Tigen","doi":"10.5222/J.GOZTEPETRH.2013.014","DOIUrl":"https://doi.org/10.5222/J.GOZTEPETRH.2013.014","url":null,"abstract":"SUMMARY In this prospective study, we aimed to identify the factors associated with the development of ventilator- associated pneumonia (VAP) and examine the etiology, and incidence of VAP. Between November 2007 and June 2008, 148 patients who required mechanical ventilation for longer than 48 hours were evaluated. VAP was observed in 54 patients (36 %). Mechanical ventilator utilizationrate was 0.87, and VAP rate in 1000 ventilator days was 22.88. The most common three microorganisms cultured from tracheal aspirates were Pseudomonas aeruginosa (n=19), Acinetobacter spp.(n=11), and Staphylococcus aureus (n=10). Of the 21 risk factors evaluated, 7 factors identified were independently associated with VAP (p<0.05) such as shock, coma (p<0.0006), antibiotic usage for at least 1 month prior to admission (p<0.04), nasogastric tube insertion (p<0.01), invasive procedures such as bronchoscopy, tracheotomy (p<0.0001), reintubation (p<0.017), intubation lasting for more than 5 days (p<0.0001), and smoking (p<0.014). Intensive Care Unit (ICU) clinicians should be aware of the risk factors for VAP to minimize the risk of VAP, patient care should be individualized, and procedures like bronchoscopy, reintubation must be performed and followed up cautiously. Ugutmen","doi":"10.5222/J.GOZTEPETRH.2011.10","DOIUrl":"https://doi.org/10.5222/J.GOZTEPETRH.2011.10","url":null,"abstract":"SUMMARY With this paper, we presented our study series on modified open carpal tunnel decompression operation with mini incision, not crossing the wrist line, performed in three centers between 2000-2004. Regional wrist anesthesia was used in all patients. The operation was carried out via a 2-2.5 cm incision made on the palm in 62 wrists of 52 patients (46 females and 6 males), with a mean age of 41 years (22-54 years), under regional wrist anesthesia and using pneumatic tourniquet. Under direct sight, distal portion of the ligament was sectioned. Proximal portion of the ligament and the antebrachial fascia were released by a blunt dissection once they were separated from the palmar aponeurosis and the subcutaneous adipose tissue. Following the decompression of the carpal tunnel, the layers were closed and compression bandage was applied. Compression bandage was removed on the same day and patients were allowed for daily activities. During postoperative follow-up visits (Mean: 21 months, range: 16-26 months) none of the patients reported recurrence of complaints and all were satisfied with the technique. The aim of this study is to research the prevalence, imaging features and related conditions of AS. Materials and Methods: This study is based on 190 patients that applied to US department with various clinical symptoms in 18 months period. Patient ages, gender; although AS counts, location, shape, dimension and probable accompanied splenomegaly researched on B-mode US. Results: Majority of AS had a dimension between 10-15 mm. Usually they are round shaped and localized at splenic hilus. They mostly visualized in female with the mean age 32.9 (SD: 19.2). There was no statistically difference between splenomegaly and dimension of AS. Positive significant correlation was found between not only age and dimension (p<0.05, r=0.305), but also gender and splenomegaly (p<0.05), dimension and shape (p<0.05). Conclusion: Sonographic features of AS are investigated.
