Tetapi, pada faktanya selalu terdapat kendala yang membatasi pengerjaan proyek tersebut, salah satu pemicu terbentuknya keterlambatan dalam proyek bidang infrastruktur adanya kecelakan kerja.  Pelaksanaan proyek pembangunan Gedung Laboratorium DLH Banyuwangi tahap II pada pekerjaan struktur terjadi beberapa kasus kecelakaan kerja yaitu tertinjak benda tajam yang dengan presentase kejadian 23%, kejatuhan barang dari atas dengan presentase kejadian 24%, jatuh terpeleset/tersandung dengan presentase kejadian 16%, terbentur barang keras dengan presentase kejadian 21%, terkena runtuhan bangunan 11%, dan terjatuh dari ketinggian dengan presentase kejadian 5% yang menyebabkan beberapa risiko. Kemudian dilakukan minimalisir dengan menerapkan manajemen risiko bidang kesehatan dan keselamatan kerja (K3) menggunakan metode FMEA dan FTA. Saat menerapkan metode FMEA ditemukan bahwa menginjak benda tajam merupakan mode kegagalan dengan hasil RPN paling tinggi, yaitu 36. Saat menerapkan FTA ditemukan 2 top event, 4 intermediete event, dan 5 basic event. Indomaret cabang ini terus mengalami penurunan performance setiap bulannya dalam hal omset dan profit yang didapatkan. Belum pernah dilakukannya penelitian serupa dan banyaknya keluhan yang masih muncul membuat penelitian terkait tingkat kualitas pelayanan dan kepuasan pelanggan menggunakan metode Retail Service Quality Scale (RSQS) dan Customer Satisfaction Index (CSI) penting untuk dilakukan. Penelitian ini membahas mengenai pengaruh signifikan variabel kualitas layanan terhadap kepuasan pelanggan. Hasil penelitian menunjukkan bahwa tingkat kualitas pelayanan pada Indomaret Trikora sudah cukup baik meskipun masih terdapat dua atribut yang bernilai gap negatif. Masing-masing pada atribut ke-5 dimensi reliability dengan nilai gap yaitu -0,6 dan pada atribut ke-2 dimensi kebijakan dengan nilai gap yaitu -0,38. Hasil uji Wilcoxon diketahui bahwa dari semua variabel H0 ditolak sehingga menunjukkan adanya perbedaan signifikan antara persepsi dan harapan.  Selain itu, didapatkan pula nilai CSI sebesar 80,171% yang berarti pelanggan sudah puas dengan layanan yang diberikan. Tingkat kepuasan pelanggan terendah terdapat pada dimensi reliabilitas yakni sebesar 70,120% dan tingkat kepuasan tertinggi terdapat pada dimensi interaksi personal sebesar 72,724%. At this time, a virus outbreak called Covid-19 has disturbed economics globally. CV. Sumber Rejeki is located in the Kec. Pangkah, Kab. East Gresik, one of the factories that carries out the processing of tofu making. Tofu made in the industry is white tofu. The process of tofu processing tries to use labor by practicing social distancing during the creation process and some small industrial scale machines. In the creation process at CV. Sumber Rejeki still uses conventional equipment so that quality is very susceptible to disabilities. On March 15, 2020 to April 15, 2020, in the graph, in the 7th treatment there were 23 tofu defects, the opposite was the lowest in the 31st observation which was 6 tofu defects. During the Covid-19 period, using six sigma if the DPMO value was 5,000.00, it was obtained from the CTQ value of 4, the number of products was 300, the number of defective products was 28. The number of known products has a sigma value from the creation of know of 3.88 when covid — 19, while the causative factor is CV. Dimana, penjualan pada dine in, takeaway, maupun pada layanan pesan antar menurun drastis. Karena, pandemi Covid-19 ini menyebabkan adanya pergeseran dan perubahan pola pembelian konsumen. Sehingga, banyak konsumen yang menggunakan jasa layanan pesan antar untuk memenuhi kebutuhan hidupnya. Namun, juga tidak sedikit masyarakat yang masih tidak tertarik menggunakan jasa layanan pesan antar dikarenakan beberapa faktor. Maka dari itu, UMKM XYZ harus bisa meningkatkan penjualan dengan pemanfaatan layanan pesan antar makanan ini. Penelitian ini bertujuan untuk menghasilkan rancangan model simulasi dan menentukan skenario kebijakan terbaik untuk meningkatkan layanan pesan antar makanan selama pandemi. Metode yang digunakan dalam penelitian ini adalah metode simulasi sistem dinamis yang dapat dibuat model sesuai kondisi nyatanya dan diformulasikan model matematisnya, lalu akan didapat desain skenario kebijakan yang akan dipilih untuk mencapai tujuan. Hasil skenario yang telah dilakukan, skenario yang dapat meningkatkan jumlah customer pada layanan pesan antar di UMKM XYZ adalah dengan meningkatkan potential market yang sudah dibuat skenario yang dibuat UMKM XYZ antara lain memberikan potongan biaya ongkos kirim sebesar 10% dan memberikan potongan harga sebesar 20% dengan minimum pembelian Rp150000. Produktifitas yang tinggi dapat di dukung dari salah satu faktor yaitu keandalan sebuah mesin, dimana mesin ini merupakan faktor penunjang suatu keberhasilan jalanya aktifitas produksi.                              Akibat dari produktfitas tinggi terjadi downtime pada mesin, pada penelitian ini obyek penelitian berfokus pada mesin sablon digital. Adapun tujuan dari penelitian ini penulis bermaksud menganlisaefektifitas mesin digital sablon. Salah satu metode yang sering digunakan yaitu Reliability Centered Maintenance (RCM). Berdasarkan hasil penelitian yang didapatkan oleh peneliti terdapat hasil nilai tertinggi dalam nilai RPN pada mesin sablon digital yaitu pada komponen Tube Tinta dan KOmponen Tinfta Kapsul Filter sebsera 156 dan 52. Kemudian penelitian di lanjutkan kidentifikasi interval waktu pemeliharaan pada komponen- komponen tersebut agar diketahui berapa lama jarak waktu pemelihraan, dan berdasarkan hasil penelitian maka didapatkan pad akompo-nene Tube Tinta yaitu 19.325 Jam serta mengamai kegagaln sebanyak 12 kali untuk 6 bulan. Diproduksi oleh perusahaan. Selama ini UD. Moeljaya dosis tidak melakukan quality control secara berkala, sehingga permasalahan yang selalu dihadapi perusahaan adalah banyaknya barang cacat. Jenis data dalam penelitian ini menggunakan data kualitatif, yaitu data non-numerik, seperti informasi sejarah perusahaan, dan data kuantitatif, yaitu data alfanumerik, termasuk data tentang angka, disabilitas pada tahun 2020. Berdasarkan penelitian yang dilakukan di UD. Moeljaya dengan menggunakan metode FMEA diperoleh hasil yang harus diperhatikan dan diperbaiki. Penyebab seringnya kegagalan pada proses pengelasan antara lain waktu pengelasan yang lebih sedikit antar lapisan ke lapisan berikutnya, dengan Nilai RPN 336. Penyebab seringnya kegagalan pada proses bentuk asimetris antara lain penempatan pelat yang tidak tepat, dengan nilai RPN 245. XYZ is a service company engaged in the maintenance of port heavy equipment and modification of port heavy equipment as well as procurement of spare parts for port heavy equipment. In 2018 along with improving the work performance of PT. XYZ expands its wings to civil works in the port sector, starting from dock repair work, maintenance of port accessories, construction of structures and reclamation. In a maintenance service company and the provision of spare parts for port heavy equipment, the company's inventory must be guaranteed. at each site PT. XYZ especially PT. X has a limited warehouse, so it must maximize the company's inventory. In the company's existing condition, the amount of inventory still exceeds the number of user requests which will accidentally fill their warehouse and will increase the Total Inventory Cost (TIC). With this problem, TIC depreciation will be carried out with a periodic review policy (R, s, S) and a periodic review policy with a consignment stock inventory system. XYZ has problems in the process of recording and planning raw materials which are still manual in the notebook, as well as difficulties in determining the exact quantity and timing of determining which have been estimated so far. In this case, of course, it takes a lot of time and is inaccurate in calculating and recording it. This study has the aim of designing a raw material inventory information system using the EOQ approach. The system is built using Visual Basic language and Microsoft Access database. Based on the results obtained, the inventory information system is designed to store data and transactions related to raw materials. With this supply program, it can help PT. PT. Benua Indah Tuban is a manufacturing company producing clay which is used as a basic ingredient for fertilizers. In its operational activities, workers in this company interact a lot with the same work environment with changing conditions. Employee performance of PT. Benua Indah Tuban during the months January-October 2020 experienced a decline. Thus it is necessary to study the influence of work environment factors on the performance of employees of PT. Benua Indah so that it can improve the performance of employees in the production department. This study uses the social environment, physical environment, and organizational environment as factors that affect employee performance. For data analysis, a model test using the Smart PLS application is used. This research method uses 50 samples with a sample as well as a population or what is called a saturated sample. The results of this study indicate that the physical environment variables have a significant effect on performance by 64.2% and the organizational environment has a significant effect on performance by 28.6%. While the social environment variable does not significantly influence the performance of -3.9%.
