As it happens in early weeks of pregnancy, abnormal vasculogenesis and disordered cell multiplication are possible causes. Hyperhomocysteinemia is associated with both these pathological processes.\u0000Objective: To assess the association between hyperhomocysteinemia and recurrent pregnancy loss.\u0000Materials and Methods: This case-control observational study was conducted in the Department of Gynaecology and Obstetrics of Bangabandhu Sheikh Mujib Medical University, Dhaka during January to December 2015. Sixty patients were divided into two groups: 30 with recurrent pregnancy loss (RPL) as cases and 30 normal healthy mothers as controls.\u0000Results: All the subjects were matched in terms of age (p = 0.504). All were within normal BMI range (19.5 to 25 kg/square meter). Subjects in both groups were comparable in height, weight and BMI. 33.3% women of control group were in the low income stratum (monthly income 20000 taka or less) compared to50% of the RPL group. Control group had an equal mix of educational level from below Secondary School to Post-Graduates whereas there were fewer Post Graduate patients (3.3%) in the RPL group compared to control group (23.3%). The frequency of hyperhomocysteinemia (>15 micromoles/L) was significantly higher in cases compared to controls (46.7 vs.16.7%, p= 0.012). Mean homocysteine level was also significantly higher in cases compared to controls (13.67+/-4.80 vs. 9.87+/-4.84 micromole/L; p= 0.003).\u0000Conclusion: This study shows that blood homocysteine level in recurrent pregnancy loss patients is significantly higher compared to normal fertile mothers. Islam, Nelufa Tahera Rahman","doi":"10.3329/jemc.v11i1.63170","DOIUrl":"https://doi.org/10.3329/jemc.v11i1.63170","url":null,"abstract":"Background: Acute kidney injury (AKI) is a public health issue associated with multiple clinical conditions which may occur due to slight elevation in serum creatinine to anuric renal failure with electrolytes and acid-base imbalance, chronic kidney diseases, end-stage renal diseases, impaired innate immunity associated with higher infection rate, and increased duration of hospital stay with higher cost. Sometimes severe AKI patients may need intensive care support and renal replacement therapy. Severe sepsis is the most common cause of ICU admission.\u0000Materials and Methods: This observational study was conducted during the period of July 2018 to May 2019 in the Department of Anesthesiology and Intensive Care Unit of Enam Medical College & Hospital, Savar Dhaka. A total of 87 AKI patients were selected among which 48 were male and 39 were female. Acute kidney injury was selected with an increase in serum creatinine >0.3 mg /dL within 48 hours or ≥1.5 mg /dL from the base line within prior 7 days or urine volume <0.5mL/kg/hour. The last recorded lowest value or value within 24 hrs before admission was considered as baseline S. creatinine. Staging was done according to the KDIGO classification. Chronic Kidney Diseases (CKD) patients and acute on chronic kidney diseases patients aged <18 years were excluded from our study.\u0000Results: Highest age group was between 41 to 60 years. According to KDIGO definition 27.58% fell in stage-I, 39.8% in Stage-II and 33.33% in Stage-III. Hypertension (34.48%) and diabetes (24.18%) were found common co-morbid conditions. The most common indication for admission in ICU was septicemia (21.83%). Other causes include pneumonia and other respiratory illness (18.39%), gynae and obstetrical cases (16.09%) AGE (acute gastroenteritis) 13.79%, poisoning 9.19%, polytrauma 8.0%, cardiac diseases 6.89%, cerebrovascular diseases 3.44%, malignancy 2.29%. Total 62.06% (54) patients needed ionotropic support, 55.17% (48) needed artificial ventilation. 26.4% patients needed renal replacement therapy (8.04% HD, 12.64% SLED, 5.74% CRRT). Hence, this study was aimed to assess the prevalence and antibiotic resistance pattern of ESBL and AmpC beta-lactamases producing E. coli in Bangladesh Institute of Health Sciences (BIHS) General Hospital.\u0000Objectives: To assess the prevalence of ESBL +ve and AmpC +ve producing E. coli and to see their antibiotic resistance pattern in BIHS General Hospital.\u0000Materials and Methods: A total of 675 samples were subjected to aerobic bacteriologic culture in the department of Microbiology, BIHS General Hospital, Mirpur, Dhaka, during the period of 1st July 2016 to 30th June 2017. Specimens were collected from hospitalized and outdoor patients of different age and sex groups. All the isolates were identified by standard microbiological technique and their antibiotic susceptibility was observed by disk diffusion method. Strict aseptic precautions were taken all through the culture system.\u0000Results: Out of 675 specimens, 150 (22.22%) culture yielded growth of E. coli. Among them 47 (31.3%) were ESBL +ve, 27 (18%) were AmpC +ve and 7 (4.7%) were both AmpC +ve and ESBL +ve E. coli.\u0000Conclusion: Advance antibiotic resistance surveillance is necessary to guide the appropriate and judicious antibiotic use. Diverse interpositional materials have been proposed in this regard with especial advantages and limitation. Interpositional arthroplasty is the best solution. The aim of this review article is to provide a useful clinical update about interpositional arthroplasty in temporomandibular joint ankylosis, its advantages and limitations. For this purpose eighteen articles were selected through searching internet, reviewed and are summarized here. Dental surgeons need proper training for proper diagnosis and adequate treatment. Research in this field will be helpful for both patients and doctors. Maternal employment has also been rising steadily. However, little is known about the relationship between maternal employment and child’s behavioral and cognitive problems.\u0000Objective: To evaluate the child’s behavioral and cognitive problems and their relationship with maternal employment.\u0000Materials and Methods: This cross-sectional analytical study was conducted on 30 children aged 3−8 years, having cognitive and/or behavioral problems who fulfilled the enrollment criteria. Data were analyzed descriptively and Chi-square/Fisher’s exact tests were used wherever appropriate with significance set at p<0.05.\u0000Results: Children’s mean age was 5.5±2.0 years (M:F 1.3:1). Of all children, 73.3% were family’s 1st child, 83.3% belonged to nuclear family. Among parents 43% spent quality time with children. Among mothers 40% were service holders and others were housewives. All working mothers were employed before 3rd year of their children. Among working mothers 84% worked 8 hours per day. Externalizing and internalizing behavior problems, non-satisfactory school performance and non-disciplined activities were found higher among working mothers’ children compared to housewife mothers’ children (50.0% vs 38.9%; 16.7% vs 11.1%; 91.7% vs 88.9%; 50.0% vs 44.4%, respectively). In this study >1 behavior problems and delayed cognition were found higher among housewife mothers’ children (33.3% vs. 38.9%; 41.7% vs. 66.7%). In both groups, 83.3% children were not involved in extracurricular activities. Overall, Risk Ratios did not predict any risk of having behavioral problems with mothers’ employment (all p values >0.05).\u0000Conclusion: This study demonstrated no significant effect of maternal employment on developing child’s behavioral and cognitive problems. We report two young girls of limb girdle muscular dystrophy (LGMD), who presented with gradual onset of weakness in proximal muscle of all four limbs. There was positive family history in one girl. Neurological examination revealed pseudo hypertrophy of both calves, hypotonia in all four limbs, muscle power diminished, more on proximally. All deep tendon reflexes were diminished with planters bilateral flexors. Gower sign was positive and winging of scapula was also present. Electromyography (EMG) showed myopathic pattern.
