Multi-detector computed tomography (MDCT) CT angiography (CTA) is currently the gold standard imaging modality used to identify the site and cause of bleeding in patient presented with haemoptysis. Bronchial artery anatomies and precise location can be obtained by scrutinizing CTA prior to interventional procedures. CTA has the advantage of not only can preclude the need of digital subtraction angiography (DSA) in inappropriate cases, but also can shorten the intervention procedure timing. More commonly observed in children, foreign body aspiration is less common in adults. Traditionally observed amongst Indian women, nose piercing and jewelleries are gaining popularity in various communities for the past decade. Aspiration of tiny nose jewelleries such as nose stud screws is a serious and potentially fatal event. Small foreign bodies may dislodge further into the subsegmental bronchus, rendering bronchoscopic retrieval technically challenging. Herein, we report the first 2 cases of nose stud screw aspiration. Both of our patients were asymptomatic. Both foreign bodies were successfully retrieved via bronchoscopy using different tools and methods. The initial presentation was a sudden onset of right hypochondriac pain, nausea, and vomiting. CT scan revealed large heterogenous exophytic enhancing mass with mixed solid and fat density within, arising from the right kidney likely represent a giant right renal angiomyolipoma. There is associated right perinephric hematoma and active bleeding within the mass. No features suggestive of tuberous sclerosis. This catheter is used to administer drugs and fluid, especially in critically ill patients, and monitor central venous pressure. We present a case of a retained guidewire for 17 months which was found incidentally from the abdominal and chest radiograph. We retrieved the retained J-tip guidewire using four loops retrieval snare. The retrieval process was complicated with the rupture of the guidewire, causing the unwinding of its outer sphere, leaving the inner monofilament dangling in the heart chamber. The use of lipiodol in lymphangiography in later date has gain popularity as this agent tend to retain in the lymphatic system as opposed to other iodinated hydrosoluble contrast media that diffuse out of lymphatic system rapidly. Over the course of several days and weeks, the iodine within the lipiodol is released by enzymatic cleavage and the fat molecules are degraded. In our case, there is retention of lipiodol in the left axilla which showed as group of round calcifications in Mammogram. This has raised the suspicion of talcum powder usage for the reporting radiologist without revisiting the past surgical and medical procedure. Even though lipiodol washout is a time-dependant process, it can retain in our body or site of injection as long as few years as evidenced in our case. Nasaruddin","doi":"10.32896/tij.v2n1.16-22","DOIUrl":"https://doi.org/10.32896/tij.v2n1.16-22","url":null,"abstract":"Background: Bronchial carcinoid tumours are rare, indolent, malignant neuroendocrine tumours derived from Kulchitsky cells and are not related to smoking. As these tumours can be asymptomatic or present with nonspecific symptoms, a high index of suspicion is essential to make an early diagnosis which determines the prognosis. Surgery is curative and remains the mainstay of treatment. \u0000Case presentation: A 41-year-old female with no background medical illness first presented with a spontaneous left sided pneumothorax requiring a chest tube insertion. High-resolution CT (HRCT) thorax detected an incidental solitary pulmonary nodule. Bronchoscopy revealed a smooth round tumour sitting at the ostium of the basal right lower lobe bronchus. Endobronchial biopsy was complicated with massive bleeding requiring emergency exploration via rigid bronchoscopy. Multiple attempts to secure haemostasis using Watanabe spigot and argon plasma coagulation failed. She was intubated with a double lumen tube to isolate the healthy left lung. An urgent CT pulmonary angiogram (CTA) was performed to look for collaterals and feasibility of embolization, but no collaterals were seen. She was then referred to the cardiothoracic surgeon for an emergency right lobectomy. Histopathological examination revealed typical carcinoid tumour. She was discharged from the hospital in a stable condition. \u0000Discussion: Bronchial carcinoids embryologically originate from the foregut and patients rarely present with features suggestive of carcinoid syndrome and crisis. Mostly are asymptomatic resulting in late presentation and diagnosis. Majority of the typical carcinoids are centrally located and may present with obstructive symptoms and recurrent pneumonia. Bronchoscopists may face massive bleeding following endobronchial biopsy in bronchial carcinoids. \u0000Conclusion: Massive bleeding after endobronchial biopsy can occur and therefore the bronchoscopist should have anaesthesia, interventional radiology, and cardiothoracic support to handle this complication. Unplanned immediate catheter ejection with manual external compression is hazardous due to potential torrential blood leakage or formation of a pseudoaneurysm. Endovascular removal with a vascular closure device deployment is preferred in this situation.
