Alahdal","doi":"10.4197/MED.25-2.2","DOIUrl":"https://doi.org/10.4197/MED.25-2.2","url":null,"abstract":"The study aims to evaluate safety and efficacy of Al-Hijama (wet-cupping) either as an adjuvant or an alternative to conventional medication therapy among adult patients with sinusitis. A randomized controlled clinical trial was conducted at Prophetic Medicine Research Cupping Clinic, King Abdulaziz University Hospital, Jeddah Saudi Arabia. The study used a simple randomization technique. Sixty (60) patients of chronic or recurrent acute sinusitis were divided equally into 3 groups. The first group had Hijama with drug therapy, second had Hijama plus salt nasal solution and mild analgesic if needed, and the third was treated with ordinary medicine regularly. The effect of Al-Hijama on sinusitis symptoms, use of medications, were studied. The results showed that both alternative and adjuvant Al-Hijama therapies were superior to conventional therapy with regard to the reduction of symptom frequency, severity, duration and use of medication. Al-Hijama therapy was effective, simple, affordable, acceptable, and safe when used correctly. They are the largest family of receptors with almost 800 genes in humans. Different types of ligands can activate these receptors, such as catecholamines, nucleotides, lipids, and gut microbiota, where some ligands could be bitopic. Nevertheless, some receptors have internal ligands bound to them. Activated G protein-coupled receptors have complex signaling pathways that are involved in almost all bodily functions. Furthermore, they constitute a large percentage of Food and Drug Administration marketed drugs and global share of drugs, in addition to a great proportion of drugs currently in clinical trials targeting these receptors. The approved G protein-coupled receptors targeted drugs and potential drugs are involved in the management of many diseases including cancer, inflammatory diseases, diabetes mellitus, hypertension, obesity, pain, and diseases of the central nervous system. Only 10% of G protein-coupled receptors are targeted. Different pharmacological approaches have been considered in drug discovery of these receptors including polypharmacology, allosteric modulators, biased agonism, tethered agonism, and pharmacogenomics. Advances in the technologies are promising to help in the discovery of new targets. It is characterized by high temperature, extrapyramidal symptoms, autonomic variability, and changes in consciousness. Although olanzapine is an atypical antipsychotic, which exhibits more affinity to serotonin receptors than to dopamine receptors, atypical antipsychotics (like olanzapine) can cause \"typical and atypical\" forms of neuroleptic malignant syndrome. Previous reports of neuroleptic malignant syndrome associated with olanzapine have described patients who had been previously treated with other antipsychotic medications, patients who had previous incidents of neuroleptic malignant syndrome caused by other neuroleptics, or patients treated with olanzapine and another neuroleptic. This article describes a case of neuroleptic malignant syndrome linked to olanzapine in a male adolescent, 17-year old, who had not previously been treated with antipsychotic medications. Demographic and clinical features, drugs, rheumatoid factor-positivity, and anti-cyclic citrullinated peptides-positivity were recorded for 205 Saudi rheumatoid arthritis patients (185 females; mean age was 45 years and mean disease duration was 5 years). Anti-cyclic citrullinated peptides and rheumatoid factor were assessed in serum. Disease activity scores for 28 joints was used. There were 36% rheumatoid factor+ve and 45% anti-cyclic citrullinated peptides+ve. 21.5% of the rheumatoid factor-ve subjects were anti-cyclic citrullinated peptides+ve. 13.3% of the rheumatoid factor positive patients were anti-cyclic citrullinated peptides-ve and 86.7% were anti-cyclic citrullinated peptides+ve. Significant association (P < 0.05) of anti-cyclic citrullinated peptides-positivity and rheumatoid factor-positivity with each other, and with gender, use of disease–modifying antirheumatic drugs, hydroxychloroquine and methotrexate. No direct impact of anti-cyclic citrullinated peptides status on the disease activity scores for 28 joints or its constituents (P > 0.5); nevertheless, anti-cyclic citrullinated peptides positive patients appear to represent a greater need for combination disease modifying drugs. It is a potentially fatal complication of untreated chronic constipation. Presenting symptoms and signs are variable according to the site of the perforation. A 60-year-old male patient presented to the emergency room with dizziness and decreased level of consciousness. He was admitted for management under care of medical team as a case of urosepsis. Two days after admission he developed picture of Fournier gangrene. The underlying cause was found to be rectal perforation. Stercoral perforation is an emergency that can be missed if not highly suspected. We present a case of stercoral rectal perforation with picture of urosepsis and Fournier gangrene as presenting symptoms.
