Dubenko","doi":"10.30978/unz2018211","DOIUrl":"https://doi.org/10.30978/unz2018211","url":null,"abstract":"Trigeminal autonomic cephalalgias is the group of primary headaches that include cluster headache, paroxysmal hemicranias, hemicranias continua and short-lasting unilateral neuralgic-like headache attacks. The common manifestations of these headache types are spontaneous attack of unilateral severe pain of orbital, supraorbital and/or temporal localization which are accompanied with ipsilateral cranial autonomic symptoms including conjunctival injection, lacrimation, nasal blockage, rhinorrhea, eyelid edema, facial sweating, feeling of ear fullness, miosis or ptosis. Agitation and restlessness during attacks are typical for all trigeminal autonomic cephalalgias. Pathophysiology has centered on the activation of the posterior hypothalamus and trigeminal autonomic reflex. Gorbenko","doi":"10.30978/unz2018274","DOIUrl":"https://doi.org/10.30978/unz2018274","url":null,"abstract":"In this article deals with a typical clinical example of a severe form of acute neuroinfectious lesion of the spinal cord HSV2‑etiology in a patient with minor cellular immunodeficiency, who suffered from often recurrent genital herpes without adequate prophylactic antiviral treatment for the prevention of CNS damage. The patient, M., 54 years old, had acute lumbosacral myelitis, caused by herpes virus type 2, with a picture of the lower deep paraparesis, paragypesthesia, impaired pelvic organs functions. The diagnosis is confirmed on the basis of a PCR of the cerebrospinal fluid. Immunological examination revealed a selective deficiency of natural killers (CD3 – CD1 + CD56 + lymphocytes) — cells that are important in controlling the latent virus in the sacral sensory ganglia. A genetic survey identified a deficiency of the folate cycle that caused the minor cellular immune deficiency (MTHFR 677 С > Т homo, MTHFR 1298 А > С hetero, MTRR 66 A > G hetero). Therefore, the patient received dual treatment — acyclic analogues of guanosine to suppress viral reproduction and interferone therapy to compensate cellular immunodeficiency. Methods and subjects. The experiments were carried out with hippocampal cell cultures. A special MP device for cellular cultures has been developed. Lipopolysaccharide (LPS) was used for modeling of the inflammatory process. The viability and mitochondrial activity of cultured cells were evaluated using LDH‑ and MTS‑tests, respectively. Results. The most effective concentration of LPS was found while the modeling the inflammatory process on hippocampal cell culture. The micropolarization mode was optimized for studying of MP effects in vitro under conditions of neuroinflammation modeling. Using LDH‑test, it was found that the viability of the hippocampal cells was significantly lowered at LPS 10 mg/ml. MP (0.25 mV during 4 h) increased the resistance of cultured cells to LPS. It has been shown that MP significantly enhances mitochondrial activity under normal conditions, and also normalizes cellular metabolism at LPS (MTT‑test). Conclusions. The conditions for experimental MP study in vitro in the modeling of the inflammatory process of the neuronal tissue have been optimized. Kovalenko","doi":"10.30978/unz2018237","DOIUrl":"https://doi.org/10.30978/unz2018237","url":null,"abstract":"Мета  — вивчити особливості когнітивних та емоційно‑вольових виявів, їх взаємозв’язок з больовими синдромами та динаміку у хворих у різні періоди інсульту. Матеріали і методи. Обстежено 62 хворих з больовими синдромами після гострого порушення мозкового кровообігу в анамнезі, з них 24 жінки (середній вік — (67,5 ± 1,9) року) та 38 чоловіків (середній вік — (71,6 ± 1,3) року). В основну групу залучили пацієнтів з больовим синдромом у різні періоди після інсульту (ранній та пізній відновний, резидуальний). Наявність афазії та показники тесту MMSE (Mini‑Mental State Examination) нижче ніж 20 були критеріями вилучення з дослідження. Для порівняння емоційно‑вольових показників було набрано додаткові групи: хворі з хронічними больовими синдромами без інсульту (n = 30) та практично здорові особи (n = 30). Магнітно‑резонансна томографія підтвердила ішемічний інсульт у 80,65 % хворих, геморагічний — у 11,29 %, у решти — змішаний характер процесу. В групі пацієнтів, які мали інсульти в басейні середньомозкових артерій (СМА), у 42,4 % випадках інсульт локалізувався в басейні правої СМА, у решти — у басейні лівої СМА. У групі пацієнтів з двома або трьома інсультами в анамнезі 6 мали вогнищеві пошкодження в басейнах кровопостачання обох СМА. В резидуальному періоді перебували 73 % хворих. Глибокий геміпарез діаг­ностовано у 9,67 % хворих, помірний — у 8,06 %, легкий — у 61,29 %, пірамідну недостатність — у 20,98 %. Серед 85,48 % хворих з порушеннями статики та координації виражену атаксію спостерігали у 7,55 % осіб, помірну — у 47,17 %, легкого ступеня вираження — у 45,28 %. Усім хворим проведено оцінку когнітивного стану за шкалою MMSE. Реактивну та особистісну тривожність визначали за шкалою Спілбергера — Ханіна. Всі дослідження здійснювали при госпіталізації в неврологічне відділення. Результати. Показники особистісної та реактивної тривожності у хворих з больовими синдромами після інсульту статистично значущо (р   0,05). Аналіз показників тривожності залежно від періодів інсульту виявив зростання реактивної тривожності від пізнього відновного до резидуального періоду. Установлено статистично значущі прямо пропорційні зв’язки між рівнем реактивної тривожності та інтенсивністю дорсалгій (r = 0,4, p = 0,02), локалізацією головного болю (r = 0,3, р = 0,045). Виявлено різницю між кількістю активних скарг хворих та скарг за результатами ретельного опитування, що вказує на недостатню увагу до виявів і своєчасного лікування хронічного болю, порушення сну тощо у пацієнтів після інсульту. Висновки. Аналіз показників особистісної та реактивної тривожності залежно від періоду інсульту виявив зростання рівня реактивної тривожності від пізнього відновного до резидуального періоду, а також аналогічну залежність інтенсивності больових синдромів від періоду інсульту.
