Mallik","doi":"10.46347/jmsh.v8i2.21.206","DOIUrl":"https://doi.org/10.46347/jmsh.v8i2.21.206","url":null,"abstract":"Background: Every year in India about 7 million people suffer from burn injury, which is a leading cause of not only mortality, but morbidity and disability among the burn patients, leading to emotional trauma and stigma. Objectives: The present study was conducted in a peripheral medical college of West Bengal to describe the epidemiology, clinical presentation, and outcome in burn patients and also to identify contributing factors influencing the outcome in burn patients. Methodology: In this descriptive cross-sectional study, all consecutive burn patients admitted in Dept of surgery in 2018 were included. Data was collected regarding socio demographic profile, mechanism, type and degree of burn, total body surface area involved, time taken to reach health facility and outcome. Results: Out of 514 burn patients, majority were 20 to 60 years age group, females, Muslim in religion, from rural background, completed primary education, homemaker and having monthly income of the family below Rs 5000. Most of the burns were accidental, thermal in nature occurred at household level. About one third had burns involving dangerous areas of body. Average length of stay in hospital was 6.104 days with 20% case fatality and directly proportional with body surface area involved. Conclusion: Safety measures and building awareness can prevent burn injury at household level. Early initiation of treatment, infection control and following standard management protocol can avert mortality in tertiary setting. Rehabilitation and psychosocial care are urgently needed to combat disability. It is the second most frequent fusion abnormality of the kidney after the horseshoe kidney. The crossed kidney is horizontally fused to the lower pole of the normal kidney in the case of tandem kidney, which is a kind of crossed fused renal ectopia. A 33-year-old woman was diagnosed with pelvic inflammatory disease and a urinary tract infection. She was found to have left crossed renal ectopia with a Tandem kidney subtype during her ultrasonography assessment. Crossed fused renal ectopia may be asymptomatic, associated with other anomalies such as hydronephrosis, or represent a risk factor for renal neoplasms. It has been associated with genital and anorectal malformations. Fused renal abnormalities are of significance during abdominal surgeries such as malignant kidney excision, renal transplantation, or surgical management of aortic aneurysm. Here we have presented six cases of MIS-C with significant echocardiographic abnormality. They have a varied presentation like acute encephalitis, acute renal failure, incomplete Kawasaki, shock or acute gastroenteritis along with fever. Our cases have raised inflammatory markers like ESR, CRP, IL-6 with increased D-dimer, CPK-MB and positive COVID serology. Except case5, none has any cardiovascular presentation but every case has some important echocardiographic finding. Mild pericardial effusion was present in two cases, gross LV systolic dysfunction with LVEF 40-45% in one case, significant coronary artery dilatation in three cases and valvular abnormality like MR or TR in three cases. So it is extremely important to evaluate every case of MIS-C with proper echocardiography even if they had no cardiac manifestation at presentation to prevent complication afterwards. Anganwadi are the backbone of non formal education in rural India and place for inculcating healthy habits for children at an early age. Objectives: To assess the environmental sanitation of Anganwadi Centers, to describe personal hygiene of preschool children in Anganwadi and to assess personal hygiene practices of Anganwadi staff. Materials and methods: A cross-sectional study was conducted among Anganwadis located in the rural field practice area of a medical college, Bangalore over a period of two months. Anganwadi teachers, helpers and workers were interviewed through pre tested, semi structured questionnaire regarding their awareness on personal hygiene practices. Anganwadi children were examined for personal hygiene and morbidities. The Anganwadi centers environmental sanitation was assessed by observation. Results: 50 Anganwadi centers were visited. 45(90%) anganwadi center’s vessels and kitchen were kept clean. Lighting was adequate in 43(86%). 32(64%) Anganwadis had sanitary latrines. House hold purification of water was seen in 33(66%) anganwadi’s. Out of 1113 children registered, 689(61.9%) were examined. 479(69.5%) were found to be bare foot walkers, 55(7.9%) had poor face hygeine, 24(3.4%) poor hair hygeine, 95(13.78%) poor oral hygiene, 15(2.17%) poor feet hygeine, 51(7.4%) poor nail hygiene. Conclusions: The environmental sanitation of Anganwadi centre and personal hygiene status of children was not satisfactory. But the donor site morbidities have been reported with bone patellar tendon bone graft and hamstring tendon graft. The objective of the study is to evaluate the functional outcome of and donor site (ankle) morbidities with full thickness peroneus longus tendon (PLT) autograft in arthroscopic anterior cruciate ligament (ACL) reconstruction. Methods: A Prospective study was done in 30 patients with ACL injury. Knee stability and function were evaluated using the Lachman test, pivot shift test, International Knee Documentation Committee score (IKDC) and Lysholm knee score. Donor ankle was assessed using American Orthopedic Foot and Ankle Score (AOFAS) and grading muscle power by Medical Research Council scale. Data collected preoperatively and postoperatively at 6 & 12 months were compared. Results: The IKDC score was normal or nearly normal in 24 patients and abnormal in 6 patients. The mean Lysholm score had excellent or good results in 24 patients. The Lachman test showed normal findings in 24 patients, 6 patients had 1+ laxity. The pivot-shift test was negative in 22 patients and 8 patients had 1+ glide. The mean AOFAS score was 100±0.0 (preoperatively) and 96.0±9.6 (at final follow up). No patient experienced ankle pain and instability. Conclusion: PLT autograft gave painless stable knee, with a good range of motion. It didn’t have any morbid effect on ankle and gait parameters. So, PLT autograft can be suitable graft with respect to its strength, safety and donor site morbidity. Out of 40 different cell types of lungs, Type‑1 and Type‑2 pneumocytes, pulmonary capillary endothelium, and cells of lung interstitium play a vital role in functioning of the lung. Exploring the structure of these cells would shed light on the pathogenesis of various diseases affecting the lungs. A systematic literature search was conducted using three research databases – Medline, Google Scholar, and Cochrane. Out of ninety articles published between the year 1981 and 2020, 41 articles were included in the study based on the inclusion and exclusion criteria. Among the cellular components of the alveolar cell complex of the lung, the Type‑2 pneumocytes show versatility in its function in the form of surfactant synthesis, immune reaction, and regenerative capacity. The pathogenesis of the lung injury is due to the loss of functions of the Type‑2 pneumocytes and the intercellular junction functional discrepancy. Irrespective of the cause of lung disease, there is an increase in permeability in the alveolocapillary membrane leading to interstitial edema. This, in turn, leads to hyperventilation, low oxygen saturation, and a decrease in lung capacity. The pericytes present in the interstitium have a preventive role to play in lung injury and potential stem cell function. Currently, this differentiation is done with the help of biochemical analysis of the aspirated fluid after thoracentesis, which is an invasive procedure. Objective: To evaluate the utility of CT attenuation values in pleural effusion and define a threshold value to differentiate between exudative and transudative pleural effusion. Materials & Methods: A cross sectional study was done on 130 patients showing pleural effusion on CT thorax, mean attenuation was calculated using nine regions of interest on three slices. Within 48 hours of imaging, biochemical assessment was done to decide exudative or transudative nature of fluid as per Light’s criteria. A Receiver operating characteristic curve was drawn to assess the cut off CT attenuation value for distinguishing between exudative and transudative effusion and evaluate its accuracy. Result: Mean CT attenuation value of 57 exudative effusion cases was 5.9 + 6.48 HU, which was higher than 73 cases with transudative pleural effusion (2.97 + 2.69 HU). The difference was statistically significant (p value < 0.0001). The ROC curve revealed a cut off value of 7.5 HU. When the mean CT attenuation value was > 7.5 HU, the specificity and sensitivity of CT scan for diagnosing exudative effusion was 98.6 % and 59.6% respectively. Area under the curve was 0.79, which revealed moderate accuracy of this threshold value.
