{"title":"Audit on the safety and effectiveness of the use of KTP laser in stapedotomy: One year experience in an Otorhinolaryngology Center at a Tertiary Care Hospital in Sri Lanka","authors":"S. M. Daminda","doi":"10.4038/cjo.v13i1.5379","DOIUrl":"https://doi.org/10.4038/cjo.v13i1.5379","url":null,"abstract":"Introduction: This retrospective descriptive study was conducted to assess the effectiveness and safety of using KTP laser in surgically diagnosed patients with otosclerosis who underwent Endoscopic Laser Stapedotomy at the Teaching Hospital Anuradhapura from October 2020 to September 2021. Endoscopic Laser Stapedotomy is used as a minimally invasive technique and it is considered superior to conventional stapedectomy.Methods: Nine patients who underwent laser stapedotomy were incorporated in the study. Data was collected via a pre-formed paper-based operator filled questionnaire. The results of pre-operative pure tone audiogram (PTA) were compared with a three-month post-operative audiogram in addition to demographic and clinical variables and post-operative complications.Results: Mean post-operative air bone gap closure within 10dB was achieved in 55.5% patients. Mean four frequency air bone gap improvement was 25.3dB. Jayasooriya","doi":"10.4038/cjo.v13i1.5365","DOIUrl":"https://doi.org/10.4038/cjo.v13i1.5365","url":null,"abstract":"A 42-year-old female patient who underwent left hemithyroidectomy for a benign thyroid nodule, was referred for further management, as histology revealed a minimally invasive follicular carcinoma requiring completion thyroidectomy as recommended by Oncologists. She had developed unilateral vocal card palsy following the initial hemithyroidectomy. Pre operatively vocal cord function was evaluated by fibre optic laryngoscopy, which confirmed a left vocal cord palsy. During the completion thyroidectomy, continuous intraoperative nerve monitoring was done using an electrode attached to endotracheal tube. A delta electrode was attached to the Vagus nerve and continuous monitoring of the Vagus nerve was done while performing intermittent monitoring of the Recurrent Laryngeal nerve. Stimulation was performed using a current of 1–2 mA. According to the International Nerve monitoring Study Group (INMSG) guidelines initial amplitude value of at least 500 μV was considered as the normal value.According to INMSG guide lines, change of EMG signal curve amplitude below 100 μV was considered as loss of signal during the operation. Post-operative period of the patient was uneventful and the right vocal cord function was confirmed as normal by fibre optic endoscopic examination. Kirihene","doi":"10.4038/cjo.v13i1.5358","DOIUrl":"https://doi.org/10.4038/cjo.v13i1.5358","url":null,"abstract":"Introduction: Extramedullary plasmacytoma (EMP) is a disorder involving plasma cells, leading to the development of neoplastic masses outside the bone marrow. While EMP can occur in different body regions, its predominant manifestation is observed in the head and neck area. A 44-year-old female presented with nasal obstruction, left-sided proptosis, and infraorbital swelling. Rigid nasal endoscopy revealed a polypoidal mass filling both nasal cavities, and imaging showed a uniform density mass lesion involving bilateral sinuses extending into the left orbit. The lesion was resected endoscopically, with subsequent histopathological examination confirming the diagnosis of EMP. Ancillary studies ruled out multiple myeloma (MM). The intraorbital component of the tumour was successfully treated using primary radiotherapy, leveraging the tumours’ inherent radiosensitivity to minimize surgical morbidity.Conclusions: This case underscores the importance of including EMP in the differential diagnosis of sinonasal and other head and neck tumours. Sulcus vocalis is a condition where there is a groove running parallel to the edge of the vocal cord extending from the anterior commissure to the anterior edge of the vocal process of the arytenoid cartilage. Injection laryngoplasty (IL) is a minimally invasive technique which can successfully deal with both of the above-mentioned conditions directly as well as indirectly with only a slight modification in technique using autologous ‘insulinated fat’. Additionally, when treating sulcus vocalis, cold steel dissection of the sulcus through a small parallel incision was preferred without subsequent mucosal suturing. Using 2 surgeons (4 hand technique) improves precision of instrumentation when using a high-definition rigid bronchoscope through a suspension laryngoscopy setup. Following the incident, he refused to feed, displaying irritability. The detection of neutrophil leucocytosis and dehydration prompted IV antibiotics and fluid. Persistent symptoms led to a lateral neck x-ray, revealing a radiopaque material near the epiglottis. Fibre optic laryngoscopy identified a metallic wire lodged in the right arytenoid cartilage and posterior pharyngeal wall, possibly a fragment from a rice polishing machine. Extraction under general anaesthesia resulted in minimal mucosal injury, and the child quickly recovered postoperatively.Conclusion: This report serves as a reminder to consider foreign body ingestion in the differential diagnosis, particularly in cases with subtle or unrelated symptoms in the paediatric age group. The multidisciplinary approach emphasises prompt intervention to prevent life-threatening complications in paediatric patients.
