{"title":"Treatment of psychiatric disorders in the middle ages","authors":"","doi":"10.26565/2312-5675-2020-14-08","DOIUrl":"https://doi.org/10.26565/2312-5675-2020-14-08","url":null,"abstract":"The article shows that medicine in the Middle Ages was based on the principles of scholasticism, which rejected any scientific discoveries and contrasted faith and science, which made its development impossible in general. Methods of treatment, used in medieval medicine, based on gross superstitions, reduced to the belief in the healing power of amulets and talismans; attributing to human health the location of stars; the role of conspiracies and the healing power of crushed stones and minerals. The medieval church linked the causes of disease with two main factors: illness, as God's punishment for human sins, and illness, as an obsession with evil forces. It is established that the basis of medieval medicine was primarily the teachings of the medieval church on the immortality of the soul, saints, diseases, which were dominated by gross pagan beliefs and philosophy, which made not only impossible the development of medicine as such, but also led to mass epidemics, unsanitary conditions, the growth of neuropsychiatric diseases, reducing the duration and quality of life. People with mental disorders have historically been either ignored or institutionalized. Their disorders were not easy to understand and therefore treat. Society often feared and carried out superstitions about people with mental illness that were based on culture and religious beliefs. Those who demonstrated evidence of mental illness were accused of committing sinful crimes and wrongdoings under the influence of the devil. Throughout history, the disease has been associated with demon possession or as punishment for some wrongdoing. Terrible surgeries aimed at freeing demons or healing the brain from disorders have been unsuccessful. Short periods of humane treatment have occurred throughout history. The very concept of disease, adopted in medieval medicine, according to the teachings of the papacy, reduced the disease either to God's punishment or to obsession. The aim of the study is to establish, on the basis of the results of the comparative analysis of the characteristics of the personal profile, the features and nosospecific differences of post-manifest pathopersonаlogical transformations in patients with schizophrenia, schizoaffective disorder and affective disorders in remission / intermissions. The study examined the personality characteristics of endogenous psychoses with an episodic course in remission / intermission conditions and established signs and nosospecific differences in post-manifest pathological transformations in patients with schizophrenia, schizoaffective disorder, and affective disorders. Existing pathopersonalogical transformations are characterized by signs of multidirectional tendencies of compensatory tension of personal resources, which is confirmed by the presence of several moderately elevated (65-75 T-points) indicators of individual scales and a simultaneous increase in indicators of both hyposthenic (2nd and 7th scales) and hypersthenic ( 4th and 9th scales) registers. In schizoaffective disorder, such scales are the 2nd (65.36±12.28 T-points), 4th (73.23±11.83 T-points), 8th (68.40±12.33 T-points) and 9th (66.05±12.02 T-points); in affective disorders, these are the 2nd, 4th and 8th scales (67.72±13.96, 67.08±9.53 and 65.90±10.08 T-points, respectively); for schizophrenia, such scales are the 2nd (72.37 ± 16.80 T-points), 4th (69.47±12.48 T-points), 7th (66.59±15.69 T-points) ) and 8th (71.73±19.95 T-points). As a result of treatment revealed positive dynamics of psychopathological symptoms relief in both groups was revealed; in patients II group improving electrogenesis brain manifested a decrease of irritatie, regress polymorphic paroxysmal activity, the representation of slow-wave complexes, normalization of the ratio of alpha- and beta-rhythms, the emergence of regional differences and the reactions of absorption on the functional load clinically manifested by increase of functional activity of the brain and improving cognitive function (improving ability to work and steadfastness of focus, logic of judgments, the correctness and validity of generalizations, processes of semantic memorization, the ability to highlight the main meaning of the perceived material, the ability to analyze, understand and put into words the information received), before the performance standards, the test execution time was significantly shorter in group II patients than in group I patients (p<0.05) and was closer to the test execution time in the control group. {"title":"Enuresis: modern aspects of etiology, pathogenesis, psycho-, pharmaco-, physical therapy and phytotherapy (part 1)","authors":"","doi":"10.26565/2312-5675-2020-14-02","DOIUrl":"https://doi.org/10.26565/2312-5675-2020-14-02","url":null,"abstract":"The review (part 1) discusses modern views on the etiology, pathogenesis and clinical course of bedwetting in children. Enuresis is a common condition that causes significant disruption in critical areas of a child's life. Despite the existence of an international standard and clinical guidelines for the treatment of bedwetting, specialists do not have a single approach to the management of patients with enuresis. In order to systematize modern views on these issues, 73 sources were analyzed. The criteria for enuresis according to ICD-10 and DSM-5.0 and various approaches to classifying the types of enuresis are present in the review. The characteristics of simple, neurotic, endocrinopathic, epileptic, neuropathic and dysplastic forms of enuresis are given. The modern concepts of the pathophysiology of enuresis are analyzed. Connection between bedwetting and sleep problems, high levels of arousal in the cerebral cortex, fragmentation, and poor sleep quality are accentuated in the several analyzed studies. The changes in the autonomic nervous system and hemodynamics arising in connection with these disorders contribute to the excessive formation of urine and an increase in the activity of the bladder. Some patients have nocturia associated with circadian fluctuations in the level of vasopressin, which, however, does not always explain the occurrence of bedwetting. A number of analyzed studies have established a relationship between polyuria and changes in salt and water excretion. Other mechanisms of the pathogenesis of enuresis, concerning violations of the reservoir function of the urinary bladder during sleep, are also highlighted. Knowledge of the etiology and pathogenesis of enuresis helps doctors better understand the nature of a particular child's condition and, personalize treatment, based on modern protocols. Today Ukraine takes the 9th place in the world by the quantity of IDP. Internal migrations have essentially influenced psychological state of the population`s health. The definition of emotional state of the mentioned above group of population is of great importance for strategies to prevent the psychopathology. The basis of the development of pathological conditions are the mechanisms of maladaptation, which have been studied in terms of stress theory. Stressogenic mechanisms develop according to the same rules, regardless of what stress factors affect — positive, negative, emotional or physical, its effect on the body has common nonspecific features of adaptive syndrome, which wins in three stages: primary anxiety, resistance period and ending. In order to prevent maladaptation, special attention should be paid to the first and second stages for persons in difficult situations. To achieve the aim of this research 155 IDPs were examined with the help of questionnaire, scale PANAS. Even if evaluated persons had not admitted their emotional problems, under detailed examination prevailing of negative affect was recognized. In the group of respondents with positive emotional characteristics increased anxiety and irritability were identified justifying unstable emotional state. The problem of traumatic lesions of the brain does not lose the relevance, despite numerous research, improving the technical equipment of the medical units and organizational measures [2, 4]. The basis for the successful treatment of patients with the brain injury is the emergence of hospitalization and diagnosis with the purpose of determining the most optimal therapeutic measures [1, 4]. Existing diagnostic and treatment programs of mild and moderate severity of the brain injury are far from perfection [4, 7]. The negative moment in the brain injury is progression in the remote period of vegetative, emotional and intellectual - mental disorders that form a traumatic disease of the brain injury and tract the activity of the patient's health traffic activity [8, 10, 11]. Thus, the need for dynamic study of the peculiarities of the course of the periods of brain injury and the development of the algorithm of the treatment tactics and methods of prevention is arranged. Objective: explore the effectiveness of the use of Cytoflavin in the mild and moderate severity of the brain injury in the acute and remote periods in fighters of the joint forces organization (JFO - ATO). Material and methods. After receiving written consent to the implementation of a comprehensive survey in accordance with the principles of the Helsinki Declaration of Human Rights, the Convention of the Council of Europe on Human Rights and Biomedicine, the relevant laws of Ukraine and international acts in the randomized method of researching involved 117 soldiers of the (JFO – ATO) with various clinical forms of the brain injury: (concussion 42 soldiers; mild cerebral contusion 41 soldiers; moderate cerebral contusion 34 soldiers). Each clinical group was distributed to two subgroups A and B: (subgroup A to standard treatment took Cytoflavin and amounted to the group of comparison, and subgroups B in treatment received according to the clinical protocol without the use of Cytoflavin). The fighters (JFO – ATO) were in the treatment in the neurological and neurosurgical department of the Military Medical Clinical Center of the Western Region. The results of the study were treated with a statistical method of evaluating the significance of differences in Fischer. Results of the study. Significant difference in the frequency of complaints in the fighters (JFO – ATO) between the subgroups of each clinical form of the brain injury to the treatment in acute and in the remote periods we did not note be marked. The need to introduce effective and cost-effective strategies and measures to prevent and reduce the harm associated with alcohol abuse in accordance with national priorities and contexts is substantiated. The main stages of treatment are identified, namely: diagnostic and motivational (includes physical examination, choice of treatment program, conclusion of a written therapeutic agreement), detoxification (pharmacological treatment of withdrawal, strengthening the patient's motivation for further treatment), treatment of addiction syndrome (individual / group psychotherapist), symptomatic therapy), medical and social rehabilitation (supportive psychotherapy, social work). The most appropriate and effective psychotherapeutic approaches recommended by modern clinical protocols for the treatment of alcohol dependence in developed countries, including: behavioral therapy for couples (BCT), brief psychological interventions (BPI), family counseling (BSFT), cognitive-behavioral therapy (CBT), contingency management (CM), motivational therapy / motivational interviewing (MT), psychodynamic psychotherapy (PT), 12-step psychotherapy (AA). It is confirmed that the integration of psychotherapeutic and pharmacological approaches is supported in clinical guidelines for the treatment of alcohol dependence. The importance of modern research related to non-psychotic mental disorders in students of higher education is determined by the need to build a mathematical model that can identify factors that significantly affect the mental health of young people. The aim of the study. Construction of a mathematical model of the influence of various factors on the level of mental health of students with non-psychotic mental disorders. Materials and methods. During 2015-2017, we conducted a continuous comprehensive survey of 1,235 students in compliance with the principles of bioethics and deontology. Applied methods: clinical, clinical-psychopathological, clinical-epidemiological, clinical-anamnestic, experimental-psychological and statistical. The sample did not have significant differences in gender and age, place of residence, form of education. The survey was conducted in the intersessional period. Results. As a result of analysis of variance, we obtained statistically significant results (ANOVA-test) between the category variable \"group I\" and variables denoting all scales mini mult test: 1. hypochondria (Hs), p <2,2 * 10-16; 2. depression (D), p <2 * 10-16; 3. hysteria (Hy), p <2 * 10-16; 4. psychopathy (Pd), p <2.89 * 10-14; 6. paranoia (Pa), p <2 * 10-16; 7. psychasthenia (Pt), p <2 * 10-16; 8. schizophrenia (Sc), p <7.04 * 10-16; 9. hypotension [mania] (Ma), p <1.95 * 10-10; lie scale (L), p <2.74 * 10-11; probability scale (F), p <4,66 * 10-8; correction scale (K), p <3,6 * 10-13. It is established that for the model of multinomial logistic regression, the purpose of which is to assess the probability of belonging of the respondent to one of the groups (value of the variable \"group.I\"), significant predictors are marital status, presence of children, family relations, inability to adapt to the environment. quality of sleep, low productivity, increased fatigue, smoking, assessment of their own health, the need to consult a doctor, the level of anxiety, depression, neuroticism. The coefficients of the model determine the magnitude of the change in the logarithm of the chance of being in a group (\"complex\", \"control\", \"standard\") against the (\"comparative\") group of healthy people, if the value of the predictor increases by one. The prediction accuracy in the general sample was 86.35%, and in the test sample (the main group, which included patients with NPR) - 88.52%. Conclusions. Thus, the test and ANOVA made it possible to identify those variables that are related to the variable \"group.I\". Such variables are: age, specialty, etc. (a total of 92). {"title":"New opportunities in treatment of patients with discirculatory encephalopathy: emphasis on nervous growth factor","authors":"","doi":"10.26565/2312-5675-2020-13-11","DOIUrl":"https://doi.org/10.26565/2312-5675-2020-13-11","url":null,"abstract":"The paper presents the results of research on the role of neuronal growth factors in the development and progression of cognitive and psychoemotional disorders. Peculiarities of Bacopa Monier and Ginkgo Biloba influence on structural and functional changes of the brain in the experiment and in certain groups of patients are shown. The results of treatment with phytocomplex (FC) Memostim® (fixed combination of Bacopa Monier - 150 mg and Ginkgo Biloba - 120 mg) of 30 patients with DE II grade due to atherosclerosis and hypertension are described. The control group consisted of 30 patients with grade II DE who were not prescribed FC Memostim®. After 3 months of treatment with FC Memostim®, a decrease in the frequency and severity of cephalic, vestibulo-atactic and asthenic syndromes was observed in patients. There was a significant improvement in cognitive functions (on the MoSA scale) and psycho-emotional state of patients. There was a significant improvement in the calculated operations and attention (by 22% relative to baseline, p <0.05) and the overall score on the test (+ 8%, p> 0.05). The general tendency to improve visual-constructive functions, memory, speech, executive functions, abstract thinking and orientation has been identified. Similar results of the effect of FC Memostim® on cognitive functions were obtained from the FAB questionnaire. According to the results of the survey of patients on the scale of quality of life, a significant positive dynamics of the integrative index (statistically significant increase by 31%), index of psychological well-being (increase by 32%), self-satisfaction (by 28%), indicators of physical well-being (by 18%) after 3 months of treatment with FC Memostim®. The level of neuronal growth factor (β-NGF) increased statistically significantly (by 67%). The analysis of the obtained data testifies to the effectiveness and safety of FC Memostim® in the treatment of patients with DE.
