However, it cannot be assumed that its underpinning values are lived out in day-to-day practice in a hospice. At St Columba’s Hospice, Edinburgh, the five-year strategy demonstrated commitment to person-centredness and this prompted an 18-month project focusing on the evaluation and development of a person-centred culture, taking a practice development approach. Aim: To implement a person-centred practice development research project to assess, evaluate and enhance person-centred culture within St Columba’s Hospice. Methods: The theoretical underpinnings of the programme were based on the Person-centred Practice Framework. A transformative practice development approach was employed to bring about change in individuals and teams. Twelve multidisciplinary team members from different departments across the hospice formed a core research project group, which was allocated 10 protected learning and development days over the 18-month period. To assess the existing culture, data were collected at the beginning of the practice development programme, including observation in practice and real-time interviews with patients and staff. The data were analysed using a participatory approach, with group members mapping the data collected against the Person-centred Practice Framework and undertaking creative hermeneutic analysis. Discussion: The project created and sustained a space to explore and expose person-centredness within the hospice and raised awareness of what person-centred culture means in day-to-day practice. Fourth-generation evaluation highlighted further areas for action, with teams developing their own action plans aimed at enhancing person-centred culture. Conclusion and implications for practice: The context of day-to-day practice in a hospice setting is complex, and developing person-centred culture is an ongoing process. Hospices can help their staff to flourish by providing the necessary space to reflect and for critical awareness of own practices to be heightened. Yet, it is still not common practice. Aims and objectives: This discussion article aims to explore some of the historical and current perspectives on the interrelated concepts of personhood and personcentred care, and to explain how the persistence of differing perspectives affects the way person-centred care is understood, implemented and evaluated. The article then aims to explain the need for personcentred cultures and practices, and to find a way to progress towards a person-centred agenda. Methods: To develop an understanding of the evolution of and current approach to these concepts, a literature search was undertaken. This included a broad search of the grey literature and the Medline and CINAHL databases, as well as review of articles published in the International Practice Development Journal, and a number of books and literature recommendations. Discussion: Multiple perspectives were found in relation to personhood and person-centred care. How personhood is viewed by healthcare staff and organisations has a direct impact on how person-centred care is delivered. Person-centred practice is a more inclusive concept as it advocates that staff should also experience person-centredness. However, to achieve sustainable person-centred practice, efforts may need to focus on investment in developing person-centred cultures at system and team levels. A person-centred framework can guide this change. Conclusion: Person-centred care is espoused within health policies, visions and mission statements. However, the focus should be on person-centred cultures and on how these can be developed and embedded. The growing and complex healthcare needs of an ageing population mean moments of vulnerability and unhurried care provision are now often looked upon with disapproval and discomfort, when such moments should be celebrated for their altruism and humanness. Aim and objective: The aim of this article is to share a critical reflection on compassion and vulnerability in nursing. An exploration of the influence and significance of vulnerability in the nursing profession will shed light on the experiences of a student nurse during clinical placements. Conclusion: By allowing themselves to become vulnerable, contemporary nurses will be better able to establish a work ethic focused on delivering person-centred compassionate care and a high level of patient safety. For a range of reasons, however, they are marginally staffed and face recruiting challenges. These issues threaten the educational quality of the nursing students’ clinical placement, and these settings may thus be characterised as a marginal nursing home learning environment. In clinical placements, observing nursing care quality might be critical for nursing students’ learning opportunities and for their motivation to pursue a nursing career. Thus, addressing clinical placement challenges in a marginal nursing home learning environment is of crucial importance to secure educational quality, future recruitment and quality in nursing care. Aim: To explore and describe final-year nursing students’ situated learning experiences in a marginal nursing home learning environment. Methods: An exploratory-descriptive qualitative design (EDQ) using individual interviews for data collection. Eight third-year nursing students were individually interviewed by two researchers following their clinical placement in a marginal nursing home. Data were analysed using a thematic data analysis approach. Results: Four main themes emerged from the data analysis: the importance of being invited into the working community; the importance of being offered predictability in situated learning; the importance of being involved in professional dialogues; and the importance of being assigned co-responsibility combined with supportive mentoring. Conclusion: Findings reveal that having their clinical placement in a marginal nursing home learning environment offered students a range of learning and mentoring opportunities as well as mixed experiences and some challenges. Implications for practice and education: The marginal nursing home context as a learning environment challenges both the clinical and academic institutions to improve their professional collaboration.
