{"title":"Effectiveness of intracanal calcium hydroxide medicament in treating periapical lesions: a systematic review","authors":"Faisal Alghamdi, Omar Alkhattab","doi":"10.5114/jos.2022.114502","DOIUrl":"https://doi.org/10.5114/jos.2022.114502","url":null,"abstract":"Previous literature has shown that calcium hydroxide (CH) is a useful intracanal medicament for root canals. However, there is no conclusive evidence of its success rate in patients with periapical lesion. This study aimed to investigate the success rate and impact effect of endodontic treatment was done with CH in treating periapical lesions. We searched PubMed, Scopus, Web of Science databases, and Google Scholar as an engine to retrieve rele vant interventional studies. Interventional studies were done on the success rate and impact effect of CH for pa tients with periapical lesion by endodontic treatment/retreatment approaches. Reviews, duplicate, animal studies, and other irrelevant studies were excluded. Two investigators independently following the (PRISMA) guidelines conducted the study selection and data collection process. The outcomes of this review were the status of the peri apical lesion at the end of followup. We retrieved 8 studies with 481 participants. The success rate of CH in this review was 75.5%. The success rate of CH was 16% in patients with periapical/radicular cysts and 74% in patients with apical periodontitis after intracanal CH medicament. The posttreatment followup period showed a high success rate in patients with > 1year posttreatment followup (62%) while the success was 30% in 6 month – 1year posttreatment followups. CH has a higher healing effect in lesions of ≤ 5 mm diameter than > 5 mm diame ter (81.6% vs. 18.4%). Finally, intracanal CH has a clinically significant effect on the healing of periapical le sions. Objectives: This study aimed to evaluate whether surface pre-treatment with Er: YAG laser and bur, and application of various universal adhesives is effective on the repair bond strength of thermally aged nano-hybrid composite restorations. Material and methods: A total of 96 disc-shaped specimens were made from a nano-hybrid resin composite material (Clearfil Majesty Esthetic, Kuraray), using standard size (14 mm diameter × 2 mm thickness) silicon molds. Thermally aged composite disc specimens were assigned randomly into 2 equal groups according to pre-treatment methods (bur or Er: YAG laser). Then, specimens were divided into 3 sub-groups according to used universal adhesive: GPB (G-Premio BOND, GC), CUB (CLEARFIL™ Universal Bond, Kuraray), and ABU (All-Bond Universal, Bisco, Inc.). They were aged 5,000 thermocycles and subjected to a shear bond strength test. Data were analyzed statistically, with p = 0.05. Results: The groups that received pre-treated with Er: YAG laser showed statistically higher values of bond strength than the groups pre-treated with burs ( p < 0.05). There was no statistically significant difference between the groups pre-treated with Er: YAG laser. For the groups pre-treated with bur, there were no statistically significant differences between B-CUB (Bur/Clearfil Universal Bond) and B-ABU (Bur/All-Bond Universal) groups ( p > 0.05). B-GPB (Bur/G-Premio Bond) group exhibited the lowest bond strength ( p < 0.05). Conclusions: Er: YAG lasers may be used as alternative method used for pre-treatment for repairing nanohybrid composite restorations. Disturbances can vary among patients due to severity of the fracture and invasion of the procedure. It is important to evaluate these disturbances to inform patients and improve treatment results. Objectives: To evaluate neuro-sensory disturbances of inferior alveolar nerves after open reduction internal fixation (ORIF) procedure in mandibular fracture. Material and methods: This review used PubMed, Science Direct, SpringerLink, SCOPUS, Medline, and Cochrane databases as bibliographic resources. Studies with matching key words were analysed and screened with PRISMA recommendations. Results: A total of 133 studies were reviewed, but only twelve were included in this evaluation. 44.4% of mandibular cases treated with ORIF suffered from neuro-sensory disturbances. Cases were mostly evaluated objectively by clinical neuro-sensory testing (CNST) with Zuniga-Essick algorithm, and subjectively with questioner with visual analogue scale (VAS) scoring system. Post-operative evaluation was performed in 75% in the first week, and 58% of patients were continued to be evaluated in the sixth month. Conclusions: Neuro-sensory disturbance is the main risk of ORIF procedure in mandibular fracture management. Subjective and objective evaluation is important to obtain information about disturbances due to difficulties for patients to express their feeling.
