It remains an important cause of death among young children globally, despite the availability of a safe and effective vaccine. Measles transmitted via droplets from the nose, mouth or throat of infected persons. Objective: To evaluate the immune response to measles infection among immunized school aged children, and to detect the cluster of differentiation 46 single nucleotide polymorphism (CD46 SNP) in association with the immune response. Methods: The current study is a cross sectional study including 158 hospitalized patients were previously vaccinated with two doses of measles-mumps-rubella vaccine. The ages of patients were 5-10 years school aged children. All samples were collected from blood collection unit in Al-Imamein Al-Kadhimein Medical city during the period from December 2018 to April 2019. The detection was based on the presence of IgG Antibody to measles. The positive results were considered according to enzyme linked immunosorbant assay and conventional polymerase chain reaction to detect CD46 SNP. Results: Among those 158 subjects' male children count for 62% and female children were 38%. Forty-one (41) immunized children (with mean age 7.98±1.92 years) were low immune response to measles vaccine (MV). On the other hand, only two cases (with mean age 5.50±0.71 years) were negative to measles virus vaccine. Conclusion: Ensuring two doses of MV give protective immune response to MV may declines with aging. CD46 cellular receptor gene SNP (rs7144) may play role in reduction in immune response to MV. Objective:To detect human herpes virus 6 (HHV-6) in the saliva samples of patients in the early presentation of BP. Methods: A case-control study included saliva samples taken from 50 patients with BP and 50 apparently healthy controls without any neurological deficit, viral DNA was extracted from the saliva and then subjected for quantitative real time polymerase chain reaction (PCR) for detection of HHV-6 viral load (VL) in saliva. Results: HHV-6 DNA was positive in 22 (44%) out of the 50 patients, and in 12 (24%) of the controls (p=0.028), with a significantly higher mean viral load in patients than control (p=0.002), in addition, 70% of HHV-6 positive patients had severe grades with a significantly higher VL (p=0.007) and (0.015), respectively. Conclusion: HHV-6 could play an important role in the development of BP and HHV-6 might have a neuro-pathological effect in severe cases of BP. Materials and methods: Eighty-nine patients with type 2 diabetes mellitus (T2DM) were divided into two groups of controlled (HbA1c < 7%) and uncontrolled (HbA1c > 7%) diabetics. Recent laboratory data were used to collect HbA1c (glycated hemoglobin) levels, leukocyte count (WBC), hemoglobin (Hb), hematocrit (Hct), red blood cell distribution width (RDW), neutrophils, lymphocytes, and monocytes. The NLR and MLR were calculated from the laboratory data. Results: The mean age in controlled and uncontrolled diabetics was 58.30 and 55.62 years, respectively. The mean NLR in controlled and uncontrolled diabetics was 2.61 and 4.88. The difference was found to be statistically significant ( p < 0.05). The mean MLR in controlled and uncontrolled diabetics was 0.2 and 0.24, but the difference was not statistically significant ( p > 0.05). A weak positive correlation was found between HbA1c levels and the hematological indices and the results were statistically insignificant. Conclusion: The study yielded significant results in the difference between controlled and uncontrolled diabetics with respect to NLR. Although there was only a weak positive correlation found between glycated Hb levels and the hematological indices, the results showed that there is a significant difference in the NLR between the two groups. Cigarette smoke delivers a high concentration of oxidizing chemicals and exposure to these chemicals is associated to depletion of endogenous levels of antioxidants, which contribute to a number of the potential mechanism of cardiovascular disease. Altered blood coagulation, impaired integrity of the arterial wall, and changes in blood lipid and lipoprotein concentrations predisposes to coronary artery disease (CAD) in smokers. Aims and objective: To compare lipid profile in smokers and nonsmokers. Determine dose-dependent relationship, durational significance between smoking and lipid profile among smokers. Evaluation for CAD in both smokers and nonsmokers. Materials and methods: The study was carried out in 100 healthy male smokers and 100 healthy male nonsmokers selected from volunteers from RajaRajeswari Medical College, Bengaluru. All patients were subjected to fasting lipid profile, cardiac evaluation by doing ECG/ECHO/ TMT in smokers and nonsmokers. Results: The mean duration of smoking was 6.6 years. The mean level of total cholesterol, LDL cholesterol, and triglycerides is significantly increased ( p value > 0.001), whereas the mean level of HDL cholesterol is significantly low ( p value > 0.001) in smokers compared to nonsmokers. There is a direct relationship exist between the severity of smoking and an increase in serum lipids. We have observed ECHO and TMT changes in smokers who have smoked for a duration of > 6 years. Conclusion: We have observed that there is a direct relationship exists between the severity of smoking and an increase in the serum lipid profile levels as well as greater risk for the development of coronary heart disease. Diabetic foot disease (DFD) is one of the debilitating complications caused by DM and is the leading cause of non-traumatic lower limb amputation. The majority of diabetic patients are not properly educated about foot disease and foot care. In this study, we tried to assess the awareness about foot-related problems among patients with DM visiting tertiary healthcare centers. We also assessed the practice of foot care among patients with diabetes. Materials and methods: This cross-sectional study was carried out in patients with DM attending tertiary healthcare centers after Ethical committee approval. The study was conducted in outpatient departments of endocrinology at tertiary care hospitals, in Bengaluru, using a predefined questionnaire. The results were analyzed using appropriate statistical methods. Results: One hundred and twenty-three patients with DM were recruited and given the questionnaire. The mean age of patients was 52.57 ± 12.45 years and the mean duration of diabetes was 11.7 ± 8.63 years. There were 40 male patients (32.5%) and 83 female patients (67.5%). Ninety people were from the urban (73.17%) and the remaining 33 people were from the rural area (26.83%). Out of 123, only 102 patients (82.5%) were aware that diabetes causes foot problems. Though most of them (72.5%) knew that uncontrolled diabetes can cause wound/ ulcer, and 33% were aware that uncontrolled diabetes can lead to gangrene. Only 15% of patients were aware that DFD can lead to lower limb amputation. Forty-six patients (37.5%) were aware that smoking increases the incidence of DFD and 58 (47.5%) know that previous foot disease can increase the risk of foot problems in the future. Forty-three patients (35%) were aware that the presence of foot problems increases the risk of heart disease in diabetic patients. Eighty-six subjects (70%) were aware that there is specially customized footwear available for diabetic patients. One hundred and ten subjects (89.4%) were aware that good glycemic control from the beginning helps prevent diabetes-related foot problems. Only 49 subjects (39.8%) were taking care of their feet, of which only 18 patients examined and took care of the feet daily. About 120 people in the study used footwear outdoor (97.5%) and only 23 people (18.6%) used footwear both outside and inside the house. Three people in the study did not use footwear even outside the house. Conclusion: It is vital to educate the patients about foot disease and foot care as a part of diabetes management. According to the origin of the tumor, there are three types of cutaneous leiomyomas: piloleiomyoma, angioleiomyoma, and genital leiomyoma. We present an uncommon case of cutaneous leiomyoma which, due to clinical similarities, had been previously misdiagnosed and managed as keloid. Case description: A 26-year-old woman with Fitzpatrick skin type 4/5 visited the outpatient department of dermatology for the treatment of a painful skin lesion on the left shoulder. The symptoms were not improving from previous treatment modalities, where she was diagnosed as having a keloid. After examination we sent for a skin biopsy. The specimen was sent for histopathological and immunohistochemical investigations, which revealed cutaneous leiomyoma. Conclusion: Cutaneous leiomyoma may be misdiagnosed as a keloid. The former is of clinical relevance as they may denote hereditary leiomyomatosis and renal cell cancer. The outer surface of each particle is covered with universal adaptor molecules having attachment points for antigens, specific molecules on specific cells, and fight off the targeted disease. Inside of each particle, there is either cytokines, cytotoxic drugs, antimicrobial drugs, genetic material, iron, gold, herbs, and others; each for different curative purpose, yet all of them act locally with high specificity to avoid devastating side effects of the contents if given systemically or to target certain tissue for curing diseases due to genetic deletions, or as a vaccine. Genetic factors are thought to be a major component for the development of T1D. The studies on the risk of developing T1D suggesting that the environmental factors, particularly viruses may be implicated in the initiation of beta cell destruction leading to T1D. Objective:To investigate the possible relationship between Epstein-Barr virus and T1D. Methods:The sera were collected from 56 T1D patients and 30 controls of age range 3-22 years old and from both sexes. The sera divided into two parts, one part for serology for detection anti-EBV EBNA-1 IgM and IgG antibodies by enzyme linked immunosorbent assay (ELISA) technique and the other for viral genomic extraction and conventional polymerase chain reaction (PCR) to detect the viral target gene. Results:The results by ELISA technique indicated that only 7 (12.5%) of T1D patients were positive for anti-EBV IgM antibody and 24 (42.9%) of T1D patients showed positive results for anti-EBV IgG antibody. In contrast, the control group showed negative results for both anti-EBV IgM and IgG antibodies. The results of PCR technique revealed that 15 (26.79%) of T1D patients have EBV DNA compared with none of the controls have EBV DNA (P<0.001). Al-Humrani","doi":"10.22578/ijms.17.3-4.2","DOIUrl":"https://doi.org/10.22578/ijms.17.3-4.2","url":null,"abstract":"Background:The incidence of mortality and complications are high in patients with acute inferior wall ST-segment elevation myocardial infarction with right ventricular involvement, which has been reported to be an independent predictor of significant complications and in-hospital mortality. Objective:To investigate the feasibility of using electrocardiographic changes in inferior myocardial infarction represented by ST-segment elevation ratio in lead II and III as a predictor of right ventricular infarction and in-hospital morbidity and mortality. Methods:Ninety-nine patients were studied in this prospective study, their ages ranged from 19-90 years, average 58.12 (±12.7 SD). They were presented to the Coronary Care Unit of Basrah General Hospital with acute inferior ST-segment elevation myocardial infarction. The 12 leads plus right-sided precordial electrocardiograms were done to all patients within 12 hours of the onset of symptoms, and ST-segment elevation was measured. ST-segment elevation in lead III exceeding lead II was defined as a ratio of elevation in lead III: II>1. Patients grouped according to ST-segment elevation III:II ratio into either >1 or ≤1. In-hospital morbidity and mortality were studied in both groups. Results:ST-segment elevation ratio > 1was detected in 68 patients (68.7%) with acute inferior myocardial infarction at time of admission. Right ventricular infarction was diagnosed in 33 (33.3%) patients, with the majority (32 patients) have ST-elevation ratio > 1. Thirty-Six patients had at least one of the in-hospital complications with significantly higher incidence (51.4%) in patients with higher ST elevation ratio. The mortality was statistically higher when ST segment elevation level in the lead III > than that in the lead II. Conclusion:In patients with inferior STEMI, ST-segment elevation in the lead III more than lead II can be a potential marker of the presence of right ventricular infarction in association with inferior myocardial infarction. Short-term prognosis is possibly worse in the presence of a higher ratio between lead III and II ST-segment elevation. However, further studies are needed to validate this conclusion. The association and role of CMV and inflammatory bowel disease (IBD) exacerbation is still controversy. Objective:To investigate the rate of occurrence and role of HCMV in patients with IBD including demographic and clinical features. Methods:A cross sectional study involved sixty-five (65) IBD patients whom divided into 9 Crohn’s disease patients and 56 ulcerative colitis patients. The detection of local CMV reactivation (colon) was based on the presence of early and immediately early antigens (nonstructural proteins). The positive results for HCMV reactivation was considered according on Immunohistochemistry (IHC) and/or serological enzyme linked immunosorbent assay (ELISA) results. Results:Among the 65 eligible IBD patients, nine patients (13.85%) gave positive result for IHC, compared to 56 patients (86.15%) with negative result. On the other hand, only two patients (3.08%) had a positive result for anti-HCMV IgM antibody, while almost all patients (except one) were positive for anti-HCMV IgG antibody. There was a significant difference between positive HCMV and patients with long duration of IBD and non-response to treatment. Conclusion:IBD and its treatment may put those patients at risk of HCMV reactivation. Colonic active CMV was detected particularly in sever UC patients and significantly in patients with disease duration above 5 years and not response to treatment based on IHC technique for IE and E HCMV proteins detection.
