Sixty healthy recreational male runners (age<!--> <!-->=<!--> <!-->35.6<!--> <!-->±<!--> <span>11.7 years old, body mass index</span> <!-->=<!--> <!-->22.9<!--> <!-->±<!--> <!-->2.4<!--> <!-->kg/m<sup>2</sup>) performed trials in shod/barefoot running conditions on a treadmill at self-selected comfortable and demanding speeds. Photogrammetric techniques (2D) were employed. In barefoot conditions, contact time was shorter (<em>p</em> <!-->&lt;<!--> <!-->0.001) at demanding speed, flight time was shorter at comfortable (<em>p</em> <!-->&lt;<!--> <!-->0.05) and demanding (<em>p</em> <!-->&lt;<!--> <!-->0.05) speeds, and there was greater stride frequency at both speeds (<em>p</em> <!-->&lt;<!--> <!-->0.001). In addition, in barefoot conditions, runners landed with significantly greater knee flexion (<em>p</em> <!-->&lt;<!--> <!-->0.05); lower ankle dorsiflexion (<em>p</em> <!-->&lt;<!--> <!-->0.001); and lower knee flexion in take-off at demanding speed (<em>p</em> <!-->=<!--> <!-->0.002) compared with shod conditions. In conclusion, the current study has provided evidence to suggest that acute changes occur in the temporal variables and kinematics between shod/barefoot conditions at low and high speeds in habitually shod runners. Significant differences were found in spatial–temporal events between shod/barefoot conditions, with shorter times in barefoot conditions with greater knee flexion and ankle dorsiflexion. When speed was increased in barefoot conditions, duration of timing variables decreased significantly both comfortable and demanding speed (<em>p</em> <!-->&lt;<!--> <!-->0.001). The Continuum is based on three states of tendon structure: reactive tendon, tendon disrepair and degenerative tendon. In contrast to other proposals, the Continuum model describes continuous changes in tendon structure. Each state of tendon structure represents a particular clinical presentation and requires a particular type of management. Risk factors are often separated into extrinsic (those acting on the body) and intrinsic groups (those acting from within the body). In this narrative review, we will separate potential risk factors into 1) load-related (extrinsic); 2) biomechanical factors (intrinsic); and 3) other individual factors such as systemic factors (intrinsic). Too much load is clearly linked to tendinopathy, but there appears to be large variation in how much load individuals can endure before developing tendinopathy. Less active people also suffer tendinopathy, suggesting that the effect of load is likely to be moderated by intrinsic factors. These individual intrinsic factors are likely to reduce tolerance or capacity to withstand load. The collection of clinical samples from athletes that could permit the analysis of the tendinopathy<span> progression, especially in the early stages, is difficult. For that reason, the purpose of this study is to develop a new experimental animal model of patellar tendinopathy in rats which mimics the human tendinopathy by in vivo intratendinous collagenase injection in the proximal portion of the patellar tendon.</span></span></p></div><div><h3>Material and methods</h3><p>The experimental model used was 8-week-old male Wistar rats (<em>N</em> <!-->=<!--> <span>4). The administration of collagenase was performed by ultrasound-guided puncture at the level of the proximal and deep portion of the patellar tendon in anesthetized animals. However, these changes do not correlate well with the symptoms or the response to treatment. The ‘characterization of tissues by ultrasound’ (UTC) evaluates the structure of the tendon in 3 dimensions and classifies it into 4 echo-types. The aim of this study was to investigate the structure of the patellar tendon in basketball players using the UTC technique, and to study the differences between professional and amateur players by comparing the dominant leg with the non-dominant one, and the asymptomatic with the asymptomatic ones.</p></div><div><h3>Material and methods</h3><p>The study included 40 patellar tendons (20 basketball players). The structure of the patellar tendon (proximal, medial, and distal) was quantified by means of UTC at the beginning of the pre-season. The within and between observer reliability was measured using the intraclass correlation coefficient (ICC).</p></div><div><h3>Results</h3><p>The mean ages of the 10 professional players and 10 amateur players were 28.1 years (SD 4.5) and 17.6 years (SD 1.3), respectively. The thickness of the patellar tendon was greater in the professional players (<em>P</em> <!-->&lt;<!--> <!-->.05), in the dominant leg (<em>P</em> <!-->&lt;<!--> <!-->.05), and in symptomatic tendons (<em>P</em> <!-->&lt;<!--> <!-->.05). There were no significant differences in the echo-types, except in the case of echo-type <span>II</span> in the proximal portion of the right patellar tendon, between professional and amateur players (<em>P</em> <!-->=<!--> <!-->.05). Similarly, there was a difference in echo-type <span>III</span> and <span>IV</span> of the proximal portion of the right tendon between symptomatic and asymptomatic players (<em>P</em> <!-->&lt;<!--> <!-->.05). A good, high reliability was found with these techniques (ICC<!--> <!-->=<!--> <!-->.80-.99).</p></div><div><h3>Conclusions</h3><p>The ultrasound pattern of the patellar tendons of basketball players has been characterised using UTC. The thickness of the tendon is greater in professional players than in amateur players. It has also been observed that the symptomatic tendons are thicker than in asymptomatic ones. Two clinical reports of proximal LHBF injuries are presented: a typical miotendinosus junction (MTJ) injury and a less common injury affecting the free-tendon of LHBF. There is no extensive scientific evidence on the management of this situation, particularly when it is a finding in an asymptomatic individual with no history. The case is presented on a 26<!--> <!-->year-old asymptomatic athlete, a professional rugby player, with no history, who had two premature ventricular contractions in the recovery phase of the effort test (one with an image of a right bundle branch block and the other with a left block). Complementary cardiological tests (Holter, Doppler echocardiogram, ECG signal averaged ECG, and coronary tomography) were normal, except for finding a left ventricular transmural fibrosis in the cardiac resonance.
