Defined as a space for the 'self', it is threatened by illness and healthcare interventions. For dignity to be preserved rather than violated, care must respect this space that is necessary for the individual; in other words, access to and sharing of this space must be negotiated. The carer who becomes intimate thus ceases to be a threat. For their part, they must in turn negotiate the non-threatening opening up of a part of their own privacy. The concept of healthy ageing, promoted by the World Health Organisation, emphasises the maintenance of functional abilities as well as prevention. The patient is viewed as an active participant in their own ageing process, in line with an approach based on empowerment. From this perspective, interprofessional support-particularly that provided by advanced practice nurses-plays a central role in the management of frailty. However, in a critical care setting, where older people may be incapable of making decisions or may become temporarily so due to an altered level of consciousness, the burden of decision-making falls on their loved ones and the medical team. How is dignity preserved when treatment choices clash with socio-cultural issues and the sometimes conflicting interests of those involved?
