{"title":"Actigraphy as a marker for therapy efficacy in patients with restless legs syndrome against the background of diabetic polyneuropathy","authors":"N. Tkachuk, V. Gryb","doi":"10.30978/UNJ2018-3-46","DOIUrl":"https://doi.org/10.30978/UNJ2018-3-46","url":null,"abstract":"Objective — to determine the effectiveness of complex treatment in patients with restless leg syndrome due to diabetic polyneuropathy by means of actigraphy and diagnostic scales. Methods and subjects. 43 patients with diabetic polyneuropathy, demonstrating the absence of gabapentin treatment effectiveness, were diagnosed with restless legs syndrome using the main and auxiliary diagnostic criteria. Patients were divided into 2 groups. Patients of the I group (n = 21) received pramipexole, patients of the second group (n = 22) received pramipexole in combination with zopiclone. The patients were interviewed on the international restless legs scale and the Epworth drowsiness scale. To objectify the manifestations and the treatment effectiveness all patients underwent actigraphy on admission and 30 days after the treatment. Results of the research and their discussion. While determining the dynamics of periodic limb movement index during sleep, it was found that it decreased in both groups and was 6.87 ± 0.02 versus 16.29 ± 0.13 before treatment in I group (p < 0.05) and 4.79 ± 0.04 versus 17.34 ± 0.12 before treatment in ІІ group (p < 0.05). According to the Epworth scale, the reduction of the scale score in both groups was found. In patients of the I group after treatment it was 7.86 ± 0.12 points, that is typical for the moderate drowsiness, as opposed to the ІІ group, where the scale score reduced to the 4.43 ± 0.13 points, which is typical for normal sleep (p < 0.05). Conclusions. The findings proved that the monotherapy with pramipexole improves the condition of the patients with restless leg syndrome, reducing the motor component of the disease, but does not significantly affect sleep. {"title":"Characteristics of the clinical course of ischemic stroke against the background small vessel disease","authors":"L. V. Panteleenko, V. Y. Krylova","doi":"10.30978/UNJ2018-3-26","DOIUrl":"https://doi.org/10.30978/UNJ2018-3-26","url":null,"abstract":"Objective — to study the clinical course and functional recovery of patients with various pathogenetic subtypes of ischemic stroke (IS) in small vessel disease (SVD). Methods and subjects. We performed a cohort prospective study of 117 patients with acute IS in SVD. 48 men and 69 women (mean age 67.7 ± 8.2 years) were included into study. Patients were divided into three groups based on the pathogenetic subtype of IS: atherothrombotic (AT, n = 66), cardioembolic (CE, n = 24) and lacunar (LI, n = 27). Inclusion criteria were: the first IS, history of arterial hypertension (AH), signs of SVD according to MRI. All patients underwent a comprehensive clinical, neurological, laboratory and instrumental examination. The diagnosis and the subtype of IS were verified by Doppler ultrasonography of head and neck vessels, Echo CG, ECG, and cerebral MRI. The degree of impairment of neurological functions was assessed on the 1st, 7th and 14th days after the hospitalization by NIHSS. On the 14th day, the level of daily life activity was assessed using the Bartel index (BI), and the degree of disability was estimated using the modified Rankine scale (mRS). Results. There was a difference in the signs of SVD in various pathogenetic subtypes of IS: significantly more white matter hyperintensity was found in patients with the AT subtype, whereas with the CE subtype, lacunae were observed more often (p < 0.01). It was revealed that SVD has more impact in AT and LI subtypes of IS. SVD has been shown to have different interdependence between stroke severity and functional recovery in various IS subtypes: the white matter hyperintensity and expansion of perivascular spaces on MRI relate to bigger stroke severity and worse functional recovery in AT subtype, whereas microhemorrhages and lacunas have greater effect in LI. Severity of stroke and functional status of CE subtype of IS do not corelate to SVD. Conclusions. In the presence of AH, signs of SVD can be detected in all pathogenetic subtypes of IS, regardless of the presence of other risk factors. Related to SVD pronounced neuroimaging changes develop at the 2nd stage of AH. Different neuroimaging signs of SVD are recorded with different frequency in AT, CE and LI subtypes of IS. Their presence mostly affects the course of AT and LI subtypes of IS. This review testifies the effectiveness of using the in vivo and in vitro toxic cuprizone model for demyelination with the purpose of studying the pathogenic factors which include the immune, endocrine and oxidative stress factors associated with age, sex and strains of animals, as well as finding remyelinizing/neuroprotective remedies. The advantages of the cuprizone model for demyelination over other experimental models are also described. Using, as an example the effectiveness of our application of cytokines (leukemia inhibitory factor) and hormones (melatonin) with the remyelinating effects we show the promise of the in vitro demyelination cuprizone model for screening the remyelinating factors.
