The pharmacy installation is the last unit that determines the quality of hospital services, so the quality of pharmacy installation services will affect other related units. This study aims to identify value and waste using Lean Thinking approach in pharmaceutical services, especially during waiting times, the time lag that patients receive when submitting a prescription to receiving the drug that has been prescribed. It has four indicators: waiting time for finished drug service ≤ 30 minutes and concoction drugs ≤ 60 minutes, no drug administration errors 100%, customer satisfaction ≥ 80%, and prescription writing according to the formulary 100%. This research used a qualitative approach using the observational action process research method using the lean thinking method to photograph the process flow of outpatient pharmacy services conducted from mid-November to December 2022 at RSUD Sawerigading City of Palopo. Fifteen people became samples to receive the drug and joined structured in-depth interviews. Data were analyzed using Excel with lean thinking. The study results were categorized as lean and proposed improvements to the management support section. Lean thinking could create effective and efficient pharmaceutical governance to improve service quality.","PeriodicalId":31298,"journal":{"name":"JMMR Jurnal Medicoeticolegal dan Manajemen Rumah Sakit","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-04-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46777007","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The Gambia has a MMR of 400/100,000 live births. In the Gambia, 24% of pregnant women receive antenatal treatment four times. Insufficient knowledge of the value of prenatal care and lack of subsidized ANC services are the main causes of the growing maternal and newborn mortality rate in Gambia. The study's major goal was to investigate factors influencing pregnant women in rural Gambia who attend early prenatal care. A cross-sectional community-based survey using structured questionnaires was used for the investigation. Cluster random sampling was utilized in the study to choose samples from 5 distinct districts. The study found that socioeconomic factors, economic factors, and accessibility to antenatal care service center influenced early ANC Attendance. After multivariate analysis, marital status, number of children, mother's occupation, cultural acceptance, and education level significantly correlate with early ANC attendance. The Foni district's early ANC attendance rate (ANC) was comparatively low compared to WHO criteria and fell short of the national requirement. This finding may explain the high mother and child mortalities observed in the Foni districts. The study found significant knowledge of ANC services, but this did not help increase early ANC attendance rate.","PeriodicalId":31298,"journal":{"name":"JMMR Jurnal Medicoeticolegal dan Manajemen Rumah Sakit","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-04-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46952109","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, barriers to incident reporting caused by work environment factors make the patient safety incident system not run well in various countries. This study analyzes nurses’ dimensions of a healthy work environment toward reporting patient safety incidents. This study used an observational analytic design with a cross-sectional approach. This study was conducted in a regional hospital in West Nusa Tenggara province in October - November 2022 with a total sample of 151 nurses working in the inpatient room. Sample determination used a quota sampling technique. Measurement of a healthy work environment was adopted from the HWE Tools and Measurement of incident reporting. The questionnaire was adopted from the Patient Safety Incident Reporting and Learning Systems. Data collection techniques implemented primary data questionnaires and multiple linear regression tests. The results showed a relationship between communication, collaboration, decision-making, staff suitability, recognition, and leadership to patient safety incident reporting with a p-value of 0.001 each (p <0.05). The most dominant relationship is Recognition (B=0.39, p=0.001). Recognition means stimulating a person’s psychological growth and a positive mindset to complete tasks, solve problems, overcome obstacles, and bounce back from difficult situations. Therefore, individual attitudes alone are not enough to make individuals behave to report or not report incidents but must be supported by the wishes of those who have great influence in the environment and want individuals to report if an incident occurs.","PeriodicalId":31298,"journal":{"name":"JMMR Jurnal Medicoeticolegal dan Manajemen Rumah Sakit","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-04-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"41370881","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In April 2022, it was 59,80%, May at 50,55%, and June at 48,84%. It may be due to the governor’s and BPJS regulations, the development of psychiatric in other health services, the limited facilities and infrastructure for non-psychiatric patients, and the lack of information about non-psychiatric services. This study analyzed the low percentage of BOR (Bed Occupancy Rate). The sample was four respondents: the head of the inpatient, the head of the outpatient, the head of the emergency room, and the public relation. The main priority is to solve this research problem using the 5 WHYs and find an alternative solution by discussion method with all respondents. The result showed the main factor causing the low percentage of BOR in RSJRW was the lack of facilities and infrastructure. It was suggested that RSJRW should be aware of cleaning up and provide fewer facilities and infrastructure.
