Weight loss is one of the most significant aetiology of this condition. In young patients presenting with post-prandial abdominal pain, abdominal distension, nausea, anorexia and voluminous vomiting, the diagnosis of superior mesenteric artery syndrome should be considered. These symptoms are due to compression of the third part of the duodenum against the aorta by the overlying superior mesenteric artery. Computed tomography (CT) angiography of the abdomen is currently favoured in the literature to establish the diagnosis. We report a case of SMA syndrome in a 20-year-old Malay gentleman with a history of significant weight loss and kratom abuse for a year. A CT scan of the abdomen was performed and established the diagnosis. We report our 3-year outcomes of mechanical thrombectomy for ischemic stroke. \u0000Method and subjects: A descriptive study was conducted cross-sectionally. Each patient with a National Institute of Health Stroke Scale (NIHSS) score of 10 or higher who presented to our hospital within eight hours of the onset of the first symptom would be considered. The mechanical thrombectomy procedure is performed with aspiration techniques or in combination with stent retrievers, with or without rescue angioplasty and stenting. \u0000Results: From January 1st, 2018, to December 31st, 2020 (36 months), a total of 53 patients were included. Forty-four (44) patients had thrombectomy only, and another 9 patients had thrombectomy with stenting. The total technical success rates were 47/53 (88.6%). The death rate was 15/53 (28.3%). Mohd Ghani","doi":"10.32896/tij.v3n1.1-9","DOIUrl":"https://doi.org/10.32896/tij.v3n1.1-9","url":null,"abstract":"Background: Benign Prostatic Hyperplasia (BPH) is common in aging men with worldwide prevalence at 20-62%, while Malaysian prevalence was 39.3% (2001) and increased at 8% per decade. In surgical treatment of BPH, Trans-Urethral Resection of Prostate (TURP) remains the gold standard. Other surgical options would mostly also require general anesthesia (GA). Therefore, more Local-Anaesthesia (LA) based options should be made available for patients who are not fit or unwilling to be under GA. Those currently available LA-based procedure has shown promising results including prostatic stents and trans-urethral lifts, but have drawbacks due to being expensive, not widely available, less suitable in median lobe enlargement or may cause complications including migration, overgrowth of prostatic tissue or foreign-body related complications which may require GA for their treatment. Prostatic Artery Embolization (PAE), initially an LA-based emergency treatment option for persistent life-threatening hematuria from a bleeding BPH, now has been proven to be a safe elective treatment. This retrospective study aimed to evaluate the performance and safety of TBLC performed on diffuse peripheral pulmonary lesions (DPPL) and localized peripheral pulmonary lesions (LPPL).\u0000Methods: 58 patients underwent TBLC. They were divided into two groups (DPPL and LPPL) based on radiological findings. The patients’ demographic data, procedural details, complications, and radiological and pathologic diagnostic results were analyzed.\u0000Results: The overall diagnostic yield for TBLC was 72.4%. In the subgroup analysis, the diagnostic yield was 74.3% for DPPL and 69.6% for LPPL. For LPPL, the overall diagnostic yield was 87.5% and 12.5% for lesions with positive and negative bronchus signs, respectively (P = 0.01). In both groups, the success of TBLC was not influenced by the size, number, or location of the biopsy. Thirty-two (82%) patients had mild bleeding, while six (15.4%) had moderate bleeding. Pneumothorax occurred in four (6.9%) patients, with three patients requiring chest tube drainage. The mean duration of hospital stay was longer for patients who either bled or developed pneumothorax compared to those who did not (5.64 days vs. 3.47 days; P = 0.014) and (14.25 days vs. 4.24 days; P = 0.035) respectively.\u0000Conclusion: TBLC provided an acceptable diagnostic yield for DPPL and LPPL under fluoroscopy guidance without the use of ancillary devices such as radial endobronchial ultrasound or guide sheath. We present a 22-year-old lady with a known history of childhood neurofibroma type 1. Incidental findings of a huge pelvic mass during laparoscopic surgery for initially thought of a gynae pathology. Further investigation after that with Computed Tomography (CT) scandisplayed a bladder mass occupying the abdomen cavity with regards to neurofibroma of the bladder. A core Biopsy of the mass was done and confirmed the pathology. Clinically patient was asymptomatic with the mass. The patient refused surgical intervention. Follow-up for 2 years with serial imaging showed no significant progression of the disease.
