In most cases, rectal prolapse occurs only during bowel movement and the prolapsed rectum returns spontaneously afterwards. In rare cases, however, increased oedema makes it impossible for the rectum to return and may cause lower intestinal obstruction. We describe a case of a 50-year-old male with intestinal obstruction due to incarcerated rectal prolapse. The patient was qualified for an urgent surgery using the Altemeier’s technique. Perineal rectal resection is a method that has been used for many years. It is dedicated mostly to patients at an advanced age who additionally require levator repair due to pelvic floor muscle failure, which is relatively easy to perform using this access. In the presented case, however, perineal access was the only one possible regardless of the pelvic floor function. The surgery itself and the postoperative period were uneventful, and the functional treatment outcome was excellent. This is due to the increased severity of constipation in pregnant women, which is associated with hormonal factors, higher iron intake, limited physical exercise and weight gain during pregnancy. These factors promote haemorrhoidal disease, perianal venous thrombosis and hypertrophy of marginal anodermal folds. A pregnant or a postpartum patient reporting rectal symptoms becomes a multidisciplinary patient, with the overlap of two specialities, i.e. gynaecology and coloproctology. Most rectal conditions that occur in this special period of life may be managed conservatively and only some of cases require surgical approach. Therefore, obstetrician-gynaecologists with sufficient knowledge and experience in the diagnosis and treatment of the most common rectal conditions may implement treatment on their own instead of referring the patient to a proctologist. However, there are clinical situations when proctological consultation is necessary for medical and legal reasons. Ciesielski","doi":"10.25121/nm.2019.26.1.20","DOIUrl":"https://doi.org/10.25121/nm.2019.26.1.20","url":null,"abstract":"A patient who reports to the hospital due to anal abscess is often treated as not seriously ill, although in about 40% of cases, abscess may initiate anal fistula development, and the one that is not or insufficiently emptied, may also cause septic complications that directly threaten patient’s life. The majority of inflammatory diseases in the rectum, including abscesses and anal fistulae, are a growing infection in the anal crypts. In recent years, a large group of patients with rectal abscesses have been patients with inflammatory bowel diseases, as well as immunocompromised patients, e.g. in the course of HIV infection. The anal abscess in any case should be urgently treated by its opening and drainage. The majority of patients with rectal abscesses should be admitted to the hospital and operated in the operating block, in adequate anesthesia (regional or general). The article discusses recommendations for the opening of low and high abscesses, postoperative management and possible complications associated with this disease and its inappropriate treatment. In most cases, anal fistula is a consequence of abscess developed due to infection of anal the glands. The most common symptoms of anal fistula are recurrent inflammation around the anus, pruritus and purulent leakage. In the treatment of fistulas mainly surgical methods are used, however, attempts of conservative treatment (tissue adhesives glues or plugs) are also undertaken. The goal of treatment is to choose a method that will be characterized by high radicality with the lowest possible risk of complications. Literature describes only few cases of perianal fistula caused by a foreign body. The paper presents a case of a 57-year-old man who was diagnosed with fistula-in-ano caused by a foreign body of unknown origin. The patient reported long-term purulent discharge only. Surgical treatment involved resection of the foreign body and fistula with subsequent uneventful recovery during outpatient follow-up. Anal fistula was diagnosed by anamnesis and physical examination, but its cause was detected post-operatively. The introduction of novel diagnostic and treatment methods has resulted in decreasing mortality rates; however, an increase in morbidity is observed. Severe inflammation plays an important role in many aspects associated with carcinogenesis. Neoplasia and inflammatory response are mutually related. Aim. The aim of the study was to evaluate the association between preoperative serum C-reactive protein levels and total leukocyte count in colorectal cancer patients in comparison with postoperative histopathological findings depending on whether there was neoplastic infiltration of the local adipose tissue or not. Material and methods. The study population included 50 patients with colon cancer. Exclusion criteria were distant metastases, neoadjuvant therapy and a past history of cancer diagnosis. Blood findings were compared with histopathology. Patients were divided into two groups – with and without neoplastic infiltration of fat tissue in histopathology. Results. Serum C-reactive protein levels were evaluated in all patients. The tested indicator was higher in the group with infiltration, but was not statistically significant. Additionally, the WBC count was normal in both groups, but it was lower in the group with infiltration, and the result was statistically significant. Conclusions. C-reactive protein level and total leukocyte count can be a helpful and supportive marker in staging in colorectal cancer patients. Owing to the wide availability and low cost of the above mentioned blood tests, they may be easily performed in daily medical practice. In the guidelines presented in 2018 in „Diseases of the Colon & Rectum”, the American Society of Colon and Rectal Surgeons (ASCRS) hasn’t specified laser techniques as a method of treatment. The first attempts of laser trail in hemorrhoidal treatment were made at the end of the 70s by using laser CO2 instead of scalpel. Nowadays one of the newest devices is Leonardo laser which emits fan-shaped movement of 1470 nm length. It enabled the development of laser hemorrhoidoplasty technique (LHP). The advantage of this procedure is decrease of hemorrhoidal plexus by obliteration of hemorrhoidal vessels leading to their fibrosis. It reclaims almost anatomical architecture of anal canal without a necessity to excise the extended hemorrhoids. Development of the connective tissue fibers guarantees adequate adhesion of mucosa to the underlying tissue which prevents the recurrence of the disease, especially the ones connected with prolapsing of hemorrhoids. It is classified as a non-malignant disease that typically develops within the reproductive system: in the ovaries, fallopian tubes, or uterine ligaments. These changes tend to occur also beyond the reproductive structures, usually within the digestive system, where the most common sites are the peritoneum, postsurgical scars or umbilicus. Moreover, the disease can occur in the intestine, usually in the large bowel, and particularly in the caecum. The literature reports that endometriosis may affect even 10% of women of child-bearing age. The authors present a case of a patient admitted to the surgical ward due to mechanical bowel obstruction. Intraoperatively, an endophytic tumour, completely occluding the ileum, was found. Due to its location, i.e. approximately 15 cm from the Bauhin’s valve, it was resected together with the ascending colon and the right half of the transverse colon. Based on the morphology of the lesion and a positive family history, cancer was thought to be the most probable cause of the patient’s symptoms. The result of the histopathological examination did not confirm this, however. The specimen showed endometriosis. On the day of surgery, the patient was administered antibiotics, total parenteral nutrition and essential fluid supplementation. The wound healed normally, and abdominal pain did not return as diet was being extended. In addition to a visit to a surgeon-proctologist, the diagnosis of impaired defecation often requires urological and gynaecological evaluation. Many pathologies causing evacuatory difficulty, such as full thickness rectal prolapse, reduced sphincter tone and haemorrhoidal disease, can be detected by clinicians already at the stage of physical examination. A large group of pathologies may be detected using standard diagnostic techniques such as colonoscopy and pelvic imaging, e.g. computed tomography, magnetic resonance, and transabdominal/transrectal/transvaginal ultrasonography. However, certain abnormalities are only visible during functional pelvic examination. These include rectocele, sigmoidocele, enterocele, internal intussusception, cystocele, and spastic pelvic floor syndrome. Defecography is guided by either X-ray or MRI. Pelvic function may be also assessed using transperineal ultrasonography. Defecography involves 4 phases: rest, forced contraction, strain, and defecation. The anorectal angle and, in the case of MRI, the PCL line (which is a reference point for most measurements), are determined in order to evaluate the position and mobility of pelvic organs. Pelvic function evaluation helps differentiate patients with evacuatory difficulties requiring surgical intervention from those who need conservative treatment (exercises, electrostimulation). Roman medicine was highly influenced by the Greek medical tradition, relying more on naturalistic observations rather than on spiritual rituals. The writings of Galen survived more than other medical scriptures in antiquity. His theories dominated and influenced Western medical science for more than 1,300 years. This acceptance led to the spread of Greek medical theories throughout the Roman Empire, and thus a large portion of the West. Most of the actual medical terms are of Greek origin, as they were the founders of rational medicine in the golden age of Greek civilization. The Hippocrates were the first to describe diseases based on observation, and the names given by them to many conditions are still used today. Diagnosis of early-stage disease is difficult due to non-specific symptoms. Patients are usually treated on an emergency basis due to gastrointestinal obstruction or bleeding. The paper presents a case report of an 84-year-old female who underwent surgical treatment due to bowel obstruction caused by ileum cancer, and in whom diagnostic imaging did not allow for a correct diagnosis. The surgery was complicated by intestinal fistula. Histopathology confirmed local tumour progression.
