Hence, sleep disorders often present as chronic and comorbid conditions. Given the ubiquity of sleep-related complaints in clinical practice, it appears that sleep disorders can be suspected by a wide range of medical practitioners. This is particularly true for specialists treating patients with chronic and/or comorbid clinical conditions (i.e. internistic disciplines, neurology, psychiatry, physical medicine, ENT and general practitioners). The aim of the study is to evaluate in our department prognostic factors for hospital mortality and survival after hospital discharge for patients aged ≥≥≥≥≥ 65 years. Patients and Methods : Retrospective study including all patients ≥≥≥≥≥ 65 with a solid or hematological tumor admitted for an acute complication in an oncological ICU from 01/01/2012 to 31/12/2014. Results : We recorded 311 admissions for 270 patients. The main reasons for admission were cardiovascular (22 %), respiratory (17 %) and hemodynamic (13 %). ICU and in-hospital mortality rates were respectively 10 % and 22 %. The identified independent prognostic factors for higher hospital mortality were the use of non invasive ventilation (NIV) use or of invasive mechanical ventilation (IMV) use and the existence of a therapeutic limitation in the first 24 hours. After hospital discharge, independent prognostic factors for death were Charlson’s score ≥≥≥≥≥ 8, SAPS II score e» 37, administration of amines in the first first hours. A total of 77 % of patients were able to benefit from an antineoplastic treatment after hospital discharge.Conclusion : Life-sustaining therapeutic limitation is directly related to hospital mortality and post-discharge survival. Dauby","doi":"10.30637/2018.17-105","DOIUrl":"https://doi.org/10.30637/2018.17-105","url":null,"abstract":"RESUME Nous rapportons ici le cas d’un patient de 45 ans ayant présenté 2 infections invasives à Streptococcus pneumoniae en 9 mois (deux bactériémies, associées à une pneumonie puis une méningite). Après le 2 ème épisode, le bilan à la recherche d’un éventuel déficit immunitaire a mené au diagnostic d’un myélome multiple, pathologie inhabituelle à cet âge. Bien que les infections à S. pneumoniae soient une complication fréquente du myélome multiple, il est inhabituel qu’elles mènent au diagnostic. Les mécanismes de cette plus grande susceptibilité aux germes encapsulés tels que S. pneumoniae lors du myélome multiple sont discutés. En pratique clinique, ce cas nous permet de rappeler que la survenue d’au moins deux infections bactériennes sévères est un critère de réalisation d’un bilan à la recherche d’un déficit immunitaire. ABSTRACT We here report the case of a 45-year-old patient who presented 2 invasive infections with Streptococcus Pneumoniae in 9 months period (two bacteriemias, associated with pneumonia then meningitis), in which the workup of a possible immunodeficiency led to the diagnosis of multiple myeloma, an usual pathology at this age. Although S. Pneumoniae infections are a frequent complication of multiple myeloma, they do not usually lead to its diagnosis. The mechanisms of this greater susceptibility to encapsulated germs as S. pneumoniae in multiple myeloma are discussed.
