre.\u0000There were 3 female and 2 male patients in the age\u0000group of 18-50 years. Histopathology included\u0000muco-epidermoid carcinoma in 3 cases, squamous\u0000cell carcinoma in1 case and clear cell sarcoma in 1\u0000case. Pre-operative assessment included\u0000endoscopic examination and MRI. Surgical\u0000excision was done via medianlabio-mandibuloglossotomy.\u0000Outcome was assessed in terms of\u0000oncologic adequacy and the functional impact.\u0000RESULTS\u0000Disease free resection margin could be achieved\u0000in all the 5 cases. Near normal functional recovery\u0000could be achieved within 2 weeks. 2 cases\u0000required speech therapy for few months. Presenting a case of 62 year diabetic male\u0000with complaints of loss of vision, ptosis.\u0000Management of orbital apex syndrome mainly\u0000depends upon localization of the lesion,\u0000identification of the etiology followed by full course\u0000of antifungal therapy. Clinical diagnosis was\u0000supplemented with imaging modalities, like\u0000contrast enhanced computed tomography (CECT)\u0000of the orbits and paranasal sinuses, as well as MRI\u0000PNS with orbits, along with lab investigations. It exerts health benefits beyond\u0000inherent basic nutrition, and its role in increasing\u0000the immunomodulating capacity of the host is\u0000widely accepted. In the treatment of allergic\u0000diseases, the beneficial effect of probiotics has\u0000been demonstrated.\u0000OBJECTIVE\u0000This study was aimed to observe the effectiveness\u0000of probiotics in reducing the symptoms of nasal\u0000allergy.\u0000METHODS\u0000Total 50 patients suffering from Allergic Rhinitis\u0000(AR) were administered with probiotics for a period\u0000of six weeks post which the VAS and IgE scores of\u0000the symptoms (Rhinorrhea, Sneezing, and Nasal\u0000obstruction) were statistically analyzed. Detailed history and\u0000examination of all patients fulfilling the inclusion\u0000criteria was done. Preop findings were observed\u0000and recorded. MERI scoring was obtained. The\u0000data collected pre operatively was assessed with\u0000surgical outcome based on graft uptake status and\u0000PTA. Post op follow up was done at 1.5 and 3\u0000months. This data was evaluated and compared\u0000with pre op finding.\u0000RESULTS\u0000The overall success rate of tympanoplasty in the\u0000study was 86% based on graft uptake and 85%\u0000according to hearing benefit. The number of\u0000patients with graft up[take was higher among\u0000those with mild MERI score (87.5%) compared to\u0000those with moderate and severe MERI scores\u0000(83%). The graft uptake is lesser in patients with\u0000gross DNS & Eustachian tube dysfunction. Auditory\u0000perceptual assessment was done using modified\u0000GRBAS score, Voice Handicap Index and\u0000Maximum phonation time.\u0000RESULTS\u0000Statistical analysis revealed a significant difference\u0000between pre and post-injection voice quality as\u0000assessed by “GRBAS” scale and Voice Handicap\u0000Index (VHI). The purpose of the study is to\u0000put across our experience in 69 patients where\u0000endoscopic assisted myringoplasty was done\u0000using tragal perichondrium graft. The graft was\u0000placed medial to the tympanic membrane remnant\u0000by trans tympanic pop through technique and\u0000tympanomeatal flap was not raised.\u0000METHOD\u0000This study was done over a period of 3 years, 69\u0000patients with chronic suppurative otitis media\u0000fitting into the inclusion criteria underwent\u0000endoscopic transcanal myringoplasty using tragal\u0000perichondrium as graft material by underlay pop\u0000t h r o u g h t e c h n i q u e . We\u0000c o n d u c t e d a p r o s p e c t i v e , c o m p a r a t i v e ,\u0000randomized study in Deen Dayal Upadhyaya\u0000hospital New Delhi from July 2016 to May 2017.A\u0000total of 60 patients with symptomatic Deviated\u0000nasal septum within age group of 18 to 60 years\u0000were included in our study. All these patients\u0000underwent septoplasty and were randomly\u0000divided into two group of nasal packing and\u0000transseptal suturing. Patients were interviewed\u0000and assessed for nasal pain, nasal bleeding,\u0000epiphora, dryness of mouth, sleep disturbances.\u0000Mean Pain score using VAS was 8 in nasal packing\u0000group and 2.3 in transseptal suturing group in 48\u0000hours observation period, moreover mean score of\u00008 on VAS was observed during pack removal in\u0000nasal packing group. None of the patient in\u0000transseptal suturing group underwent nasal\u0000bleeding thus none of them required add on nasal\u0000packing.100% patients in medicated gauze nasal\u0000packing group presented with epiphora,\u0000headache, sleep disturbances, and dryness of\u0000mouth in 48 hours observation period, whereas\u0000none of the patient in transseptal suturing group\u0000presented with such symptom. None of the patient\u0000in either group presented with postoperative\u0000septal perforation, synechiae foramation, crusting,\u0000s t t h septal haematoma during 1 and 4 week\u0000diagnostic nasal endoscopy. {"title":"SURGICAL MANAGEMENT OF\u0000POSTAURICULAR EPIDERMAL INCLUSION CYST\u0000AFTER COCHLEAR IMPLANT SURGERY","authors":"Govind Bhuskute","doi":"10.36611/upjohns/volume10/issue1/8","DOIUrl":"https://doi.org/10.36611/upjohns/volume10/issue1/8","url":null,"abstract":"Post auricular epidermal inclusion cyst after ear\u0000surgery are rare in clinical practice and such\u0000condition post cochlear implantation is not yet\u0000reported in the literature.We are reporting\u0000management of two cases of post auricular\u0000epidermal inclusion cyst occurring few years after\u0000cochlear implantation with a functional implant insitu.\u0000OBJECTIVE\u0000To highlight the occurrence of post operative\u0000epidermal inclusion cyst in patients with cochlear\u0000implantation.\u0000MATERIAL AND METHODS\u0000Two cases with post aural cystic lesion post\u0000cochlear implantation surgery reported between\u00002017 to 2019 at a referral centre are highlighted.\u0000They were aged 8 and 13 years old and had\u0000cochlear implant surgery done 5 and 8 years prior\u0000to onset of swelling. Both cases were managed\u0000surgically.\u0000RESULTS\u0000Post cochlear implant swelling at surgical site is\u0000always a matter of concern. Both patients\u0000underwent FNAC and CT which were suggestive of\u0000epidermal inclusion cyst. Total excision was done\u0000with no evidence of recurrence.\u0000CONCLUSION\u0000Post auricular EIC are very rare after cochlear implant and should be kept in differential diagnosis\u0000of post auricular swelling years after surgery and\u0000can be differentiated from seroma by location and\u0000FNAC. Poverty, illiteracy,\u0000overcrowding and poor hygiene are all factors\u0000which play an important role in the causation of the\u0000disease.\u0000Various surgical modalities of treatment have been\u0000tried since ages to eliminate this disease from the\u0000middle ear cleft. The use of the surgical\u0000microscope brought revolutionary advances into\u0000the field of Otological surgery because its new\u0000technology expanded the ability of surgeons to\u0000see in limited confines of the temporal bone.\u0000Initially endoscopes were used only for diagnostic\u0000and photographic purposes but now endoscopes\u0000are being used more and more for surgical\u0000purposes.\u0000The current study attempts to study the\u0000advantages and disadvantages of endoscopic\u0000myringoplasty in comparison to the conventional\u0000microscope assisted myringoplasty.\u0000OBJECTIVES\u0000To e v a l u a t e t h e e ffi c a c y o f E n d o s c o p i c\u0000myringoplasty with Microscopic Myringoplasty\u0000with reference to hearing and graft uptake\u0000MATERIALS AND METHODS\u0000Fifty cases of clinically diagnosed chronic\u0000suppurative otitis media with dry central\u0000perforation were taken into account of which 25\u0000cases were undergone endoscopic myringoplasty\u0000a n d 2 5 c a s e s u n d e r g o n e c o n v e n t i o n a l\u0000myringoplasty. All patients were followed up till 6\u0000months after surgery.\u0000RESULTS\u0000There was a strong statistical significant difference\u0000between the two groups (p=<0.001) for duration of\u0000surgery, hospitalization and hearing gain and graft\u0000uptake. Massive\u0000haemorrhage has been main cause of death\u0000.Selective management is a shift from traditional\u0000mandatory exploration which is based on physical\u0000examination and selective diagnostic studies. We\u0000hereby present a case of penetrating zone II neck\u0000injury by an iron rod in a11 year old boy due to\u0000accidental fall in under construction building.\u0000AIM AND OBJECTIVE\u0000To highlight the rarity of the case and emphasize on\u0000the complex nature of injury.\u0000METHODOLOGY\u000011 year old male child was referred to our medical\u0000college hospital with history of fall over iron rod in\u0000an under construction building. He was vitally\u0000stable on examination and around 45 cms iron rod\u0000was seen penetrating through anterior tongue and\u0000floor of mouth extending through the left\u0000submandibular gland externally. There was no\u0000bleeding or hematoma .
