{"title":"Burnout Syndrome amongst Medical Students in Cameroon: A Cross-Sectional Analysis of the Determinants in Preclinical and Clinical Students.","authors":"Tsi Njim,&nbsp;Haman Makebe,&nbsp;Louise Toukam,&nbsp;Belmond Kika,&nbsp;Steve Fonkou,&nbsp;Johnson Fondungallah,&nbsp;Azingala Fondong","doi":"10.1155/2019/4157574","DOIUrl":"https://doi.org/10.1155/2019/4157574","url":null,"abstract":"<p><strong>Background: </strong>Due to a series of recognised psychological stressors in the traditional path of medical studies, medical students are prone to experience burnout syndrome.</p><p><strong>Objective: </strong>This study aimed to determine the predictors of burnout syndrome amongst medical students in Cameroon.</p><p><strong>Methods: </strong>This was a cross-sectional study which recruited 413 medical students consecutively after sampling three of the five medical schools in Cameroon using a random sequence generator. Data were collected via a printed self-administered questionnaire from consenting participants assessing burnout syndrome using the OLdenburg Burnout Inventory (OLBI). Data was analysed using Stata version 12 and p value significance was set at 95%. Multivariable linear regression was used to identify independent determinants of burnout syndrome.</p><p><strong>Results: </strong>An alpha Cronbach coefficient of 0.74 showed that the OLBI assessed the same underlying construct of burnout syndrome in this population. The results of the multiple linear regression showed that five predictors explained 35.0% of the variance (R<sup>2</sup>= 35.0, F(9, 184) = 10.99, p <0.001). It was found that marital status significantly predicted burnout (Beta: 4.82, p value: 0.024), as well as relationship difficulties (Beta: 3.17, p value <0.001), cumulative GPA (Beta: -2.15, p value: 0.006), regretting the choice of medical studies (Beta: 7.85, p value <0.001), and recreational drug use (Beta: 6.99, p value: 0.005).</p><p><strong>Conclusion: </strong>Early detection of burnout in medical students in Cameroon could be done by identifying and addressing the potential determinants. Studies on remission from alcohol use disorders are sparse.</p><p><strong>Objective: </strong>The aim of this study was to analyse the rate of remission from AUD and the possible influence of other mental disorders and sociodemographic factors on the remission in the Lundby Cohort.</p><p><strong>Method: </strong>Remission from AUD was studied for 312 male subjects in the Lundby Cohort, which was followed for 50 years. Cox regression analyses were used to study the possible influence of sociodemographic variables and other mental disorders on AUD remission.</p><p><strong>Results: </strong>In all, 64/312 (21%) subjects achieved remission during the study period. The presence of a severe mental disorder, such as delirium tremens and organic disorders, was related to remission. Blue-collar workers had higher rates of remission than white-collar workers. There was indication that treatment improved the prognosis.</p><p><strong>Conclusions: </strong>The overall remission rate was low, but treatment may improve the prognosis. The final Persian version and its back-translation were submitted for approval to the DSM-5 design workshop in the United States. Mentally ill individuals are not only suffering from the illness but also suffering from the stigmatizing attitude generated by the community. The objectives of this study were to determine attitude of SSs towards mental illness and its associated factors. A cross-sectional study design employing stratified random sampling was applied to select a sample of 402 students. Data was obtained using a self-administered Belief towards Mental Illness (BMI) questionnaire. Independent sample t-tests and one-way ANOVA were used to determine possible differences in scores of attitude. From a total of 21 BMI scale items, positive attitudes were found in eight items and negative attitudes were found in the remaining thirteen. The mean score of the full BMI scale was 2.47 (95% CI: 2.41, 2.54). The mean (95% CI) scores of dangerousness, poor social relations and incurability, and shame subscales were 2.68 (2.60, 2.76), 2.55 (2.48, 2.62), and 1.22 (1.09, 1.34), respectively. A significant negative correlation was found between attitude scores and the average mark of students (r = -0.257, <i>p</i><0.0001). Moreover, significant differences in attitude scores were observed between students with a relative of mental illness and those without such a relative (<i>p</i>=0.004). There was an increasing trend of positive attitudes with increased educational level among 9th, 10th, and 11th graders (<i>p</i>-trend<0.0001) and with an increase in the educational level of the students father (<i>p-</i>trend=0.028). However, no significant difference in attitude score was found across categories of sex, religion, living condition of father, presence of a mentally ill neighbor, educational level of mother, or ethnicity. In conclusion, considerable numbers of SSs have negative attitudes towards mental illness. Only paid workers were selected (n=160). Sociodemographic variables including prevalence of moderate depression were compared between the genders using Chi square test. Significant variables were subject to logistic regression. Validated Nepali version of the Beck Depression Inventory scale (BDI-Ia) was used to determine depressive symptoms with a cutoff score of ≥20 considered as moderate depression.</p><p><strong>Result: </strong>The overall prevalence of moderate depression was 15%, with higher prevalence among working women compared to men [<i>χ</i> <sup>2</sup> (df) = 6.7(1), <i>P</i>=0.01], those practicing religions other than Hinduism [<i>χ</i> <sup>2</sup> (df) = 5.5(1), <i>P</i>=0.01], those educated up to primary school compared to other education criteria [<i>χ</i> <sup>2</sup> (df) = 9.4(4), <i>P</i>=0.03], those having vitamin D deficiency compared to others [<i>χ</i> <sup>2</sup> (df) = 8.5(3), <i>P</i>=0.03], and sedentary lifestyle compared to active lifestyle [<i>χ</i> <sup>2</sup> (df) = 6.7(1), <i>P</i>=0.009]. The OR (95% CI) for moderate depression was significantly higher in women than in men [3.2 (1.1-9.6), <i>P= 0.03</i>] and sedentary lifestyle [2.9(1.1-8.2), <i>P= 0.04</i>] even after adjusting for confounding variables.</p><p><strong>Conclusion: </strong>Working women have increased odds of depression compared to men. A cross-sectional study design was employed to assess 200 randomly selected patients attending the clinics. Questionnaires were administered to obtain sociodemographic and medical history data. The perceived stress scale (PSS) was used to determine the presence of subjective psychological stress and PHQ-9 was used to screen for depression. The prevalence of depressive symptoms was 54.9% with 16.5% categorised as having major depression. After adjusting for confounding variables, age younger than 60 years was associated with less odds of having depressive symptoms (AOR 0.32, 95% CI 0.17, 0.62; p=0.001), while only significant psychological stress increased the odds of having depressive symptoms (AOR 2.78, 95% CI 1.37, 5.64; p=0.005). These children and adolescents living as orphans or in stigmatized environments are vulnerable because of the loss of parent figures. The HIV/AIDS epidemic has contributed to a drastic increase in the number of orphans and vulnerable children and other causes in sub-Saharan Africa. However, little is known about the prevalence of depression and associated factors among orphanage children in areas such as Ethiopia.</p><p><strong>Objective: </strong>The aim of this study was to assess the prevalence of depression and its associated factors among orphans in Ilu Abba Bor Zone orphanages, 2016.</p><p><strong>Methods: </strong>An institutional based cross-sectional study was conducted among orphan children in orphanages at Mettu and Gore. A total of 220 orphans were included from the two orphanages and make the response rate of 98.2%. Pretested semistructured questionnaire was used for interviewing the study participants. The collected data were coded, entered into EPI-INFO 7.0. Software, and exported to SPSS version 20 for statistical analysis. The strength of association between variables was assessed using crude and Adjusted Odds Ratio by running logistic regression and the cut-off point for declaring statistical significance was P- value <0.05 or 95% confidence interval which does not contain the null value.</p><p><strong>Results: </strong>A total of 216 orphan children were interviewed with response rate of 98.2%. The overall prevalence of depression was 24.1%. The mean age of participants was 14.2 years ± 9.90 SDs and range from 11 to 17 years. This study aimed to examine whether individuals with specific mental disorders, including affective psychoses, neurotic disorders, schizophrenia, personality disorders, and adjustment reaction, have higher risks for subsequently developing substance-related disorders compared to those without. <i>Methods.</i> A large-scale study with longitudinal data was conducted using the Taiwan National Health Insurance Research Database (NHIRD) consisting of 2,000,118 patients' medical records from 2000 to 2009. A total of 124,423 people diagnosed with selected mental disorders and the same number of people without the diagnoses of the selected disorders were identified between January 1, 2001, and December 31, 2006, and followed up for the diagnoses of substance-related disorders till the end of 2009. We estimated the risk for subsequently developing substance-related disorders among patients with the selected mental disorders compared to those without by using Cox proportional hazard models. The cumulative incidence of substance-related disorders was calculated using the Kaplan-Meier method. <i>Results.</i> The risk for developing substance-related disorders in patients with selected mental disorders is about 5 times (HR=5.09, 95% CI: 4.74-5.48) higher than those without after adjusting for potential confounding variables. From the multivariate analyses of subsamples stratified by age, sex, and urban and income levels, we found all adjusted hazard ratios were significantly higher than 1.0, ranging from 2.12 (95% CI: 1.72-2.62) to 14.55 (95% CI: 7.89-26.83). For children and adolescents aged 10-19 years, those with specific mental disorders had 14.55-fold higher risk for developing substance-related disorders in later life compared to their counterparts. Furthermore, patients with personality disorders had the highest risk (HR=25.05). <i>Conclusions.</i> The earlier onset of the selected mental disorders is a potential risk for developing substance-related disorders in later life, particularly for personality disorders. Cross-sectional studies in different localities suggested a significant burden of mental distress among caregivers of patients with epilepsy, but we are not aware about the condition in Ethiopia. Therefore, the aim of the current study is to assess the prevalence and associated factors of mental distress among caregivers of patients with epilepsy in Ethiopia.</p><p><strong>Methods: </strong>An institutional based cross-sectional study was conducted in Neuropsychiatric Department of Amanuel Mental Specialized Hospital, Addis Ababa, Ethiopia. Using systematic random sampling technique, 409 caregivers participated in the study. Data was collected by face to face interview using standardized and validated Kessler Psychological Distress Scale (K-10) to assess mental distress. Descriptive, bivariate, and multivariate logistic regression models were used for analysis. Adjusted Odd Ratio (AOR) with 95% Confidence Interval (CI) was used to show the odds, and <i>P</i>-value < 0.05 was considered as statistically significant.</p><p><strong>Results: </strong>The mean age of respondents was 43.3 years with standard deviation of ±11.4 years. Two hundred eighteen (53.3%) of the respondents were male. The prevalence of mental distress was found to be 27.1% with 95% CI [22.6-31.1]. Relationship with patient of being mother [AOR: 5.67, 95% CI: (1.68-13.70)], father [AOR: 4.42, 95% CI: (1.25-12.58)], wife/husband [AOR: 10.59, 95% CI: (2.43-14.19)], and child [AOR: 5.37, 95% CI: (1.27-12.69)]; caring for young person below 20 years of age [AOR: 4.00, 95% CI: (1.43-11.21)]; poor social support [AOR: 7.26, 95% CI: (3.60-14.65)]); and experienced stigma [AOR: 3.03, 95% CI: (1.63-5.66)] were statistically and significantly associated factors of mental distress among caregivers of patients with epilepsy.</p><p><strong>Conclusion and recommendation: </strong>We found a lower prevalence of mental distress among caregivers of patients with epilepsy compared to other low- and middle-income settings. Being caring for young patients, being parents to the patient, poor social support, and stigma were statistically significant associated factors of mental distress among caregivers. Adjustment difficulties are the foremost common issues among freshman students; World Health Organization area unit researched a full-of-life adjustment innovation in universities. Despite this reality, there is a few literature that identifies prevalence and factors related to the adjustment downside among university/college students.</p><p><strong>Methods: </strong>Institutional based quantitative cross-sectional study style was conducted from May to June 2016. A total of 537 haphazardly elite students concerned within the study. The result adjustment downside was explained with Student Adjustment to College Questionnaire (SACQ). Data was described using descriptive analysis; logistic regression was used to assess the relation between adjustment downside and associated factors. An odds quantitative relation 95% confidence interval was used to point out the strength of association. Applied mathematics significance was declared as a p value less than 0.05.</p><p><strong>Result: </strong>A total of 537 respondents participate with a response rate of eighty-five percent. the prevalence of adjustment downside among the respondents was 228 ( 42.5%). Of the respondents, 327 (60.9%) were male and therefore the mean (sd) age was 19 (+- 1) years. Being away from home and homesickness (AOR=4.15,95% CI: 2.46,2,99 and AOR=5. 9,95% CI: 2.35,14.76) and difficulties in socializing or making friends (AOR=3.99,95% CI:2.29,6.98) and difficulties in managing time and study skill (AOR=3.02,95% CI: 1.3,7.02) were abundant associated factors with adjustment downside.</p><p><strong>Conclusion: </strong>The study confirms that freshman students joining university do face problems in adjusting themselves at the university.
