The clinical course and prognosis are however markedly different .The entity may commonly present as a pelvic recurrence after initial surgery for unsuspected leiomyosarcoma. The rarity of this clinical entity renders pre-operative diagnosis to be often missed. The staging of this uncommon variant is significantly influenced by the histopathologic diagnosis. Preoperative suspicion and relevant imaging is needed to improve diagnosis and thereby prognosis through proper surgical staging/sampling and optimal surgical resection as there are no specific tumour biomarkers yet. This is a case report depicting prospectively the outcome of a post-menopausal woman who underwent Total abdominal hysterectomy and bilateral salpingo-oophorectomy for a large fibroid which was confirmed histopathologically as Uterine Leiomyosarcoma( uLMS). After the histologic confirmation of uLMS, the patient was advised Combination chemotherapy, but she declined the adjuvant treatment. After 4 months of primary surgery with no initiation of Chemotherapy , she developed pelvic recurrence with invasion of bladder as detected by CT scan. The patient received 3 cycles of palliative combination Chemotherapy with Inj. Doxorubicin , Inj. Ifosfamide and Inj. Mesna ,Inj. Neukine after the detection of recurrence. She sadly succumbed a few days after completion of the third cycle of chemotherapy. This case report highlights the necessity to suspect the possibility of leiomyosarcoma in a post –menopausal lady with fibroid presenting for surgical management. \u0000Bangladesh J Obstet Gynaecol, 2021; Vol. 36(1): 69-73

Here remote diagnosis and treatment of patients is done by means of telecommunication technology. First published record of telemedicine is in the first half of the 20th century when ECG was transmitted over the telephone line. In Bangladesh telemedicine was not so popular as patients prefer face to face visit to a medical practitioner. But in Covid-19 Pandemic to prevent transmission and Lock down situation telemedicine becomes popular and emerges as a natural evolution of Obstetrics and Gynaecologic health care system in Bangladesh. \u0000The Aim of the study: To see the outcome of patients taking telemedicine for their Obstetric and Gynaecologic problems. The reasons of seeking consultation through telemedicine are also studied. \u0000Materials and Methods: This is a Cross sectional prospective study done during Lock down period in Bangladesh. Study period was 27th March to 20th May. Total 201 women got telemedicine service free of cost via sms, messenger, imo, whatsapp, email using smart phone were included in this study. The data was collected, edited and tabulated by hand tabulation. Calculation was done on scientific calculator. \u0000Results: About 96.51% patients suffering from Obstetrics and Gynaecological diseases were cured by taking medicine, counselling and assurance through telemedicine. Majority of pregnant women(23.88%) sought telemedicine service for consultation about advice regarding diet, medicine, anxiety about ANC visits. 2.48% women were worried about Tetanus Toxoid vaccine.Women (.99%) suffering from Gynaecological diseases such as Fibroid uterus, Uterine Prolapse were seeking for operation.3.48% women suffereing from DUB and managed by medication . Only 3.48% patients were required to refer to Hospital for Emergency admission. \u0000Conclusion: Telemedicine can’t give solution to all problems, but it definitely helps in managing women suffering from Obstetric and Gynaecological diseases at a large extent especially during awakward situation like Covid 19 pandemic. \u0000Bangladesh J Obstet Gynaecol, 2021; Vol. 36(1): 23-27

The current study was undertaken to find out sociodemographic and pregnancy related risk factors that may exist with cases of stillbirth and death during first 7 days of life. This was a cross sectional study conducted at Institute of Child and mother health (ICMH) between February to June 2015. Mothers delivered still birth baby at ICMH and whose babies were admitted in neonatal ward immediately after birth for delay to cry were enrolled consecutively. Data was collected by interviewing mothers and reviewing hospital records using structured questionnaire. Follow up was done till 7 days. Data analysis was done with SPSS version 14 Among a total 116 mothers 83.6% came from rural areas, 48.2% belong to 20-25 years age group with mean age 23±5 year, 46.6% had primary education, 89.7% were primipara, and 62.9% had more than 4 antenatal visits in index pregnancy. Common pregnancy related problems were anaemia (85.3%), preeclampsia (41.4%), less fetal movement (31.9%), polyhydramnios (13.8%), oligohydramnios (12.9%), premature rupture membrane (58.6%), prolong labour 48.3%), malpresentation (8.6%) and urinary tract infection in 12.1%. About 69.8% delivered at hospital, 59% had normal vaginal delivery, instrumental delivery 17.2% and caesarean section in 24.1%. About 57.8% babies were born preterm. and 55.1% were low birth weight. During first 7 days of life 32% developed neonatal sepsis and 32.8% died. Cause of death as recorded was perinatal asphyxia (55.17%), hypoxic ischemic encephalopathy 17.2%, low birth weight with perinatal asphyxia in 35.6% and meconium aspiration syndrome 4.5%. Stillbirth and early neonatal deaths are obstetric catastrophe and has common overlapping risk factors. Pregnancy greater than 20 weeks with severe maternal illness have five times more ICU admission than pregnancy before 20 weeks. Objective: Clinical evaluation of course of COVID-19 disease among Covid positive pregnant women to raise awareness emphasizing preventive measure, to seek early medical care & to have an institutional protocol for maternity care during pandemic. \u0000Methods: A prospective observational study in a single tertiary pandemic centre in CMOSHMC during 15th March to 15th October 2021. All pregnant women admitted with sign symptom of COVID-19 disease, RT-PCR +ve, HRCT+ve or both included in the study. Total 2639 patient admitted with in COVID-19 disease in the study period 54% were female & among them 64 (2.4%) were pregnant.Out of 64 pregnant mother 20(31%) were admitted in ICU,61(95%) in 3rd trimester 29 to39 weeks, 2 previable (aborted) & 1 in postpartum period.Among 61patient, 23 delivered 11 (48%) vaginal,12(52%) cesarean section &7 (11%) died. Indication of cesarean section 3 were for severe&critical covid status & rest 9 were for obstetric cause. 38(60%) patient recovered from covid disease among them 10(16%) from those admitted in ICU & discharged on D16 to D21 with advise for Antenatal checkup & institutional delivery later on. Planned cesarean section for 3rd gravida 39 weeks pregnancy P/ H/O 2 cesarean section was delayed for 10 days after resolution of symptom &cesarean section done at 40+3 weeks, both the mother & baby were healthy. Among patient admitted in ICU lung involvement were 50 to 90 % in HRCT & treatment was given according to guideline inj. Enoxypyrine,Inj. Ramdisivir, steroid &Tocillizumeb.SPO2 maintained by O2 progressively through nasal canula, face mask, HFNC (20patient)& mechanical ventilation in 4patient. Among 7 death 2 died in antenatal period one from cardiac arrest &1 from cardiorespiratory failure (admitted with SPO2 37%). Rest 5 died in postnatal period 3 after cesarean, 2 being for severe Covid status, one for obstetric indication onD9 in hyper immune state & 2 after vaginal delivery. All 7 patient had lung involvement 70 to 90% in HRCT,Out of 23 delivery, 8 were preterm birth with 1 IUD &15 delivered at term. Five neonate were admitted in NICU for prematurity, IUGR for observation. Breast feeding & skin to skin contact were encouraged. All neonate were RTPCR- ve, 2 neonate died on D13 &D17 due to prematurity & IUGR. \u0000Conclusion: Critical review was done to prepare an institutional protocol to optimize care & to reduce risk of severe illness & death .All patient & support partner should be counselled about infection preventive measure, risk factor reduction & importance of seeking medical care early once symptom develop. \u0000Bangladesh J Obstet Gynaecol, 2021; Vol. 36(1): 33-41

The vast majority of stillbirths, 84 per cent, occur in low and lower middle-income countries. Despite the large number of stillbirths worldwide, the topic of stillbirths in developing countries has received very little research, programmatic or policy attention. The loss of a wanted baby at any gestation is distressing, not only to the expectant parents but also to their relatives and the attending obstetrician. Every New-born Action Plan (ENAP) 2014 Global target was: 10 or fewer stillbirths per 1000 total births in all countries by 2030 and 8 or fewer stillbirths per 1000 total births in all countries by as global average and call to action to end preventable stillbirths 2030. To end the preventable stillborn or to achieve the aim of ENAP within 2030, 90% mothers need to receive 4 or more antenatal care by 2025, respectful maternal care during intrapartum with close monitoring and referral facilities for emergency obstetric care & newborn resuscitation need to be ensured. Institutional deliveries need to be increased and at least PNC for 80% women within 2 days of delivery should be ensured. Optimizing nutrition, screening infections and controlling preeclampsia, GDM, and obesity should be the priority. We must work all together to eliminate this tragedy of stillbirth. Total 70 cases of ectopic pregnancies were included. The medical records of the patients managed for ectopic pregnancies were retrived and data were collected from registers. The relevant data were analyzed with simple descriptive statistics and presented in frequency chart and tables. \u0000Results: Total 70 cases of ectopic pregnancies were managed during the study periods which constituted 3.6% of all gynaecological admissions. Most of the patients (35.7%) were between 25 – 29 years age group. It was most common in multiparae with a percentage of 85.7% . The commonest (92.9% ) clinical presentation was abdominal pain. Almost 14.3% were in a state of shock at admission. Among the identified risk factors 28.6% had a previous history of induced abortion and 22.9% had pelvic inflammatory disease. Out of 70, 60 patients (85.7%) had ruptured ectopic pregnancies, 4 (5.7%) were unruptured and 5 (7.1%) produces a complex tubo-ovarian mass. Only 1(1.4%) case was presented early & treated with intramuscular methotrexate injection. Surgery by open method in the form of salpingectomy 64(91.4%) and salpingo-oophorectomy 5(7.1%) were the mainstay of management. No maternal mortality found in this series. \u0000Conclusions: Since ectopic pregnancy remains a gynaecological catastrophe in developing countries and a major challenge to the reproductive performance of women worldwide, it should be considered a relevant public health issue. Early diagnosis, identifying of underlying risk factors and timely intervention in the form of conservative or surgical treatment will help in reducing the morbidity and mortality associated with ectopic pregnancy. \u0000Bangladesh J Obstet Gynaecol, 2021; Vol. 36(1): 28-32

Auditing Caesarian section rates can be done using Robson’s classification which in turns helps achieving a uniform basis for comparison across centers and across various countries. The objective was to describe caesarean rates in a tertiary care hospital of Dhaka using Robson’s Ten Groups Classification. \u0000Methods: A retrospective study was done in Dhaka Community Medical College & Hospital, Dhaka, over a period of one year January 2018 to December 2018. All cases of LUCS done during this period were classified according to Robson’s classification and analyzed. \u0000Results: A total number of 757 deliveries were conducted over this one year study period. C-Section was performed in 550 patients, giving C-section rates of 73%. Robson’s group 5 was the highest contributors to overall CS rate, contributing 28% of all C-sections, followed by group 2 and 4 combindly contributes 20% of overall CS rate (13.4% and 13.1% respectively) .The most common indications of primary C-section were failure to progress of labour (12.5%), Fetal distress (12.4%), and CPD (10.7%). \u0000Conclusions: Robson’s classification helps to identify and analyze the groups that contribute to the most to overall caesarian section rate.
