Over the past few decades there has been a paradigm shift in how we manage this condition with changes in diagnostic criteria, imaging modalities, staging as well as surgical approaches. Endoscopic and Endoscopic Assisted resection techniques have been increasingly popular over the past two decades and have shown to decrease surgical time, post-operative stay as well as intraoperative blood loss. It gives wide exposure to the nasopharynx, orbital apex, sphenoid sinus pterygopalatine fossa, infra temporal fossa and middle cranial base. This enables en bloc resections of tumour and proper visualization.   Objectives: To study patients who underwent this procedure during the last five years with regard to indication, technique, and complications. To compare our patient outcomes with the published literature. To identify the factors leading to complications with recommendations to reduce them in the future. Methods: Retrospective analytical study was done on patients operated from 2015 to 2020 at the ENT Unit C of the National Hospital of Sri Lanka (NHSL). Results: A total of 14 patients underwent maxillary swing approach during the study period. The most common indication was angiofibroma. About 28% had an unacceptable scar following the surgery, which was the most occurring complication while 2 patients had recurrences during this period. Conclusion: Maxillary swing is a useful surgical approach with no major complications in our series. Commonest complication was an unacceptable scar (28%).  Other complications were palatal fistula and nasal speech. Method    Cross sectional descriptive study at the ENT Clinic of Base Hospital Homagama.  Patients diagnosed as Paranasal Sinusitis amongst the patients attending the ENT clinic, age above 12 years.  Patients diagnosed as Paranasal Sinusitis further evaluated for Vertigo and Nape pain.  Total 425 patients taken for study. Result       Patients diagnosed as Sinusitis, 425, 100% Patients diagnosed as Vertigo, 396 – 93.176% Patients diagnosed as Nape Pain, 322 – 75.76% There were Clinical improvement of Sinusitis – 423, 99.523%, among the research group. Patients with Vertigo – 396, all the symptomatic patients improved. Superior view, better access to difficult corners of the middle ear, and better ergonomics have been cited as the reasons for its success. It's a very well-established approach for managing ear cases. We are presenting our experience herewith. Objectives: To assess efficacy of endoscopic ear surgery in local setting Methods Otologic surgical patients attending Nawaloka ENT unit were selected for endoscopic approach with the sole exclusion criteria of extensive cholesteatoma. Data collected using a digital portfolio system. Patients were followed up at 1 week, 2 weeks 1 month and 2 months. Period of study was from March 2017 to December 2019. Results Total number of cases 92 (Myringoplasty 59, Mastoid exploration 17, Excision of Glomus Tympanicum 2, Stapedectomy 2, Excision of osteoma 2, Myringotomy and grommet insertion 10) In suppurative ear cases a success rate (as defined by dry ear without residual perforation) of 72/76 (94.7%) was achieved. Grommet insertions were done without difficulty and all cases had in situ grommets at follow up. A Stapedectomy patient had a closed air bone gap. One glomus tumour excision had a residual perforation, the other no complications. Conclusions Endoscopic ear surgery is an excellent method of attending to ear pathologies. Methods This is a prospective study of transsphenoidal pituitary surgery at National Hospital Sri Lanka from May 2013 to August 2019. 108 patients who underwent transsphenoidal hypophysectomy for pituitary tumours were included. All the patients who underwent the procedure for the first time and for revision surgery were included in the study. Results Out of 108 patients, there were 87 primary cases and 21 revision cases. The average age at presentation was 45.39 years. The youngest patient was 2 years and 6 months old while the oldest was 76 years. 55 patients were male and 53 patients were female. The most common complaint at presentation was headache which affected 36 patients (33.3%). The visual disturbance was the second most common presentation, occurring in 32 individuals (29.6%). 103 patients out of 108 have been operated using trans-nasal transseptal approach and rest (5) by trans labial transseptal method.  The mean postoperative hospital stay was 9.8 days (range 3-17 days). The mean follow-up period was 35.7 months (range 3-75). 6 patients had died during the follow up period and 23 patients had defaulted follow up. Postoperative CSF leak occurred in 8 patients (7.4%) and anosmia was noted in 6 patients (5.5%). Conclusion Microscopic transsphenoidal pituitary is a safe and effective treatment for pituitary tumours, and the commonest complication is CSF leak. Materials & Methods A retrospective study was carried out on 63 patients who were euthyroid and had a clinically palpable solitary thyroid nodule, who underwent either total or hemi-thyroidectomy, during a period of two years (January 2018 to December 2019) in the ENT unit of a tertiary care hospital in Sri Lanka. All patients with FNAC of Bethesda ≥5 underwent total thyroidectomy. Diagnostic hemi-thyroidectomy was performed only in patients with a FNAC of Bethesda ≤4, irrespective of US suspicious features. Results Mean age was 46.9 years (±13.2). 53(84.1%) patients were females and 10 (15.9%) of them were male. 28(44.4%) patients had histologically proven malignancy while the other 35(55.6%) had benign histology. Sensitivity, specificity, positive predictive values and negative predictive values for FNAC and ultrasound were calculated. Out of 34 patients who underwent diagnostic hemi-thyroidectomy, 12(35%) had to undergo completion surgery due to malignant histology. 58% who underwent completion surgery had suspicious features of malignancy in their pre-op US. Conclusion Both ultrasonography and FNAC are cost-effective, minimally invasive investigations in the diagnosis of malignancy in patients with solitary thyroid nodules. Voice therapy is one modality of management of Puberphonia. Aim The Objective of our study was to evaluate the efficacy of voice therapy in patients as the sole treatment to improve the pitch and quality of voice, the psychosocial impact and patient's satisfaction. Methods A retrospective study was carried out on 71 patients who were presented to ENT unit at the National hospital of Sri Lanka for fifteen years from 2002. Pre and post voice therapy subjective assessment was done by the speech therapist for pitch and quality and patient’s perceptual assessment using an analog scale (0-extremely poor to 10-normal voice) and a psychosocial impact assessment was also done. Results The time duration of voice therapy ranged from 1 to 10 months (average-3.6 months). Pitch and quality of the voice became normal in 78.9% and 35.2% of patients respectively. Pre-therapy perceptual score range(PSR) was 0 to 6 (mean-3.0). Post-therapy PSR was 5 to 10 (mean-7.7). 95.8% patients had improved psychosocial impact and patient satisfaction after voice therapy. Remaining opted for surgical management. The incidence of cancers in these clinically benign cysts was nearly 14%.
