However, although the incidence of CVD is higher in obese humans, numerous studies have found that obese individuals with CVD have a better prognosis than lean individuals, a phenomenon known as the “obesity paradox” (1). Adipose tissue has a well-defined role as an endocrine organ that differs by anatomical location [e.g., inguinal white adipose tissue (WAT), perigonadal WAT, interscapular brown adipose tissue (BAT), etc.] as well as adipocyte cell content (white vs. brown vs. beige), and recent work has focused on the adipose-cardiac signaling axis to decipher the endocrine mechanisms of adipocytes. We and others have recently reviewed adipose tissue-derived signals that are known to directly impact the myocardium (2–4). Some of these adipose-derived endocrine signals are mediated by small extracellular vesicles (sEVs) that can deliver diverse cargo, including RNA, protein, and lipids (4). However, the cardiac effect of adipose-derived sEVs is not always consistent. Multiple recent studies describe EV-mediated delivery of specific microRNA (miRNA) from adipose-derived stem cells that impact mouse models of cardiac ischemic injury (5–9). BAT-derived EVs blunt obesity-induced declines in cardiac function, but their delivered EVs were shown to accumulate mainly in the liver, spleen, and lungs, and the direct effects on the myocardium were not investigated (10). Conversely, EVs secreted from WAT in response to PPARγ activation induce hypertrophic signaling in neonatal cardiac myocytes via transport of miRNA (11). sEVs isolated from epicardial adipose tissue were associated with atrial fibrillation in humans, were sufficient to induce cardiac fibrosis when injected into rats, and implicated sEV-mediated delivery of pro-fibrotic miRNAs as a potential mechanism (12).","PeriodicalId":33060,"journal":{"name":"ExRNA","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://ftp.ncbi.nlm.nih.gov/pub/pmc/oa_pdf/47/e4/nihms-1774408.PMC8865429.pdf","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"39959237","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Stroke remains one of the leading causes of disability and death worldwide (2) and is predominantly due to thromboembolic occlusion of a major artery feeding the brain. While the initial management of stroke has understandably focused on clotting as manifest in the 1996 FDA approval of tissue plasminogen activator (tPA), often known as a “clot-busting” agent, there has been increasing recognition that other factors including inflammation are important determinants of outcome. In the new issue of Circulation Research (3), we learn that the complement system activated through exosomal signaling may also play a critical role after ischemic stroke. Age-associated increases in circulating inflammatory mediators have been suggested as contributors to the development of stroke and other neurological disorders (4). This notion has been supported by data showing that exposing young mice to plasma from aged mice led to cognitive functional decline (5). In contrast, heterochronic parabiosis experiments, in which the circulations of mice of different ages are connected, demonstrate that neurogenesis in aged mice can be restored by exposure to youthful circulation (6). However, the mechanism by which age-associated inflammatory mediators influence stroke remains elusive.","PeriodicalId":33060,"journal":{"name":"ExRNA","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://ftp.ncbi.nlm.nih.gov/pub/pmc/oa_pdf/24/f7/nihms-1769490.PMC9585525.pdf","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"40581453","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Patients that present with advanced stage cancers have a poor survival. Detection of HCC at early stages can enable the use of curative treatment options and improve long-term survival. The current practice for surveillance for HCC relies on abdominal ultrasound with or without alpha-fetoprotein (AFP) and has limitations. Ultrasound performance can be dependent on patient access, presence of obesity or operator experience. Consequently, the sensitivity of these approaches for the detection of early stage HCC detection is very low (2). While single or individual biomarkers have shown high sensitivity and specificity for HCC, their performance for the detection of early stage disease is unknown. In order to reduce the impact of HCC, there is a need for more effective methods for early detection of HCC (3).","PeriodicalId":33060,"journal":{"name":"ExRNA","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://ftp.ncbi.nlm.nih.gov/pub/pmc/oa_pdf/01/1b/nihms-1769485.PMC9307225.pdf","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"40621954","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
