Calcium pyrophosphate deposition disease is defined by deposition of calcium pyrophosphate crystals in hyaline cartilage, fibrocartilage and soft tissues. The study of bone markers is of great interest in different types of arthritis. In patients with calcium pyrophosphate deposition disease, bone turnover markers may yield additional information from many perspectives. Objectives. To measure synovial and serum levels of osteocalcin, osteoprotegerin and β-CrossLaps in patients with calcium pyrophosphate deposition disease. Material and methods. In this pilot, prospective study were enrolled eight patients with calcium pyrophosphate deposition disease. Osteocalcin, osteoprotegerin and β-CrossLaps were measured in serum and in the synovial fluid using ELISA kits. Results. Synovial fluid osteocalcin was significantly lower comparative with serum osteocalcin (2.1±0.6 versus 9.21 ±46, p=0.02). Synovial fluid osteoprotegerin was significantly higher than serum osteoprotegerin (55.5±284 versus 8.44±7; p=0.0001). Synovial fluid β-CrossLaps wasn’t statistically different from serum levels (6146±66818 versus 6077±12212; p=0.5). Ratios between synovial fluid and serum levels were 0.22 for osteocalcin, 6.57 for osteoprotegerin and 1.01 for β-CrossLaps. Conclusions. Osteocalcin was higher in serum than in the synovial fluid. β-CrossLaps had similar values in serum and synovial fluid. Horton’s disease is the most common form of vasculitis that occurs in patients older than 50 years. Due to its unusual clinical manifestation, the disease is often misdiagnosed and managed inappropriately. Early diagnosis and prompt initiation of treatment are essential to reduce the risk of severe neuro-ophthalmic complica- tions. Case presentation. We present the case of a 71-year-old woman, with a family history of autoimmune disease, who complains of bilateral shoulder and pelvic girdle pain, low fever, jaw claudication and amaurosis fugax. After the initial tests that have been made in other departments, the patient has been diagnosed with iron deficiency anaemia and treated with iron tablets. Because of the lack of improvement in the clinical manifestations and unresponsive anaemia to oral iron treatment, the patient was admitted to our rheumatology department 5 months later. The anamnestic, clinical and paraclinical tests results along with the presence of the diagnosis criteria, allowed us to establish the Horton’s disease diagnosis. The patient immediately received corticosteroid therapy having a prompt clinical and paraclinical response. Conclusion. Even if sometimes it can be hard to reach the diagnosis, every effort should be made to achieve it. We aimed to evaluate the prevalence and differences of clinical and ultrasound involvement of ankles in rheumatoid arthritis (RA) patients in remission. Methods. RA patients were recruited in 2018 in the random order of presentation from the out-patient clinic. In the day of enrolment, all patients underwent clinical examination (by a senior rheumatologist, blind to other evaluations), inflammatory markers (e.g. C-reactive protein – CRP) and ankle ultrasound (performed and interpreted by a single rheumatologist with more than 7 years of experience). Results. The sample included 59 patients in DAS28CRP remission, 27 in SDAI remission and 20 in Boolean remission. Among these 3 categories of remission, overall clinical ankle involvement presented similar prevalence (revolving around 30%). On average, 77% of patients presented overall ultrasound ankle involvement among the 3 categories of remission, with similar frequencies of ankle joint synovial hypertrophy (SH; 33%), power Doppler (PD) positive joint SH (15%), ankle joint effusion (60%), ankle tenosynovitis (27%) and PD positive ankle tenosynovitis (15%). In the subgroup of RA patients in DAS28CRP remission, compared to patients without clinical or ultrasound ankle involvement, those with at least one involved ankle had a significantly higher median CRP (0.34 mg/dl versus 0.19 mg/dl, p = 0.042). Conclusion. Among RA patients in remission, regardless of its definition, clinical and ultrasound involvement of ankles is frequent. The purpose of this paper is to contribute to the literature by analyzing the top 50 cited articles on fibromyalgia with bibliometric methods. Material and methods. First of all, fibromyalgia keyword was searched on PubMed and Google Scholar, and 50 articles with the highest citation index were selected from among the results. We then manually reviewed the contents of these articles by examining characteristics such as the number of citations, authorship, article title, year of publication, and topic categories. Results. The top-50 cited articles were published in 18 journals. The most frequently cited journal was Arthritis and Rheumatism (16 articles) and the most common topic subject was treatment (17 articles), followed by pathophysiology (12 articles). The 50 articles were reported from 8 different countries and the USA became the leading country with 38 articles published on this topic . The 50 articles we analyzed were originated from 20 different clinics and the clinic with the highest number of publishing became Department of Rheumatology, University of Kansas School of Medi-cinewhich is situated in US (5 articles). Conclusion. The present study has produced a detailed list of the most-cited articles on fibromyalgia, which is the first such study in this field. Among them, the urinary tract involvement is rarely cited, most data focusing on bladder dysfunction due to neuroBehçet. This article presents a rare case of a young female patient with urological complaints that was diagnosed with right ureteral ulceration, later confirmed as vasculitis at the histopathological examination. Urological intervention together with adequate immunosuppression let to the healing of the ulcerative lesion. There clinical benefit is similar to the oral form of administration, but with significant less systemic adverse events. There are many forms of topical administration such as gels, cream, spray, patches. The newly topical patch of ibuprofen is an option. The mean level of ibuprofen delivered from the topical patch is consistent with the levels required for therapeutic relief and this dose is much lower that the dose required for oral dosing. Conclusion: In our series, the most frequent non-ASSD profiles associated to ARS positivity was DM/PM, SSc and Sjögren ́s syndrome. This, considering the most frequently associated non-ARS, the most frequent non-ASSD clinical manifestations and that the most frequent non-ASSD diagnosis diagnoses or profiles described by the Rheumatologist. Pulmonary arterial hypertension (PAH) is an important cause of morbidity and mortality in patients with systemic sclerosis (SSc). This condition is diagnosed by cardiac Doppler ultrasonography, right-heart catheterization, or by serum levels of N-terminal pro-brain natriuretic peptide (NT-proBNP). The aim of this study was to assess the association between serum NT-proBNP and nailfold capillaroscopic patterns in SSc patients. We also analyzed the association between pulmonary function parameters, NT-proBNP, and nailfold capillary findings in patients diagnosed with SSc and PAH. Material and methods. We retrospectively analyzed SSc patients and healthy controls from our institution between July 2016 December 2018. We assessed by chart review: pulmonary artery systolic pressure (PASP), forced vital capacity (FVC), forced expiratory volume in 1 sec/forced vital capacity ratio (FEV1/FVC ratio), the number of nailfold capillaries/mm and NT-proBNP. Statistical analyses were performed using the Student’s t-test, ANOVA test and the Pearson’s correlation. Results. Seventeen patients with SSc and 17 healthy controls matched for age and gender were included. Among SSc patients, 13 had diffuse cutaneous SSc (dcSSc) and 4 patients had limited cutaneous SSc (lcSSc). PAH was identified in 10 SSc patients. In SSc patients, significant correlations have been identified between PASP and NT-proBNP (r=0.9, p<0.0001), nailfold capillaries density and PASP (r=-0.95, p<0.0001), and nailfold capillaries density and NT-proBNP (r=-0.84, p<0.0001). Conclusion. Not all risk calculators adjust for RA status, yielding discording results. A 56-year-old woman with RA presented for bilateral pain and swelling in the metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints. She was diagnosed with RA 10 years ago, currently treated with methotrexate (MTX), sulfasalazine, and hydroxychloroquine. She has a history of type 2 diabetes mellitus, a total abdominal hysterectomy with bilateral salpingo-oophorectomy for an epidermoid carcinoma of the cervix, and surgical excision of a pulmonary rheumatoid nodule. Multiple subcutaneous nodules are seen bilaterally on the MCP and PIP joints. MTX may be associated with nodulosis in RA patients, which in turn is related to a further increase in cardiovascular risk compared to RA alone. MTX was discontinued. Abatacept was the biologic of choice, due to recent evidence suggesting superior efficacy in decreasing cardiovascular risk compared to anti-TNF therapies, especially in patients with diabetes and with positive rheumatoid factor.
