It has become a highly prevalent condition and a major health problem in the community and working population. Loaders are a vulnerable group of people engage in an occupation that has a higher risk of developing musculoskeletal pain. Psychological distress is also associated with CWP. The main aim of this study was to identify the prevalence and associated factors of CWP among loaders in Pettah, Sri Lanka. Further, the association of psychological distress and the health-seeking behaviors of loaders with CWP was assessed in this study.Methods: A cross sectional study was carried out in a convenience sample of 174 loaders in the main bazaar, Pettah, Sri Lanka. A criterion given in the “Manchester Definition” for CWP was used for the identification of CWP. Data were collected using a self-administered structured questionnaire and Perceived Stress Scale was used to identify the psychological distress. Descriptive and inferential statistics were used to analyze the data with SPSS version 21.Results: The mean age (SD) of the study participants was 42.65 (±12.21)ranged between 19 and 66 years. The level of education of the majority (61.5%) were between grade 6-11. The mean working years as a loader was 20.84 years and the mean weight that the participants carried per turn was 62kg. The prevalence of CWP was 18.4% (n=32) and the lower back pain was the most prevalent (64%) individual problem. High level of stress was perceived by 51.7% of the study participants. Age and the perceived stress were significantly associated with the CWP (p<0.05). Most of the participants (88%) used substances and among them 29.6 % used substances to relieve pain. It has emerged as a distinct clinical entity following the discovery of pathogenic antibody named AQP4-IgG against aquaporin-4 water channels in astrocytes. The clinical spectrum includes optic neuritis, acute myelitis, acute brain stem syndrome, area postrema syndrome, acute diencephalic syndrome and symptomatic cerebral syndrome.This case describes a young male patient who presented with subacute onset painful visual impairment of left eye along with bilateral lower limb weakness and numbness in the background of recent vaccination for covid-19 infection. Examination revealed bilateral papillitis and bilateral flaccid paraparesis with a sensory level at T4 level. Investigations revealed a longitudinally extensive myelitis spanning the entire length of the spinal cord, bilateral optic neuritis, positive serum AQP4-IgG antibodies with negative infective screening and autoimmune profile. Clinical presentation may vary from non specific symptoms such as fever, malaise, weight loss to manifestations of organ hypo perfusion. Cardiac involvement may take forms of valvular insufficiency, angina or heart failure. In this case, we report a 39-year-old woman with hypertension presenting with angina, who was found to have multiple arterial bruits, with blood pressure and pulse volume discrepancies, aortic regurgitation and renovascular hypertension. Thus, early detection of complications is essential. Primary care institutions treat and follow up patients with diabetes in their territories. The aim of the study was to assess awareness, practices, and service-related factors associated with screening for diabetes complications among patients attending the medical clinics of primary care level hospitals in the Galle district.Methods: A descriptive cross-sectional study was conducted at three Divisional Hospitals in Galle District, among patients attending medical clinics. Data was collected through an interviewer-administered questionnaire designed to assess awareness and screening practices for diabetes complications, screening done for diabetes complications, and associated factors were analyzed.Results: Four hundred patients with diabetes participated in the study. The majority (more than 95%) were aware of common organs affected by DM and were aware of investigations and their availability at primary care institutions (more than 85.0%). Adherence to recommended screening tests and frequency was very low (ranging from 3% to 16%) and nearly half of the patients never had any form of screening. Older age was associated with better retinopathy screening {OR = 0.61 95% CI (0.41-0.91)} while having co-morbidities was associated with better screening for retinopathy {OR = 1.67 95% CI (1.02-2.71)} and ischemic heart disease.{OR = 2.03 95% CI (1.22-3.40)}. A better awareness was associated with better screening for DM complications.Conclusions: In conclusion, awareness about screening for diabetes complications was satisfactory among the study sample. However, the screening practices were sub-optimal as the recommended screening protocols have not been followed. However, the direct invasion or extramedullary compression of the spinal cord is rare unlike in the case of chronic lymphocytic leukaemia. We report the case of a young man presenting with transverse myelitis who was found to have leukaemic infiltrates on neuroimaging. The uniqueness of this being the first presentation of the patient with AML in the form of epidural infiltrates in the absence of peripheral blood count abnormalities. ESS is a tool used to evaluate patients’ habitual sleepiness during the day and is used worldwide. This work examined the psychometric properties of the Sinhalese version of eight items ESS among heavy vehicle drivers in Sri Lanka.Methods: A sample of 403 participants was randomly split into two to evaluate dimensionality by exploratory factor analyses (EFA) and confirmatory factor analyses(CFA).Results: Study findings suggest that the single version of ESS is a reliable and valid instrument for sleep evaluation. The analysis identified two-factor structures for the Sinhalese version of ESS. Factor one included sitting and talking, watching TV, car in traffic, sitting in public and sitting and reading, summarized as sitting and doing something. Factor two included lying down, being a car passenger, and sitting quietly, indicating sitting without engaging in any particular activity.Conclusions: Sinhalese version of the ESS is a bi-dimensional instrument that is reliable and valid for assessing daytime Sleepiness in Sri Lankan heavy vehicle drivers.
