Accurately diagnosing peritoneal pregnancy before surgery is difficult; thus, gynecologists may encounter the disease accidentally. Many studies have reported that peritoneal pregnancy is preoperatively diagnosed as tubal pregnancy. The implantation sites varied in each case. In some cases, the implantation sites can be close to other organs such as the rectum, ureter, and bladder. Thus, it is important for operators to take measures to not injure the organs. Here, we report a case of peritoneal pregnancy implanted on the pouch of Douglas inside of the right uterosacral ligament. The gestational sac did not develop in the abdominal space but developed in the retroperitoneal space; thus, it should have been near the rectum. When laparoscopy was initiated, the positional relationship between the rectum and gestational sac was unclear. We inserted a reusable surgical probe (Rectal Sonde ® ) into the rectum and moved it slowly and carefully and assessed the positional relationship. We safely removed the gestational sac. A reusable surgical probe for the rectum has been reported to be useful for laparoscopic adhesiolysis of the posterior cul-de-sac obliteration associated with endometriosis. Precise sutures are important to ensure the safety of postoperative pregnancy and delivery. To acquire suturing and ligation techniques, a dry box is often used. However, it is different from real surgery. We report the results of using a three-dimensional (3D) model made from materials such as real objects for training. Methods: Bio-Texture Modeling technology by FASOTEC Corporation is already applied to other fields. We collaborated with FASOTEC and produced a 3D model uterus that is constituted of a myoma and the myometrium. Results: We used a 3D model to dissect the myoma and the myometrium and sutured them. Subsequently, we could evaluate our suturing techniques by making sure whether there is dead space. Furthermore, we could reuse this model by cutting the sutures multiple times. Additionally, since this model reproduces the real texture of hardness and viscosity by Bio-Texture Modeling, it is worthwhile to simulate in-bag morcellation. Conclusions: Laboratory animal training is used as a method of laparoscopic surgery simulation. Although it is good for training in dissection, vessel ligation, hemostasis, and internal genitalia, the model is not similar to humans. Furthermore, there are many problems, such as animal protection and high costs. Mucoceles of gastrointestinal origin must be differentiated from ovarian tumors. Atypical, proliferative mucinous tumors and well-differentiated intestinal type mucinous adenocarcinoma may arise in a mature cystic teratoma. Pseudomyxoma peritonei has a poor prognosis, and difficulty in preoperative differential diagnosis of ovarian tumors and mucocele is an important factor. We experienced 2 cases of pseudomyxoma peritonei diagnosed at laparoscopic surgery after preoperative diagnosis of ovarian tumor. In case 1, the tumor was on the posterior rectum, and the distance with the uterus had been observed by MRI. At surgery, the ovaries were normal, and the rectal origin of the tumor was confirmed. In case 2, the CEA was elevated, but there was not a solid portion of ovarian carcinoma suspicious for tumor, and the tumor was diagnosed as appendiceal in origin. Both cases had presented atypical shapes for ovarian tumor on MRI. Because the differential diagnosis of mucocele is difficult, gynecologists sometimes encounter them at surgery. However, repeated abdominal surgery can be a burden for the patient both physically and psychologically. A 45 year old nulligravida underwent a left abdominal salpingo-oophorectomy due to an ipsilateral ovarian cystic tumor; the tumor was subsequently diagnosed histologically as a granulosa cell tumor (GCT). She was referred to our hospital due to an incidentally-detected right ovarian cystic tumor one year after the first surgery. A laparoscopic right salpingo-oophrectomy and hysterectomy was performed out because a recurrent GCT was suspected. However, the right ovarian cyst was found to be benign by histologic diagnosis. One year and 10 months after the second surgery, a solid peritoneal tumor was found on the right abdominal wall. The tumor was well-demarcated, and was resected laparoscopically. One year and four months after the third surgery, a well demarcated tumor was detected anterior to the rectum. The tumor was located at the vaginal cuff, and was resected laparoscopically. The third and fourth tumors were histopathology confirmed as a GCT recurrence. She is currently undergoing three consecutive chemotherapy cycles with pepleomycin, etoposide, and cisplatin. It is known that ovarian GCT can recur repeatedly over a period of years, and the standard treatment is surgery. Therefore, recurrent GCT patients often endure multiple surgical procedures and are exposed to the risk of acute and chronic surgical complication. Patients with ovarian tumor who underwent surgery in pregnancy from 2004 to 2008 were reviewed.Design  During these 5-years, there were 3,837 deliveries in our hospital, and 16 of which underwent surgery due to an ovarian tumor. A retrospective analysis of these cases was performed.Results  Laparoscopic surgery was performed in 9 patients, while the remaining 7 patients underwent open abdominal surgery. The tumor was determined to be malignant in 2 patients. The mean patient age at the time of surgery was 31.4 years, and the median number of gestational age at surgery was 16 weeks. The median value for the maximal tumor diameter was 9 cm. Overall, among the patients with ovarian tumors complicating their pregnancy, the pathological diagnosis was a mature cystic teratoma in 9 patients, which was the predominant type, followed by an endometriotic cyst in 3 patients and a malignant tumor in 2 patients. These malignant tumors consisted of an immature teratoma (G2) and a yolk sac tumor, and these patients are currently undergoing chemotherapy.Conclusion  Patients should be examined for the possible presence of an ovarian tumor early in gestation. Design: Case report. Patient: A 44-year-old woman, who was previously diagnosed with stage IA1 cervical cancer following conization, was further diagnosed with multiple pelvic lymph node recurrences by laparoscopic surgery following computed tomography and [18F]-fluorodeoxyglucose-positron emission tomography-computed tomography. Conclusion: Lymphatic recurrences are rare in stage IA1 squamous cell carcinoma of the cervix; therefore, patients with low-risk features could be safely followed-up after conization. When early-stage lymphadenopathy is identified in a patient, the laparoscopic approach might be a viable option for the diagnosis of pelvic lymph node recurrence.
