Over the last decade, the use of e-health solutions by patients, healthcare providers, professionals and citizens has increased significantly. This article provides an overview of e-health innovations in Poland, Spain, Romania and Estonia, focussing on the digital development of healthcare services. It outlines the concept of e-health and the innovative solutions the countries listed have introduced in this area. They all face challenges such as the cost of implementing new technologies, data access and the security of patient data. As a result, there is a constant need for coordinated collaboration, legislation and clear guidelines for the use of technology in healthcare. The lessons learnt from these countries are helping to shape the digital future of healthcare in Europe. In Poland, the Ministry of Health oversees the eHealth Centre, which has made remarkable progress in the field of eHealth services such as the Internet patient account, electronic prescriptions and electronic referrals. The biggest challenges include financing innovation and improving digital skills. Poland has also recently introduced the no-fault system as an alternative compensation system for medical errors, modelled on similar systems in Denmark and Sweden. Technological progress is driving the transformation of the Spanish healthcare system, which is particularly evident in the area of telemedicine and innovative healthcare systems. The Spanish Digital Health Strategy lines up with broader national initiatives. Despite challenges such as underfunding, Romania has introduced digital healthcare with the Health Electronic Record since 2014. As an absolute pioneer in the digitalisation of healthcare in Europe, Estonia is proud of its achievements in the digitalisation of healthcare data and services. The Estonian eHealth Foundation and the nationwide health information system are an example of seamless integration and set a benchmark for comprehensive digital healthcare.","PeriodicalId":295830,"journal":{"name":"Prospects in Pharmaceutical Sciences","volume":" 11","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-03-20","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140388636","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In addition to developed countries, underdeveloped and developing countries are investing more in the health sector and technology and governments are trying to control increasing health expenditures. One of the important and dynamic sub-sectors of the health sector is the pharmaceutical sector. States and international companies are investing more in the pharmaceutical sector in order to combat the epidemics in recent years and are conducting more R&D studies by utilizing the developing technology. In this study in the light of the data published by the OECD, it is examined whether there is a similar relationship between the change in the share of health expenditures in the GNP (Gross National Product) of OECD countries between 2010 and 2022 and the change in the size of the pharmaceutical sector in OECD countries. The change in the size of the pharmaceutical sector and the change in the share of health expenditures in GNP have not yielded similar results in each OECD country. It is thought that different factors such as demographic characteristics, geographical location and economic conditions of countries should be taken into consideration when evaluating the relationship between these two variables.","PeriodicalId":295830,"journal":{"name":"Prospects in Pharmaceutical Sciences","volume":"22 30","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-03-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140397447","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Many of these diseases require the introduction of new therapies to improve the prognosis and quality of life of patients. Drugs with the increasing use in recent years are the SGLT2 inhibitors (SGLT2i): canagliflozin, dapagliflozin, empagliflozin. They demonstrate multiple pleiotropic actions with potential applications in CNS diseases. In addition to renal tubules, SGLT receptors are also found within the central nervous system. In numerous studies in animal models, SGLT2i have had promising results in the treatment of neurological conditions such as Alzheimer's disease, autism spectrum disorders, lesions caused by vascular diseases or complications of ischaemic stroke. SGLT2 inhibitors reduce oxidative stress and activation of inflammatory processes within the CNS, which may in the future be used to treat neurological diseases. So far, published studies on the effects of SGLT2 inhibitors on the nervous system are promising, but extensive, multicentre randomised trials on large groups of patients are needed to understand the exact mechanisms of action, therapeutic effects and potential side effects of SGLT2i. \u0000","PeriodicalId":295830,"journal":{"name":"Prospects in Pharmaceutical Sciences","volume":"29 3","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140440435","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Steroidal MRAs, like spironolactone and eplerenone, have been in use for decades. They have an established position in the management of hypertension and heart failure with reduced ejection fraction (HFrEF). There are studies showing that MR antagonism has anti-inflammatory and anti-fibrotic effects resulting in cardiovascular and renal protection. However, broader use of steroidal MRAs is seriously limited by antiandrogenic side effects and the risk of hyperkalemia. The differences in structure as well as pharmacokinetic and pharmacodynamic properties between steroidal and non-steroidal MRAs result in reduced risk of side effects while offering the benefit of cardiorenal protection. Finerenone is currently the only non-steroidal MRA approved by European Medicines Agency (EMA). It is indicated for patients with chronic kidney disease (CKD) with albuminuria and type 2 diabetes mellitus (DM2). Several studies showed a reduction of the risk of cardiovascular and renal events in this group of patients compared to placebo. The risk of hyperkalemia was increased resulting in discontinuation of treatment in more patients than placebo, but the risk of serious adverse events did not differ significantly. The other indications for finerenone, including heart failure (HF) management, are currently being researched.","PeriodicalId":295830,"journal":{"name":"Prospects in Pharmaceutical Sciences","volume":"410 ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140457476","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Traditional uses of F. ulmaria include inflammatory problems like rheumatism, arthrosis, and arthritis), gastrointestinal disorders, liver malfunction, and gout. The pharmaceutical industry has been very interested in this plant because of its health benefits. The presence of phenolic secondary metabolites is thought to be the primary cause of meadowsweet's biological activity and therapeutic efficacy. This study was intended to compare and evaluate the biological activities (antibacterial, antioxidant, antitumor and toxicity) and phenolic profiles (total phenol-flavonoid content and individual phenolic constituents) of F. ulmaria aerial parts obtained from two different sources (naturally- and in vitro-grown). Antibacterial activity was evaluated using 17 different bacteria (10 human and 7 fish pathogens) with disc diffusion method. Methanolic extracts of in vitro-grown parts showed higher antibacterial effect than naturally-grown parts with all tested bacterial pathogens (human and fish). On the other hand, other extracts (aqueous, ethanol, hexane, and ethyl acetate) demonstrated higher antibacterial potential with field-grown parts. The highest sensitivity was observed with Staphylococcus epidermidis, S. aureus and Enterobacter cloacae to aqueous extract of field-grown plants. The potent antibacterial activity of F. ulmaria extracts rendered the Agrobacterium tumefaciens-induced potato disc tumor assay inapplicable. Toxicity assay (brine shrimp) showed that aqueous extract of both sources had the lowest toxicity. Methanolic extract of both parts had a strong antioxidant potential (DPPH radical scavenging activty) having IC50 values as 205.65 µg/mL and 206.74 µg/mL, respectively, and similar level of overall total phenol-flavonoid contents. Individual phenolic analysis with HPLC-DAD showed that the most prevalent phenol was rutin in both sources. While the quantities of chlorogenic acid in both aerial parts were similar, the parts that were grown naturally had higher levels of salicylic acid and rutin. These findings demonstrated the effective antibacterial and antioxidant properties, high phenolic content, and low toxicity of both F. ulmaria sources, indicating that they can be utilized in nutraceutical industry due to their high health promoting potential.","PeriodicalId":295830,"journal":{"name":"Prospects in Pharmaceutical Sciences","volume":"19 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140461862","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

MTX jest antagonistą kwasu foliowego, o silnym powinowactwie do enzymów szlaku foliowego. Jest lekiem o dużej toksyczności, ale o udokumentowanym przeciwdziałaniu działaniom niepożądanym za pomocą kompensacyjnej terapii leukoworyną, aby korzyści ze stosowania leku przewyższały ryzyka. Stosowanie wysokich dawek metotreksatu (HDMTX) polega na podawaniu dawek MTX w bolusie dożylnym o stężeniu wyższym niż 500 mg/m2 powierzchni ciała. Ze względu na to, że stężenia te są skrajnie toksyczne, konieczne jest prowadzenie terapii monitorowanej stężeniem leku we krwi. Podczas farmakoterapii prowadzonej przy zastosowaniu MTX prowadzi się rutynową kontrolę stężenia MTX i kreatyniny w osoczu lub surowicy, jednocześnie dostosowując dawkę wlewu leukoworyny, z ewentualnym dodatkiem glukarpidazy, która przyspiesza metabolizm MTX do mniej toksycznych metabolitów. Celem niniejszej pracy był skrótowy przegląd farmakologicznych właściwości MTX, połączony z szczególnym uwzględnieniem zmienności w farmakokinetyce tego leku, uzasadniającym konieczność terapeutycznego monitorowania stężeń. Omówiono terapię HDMTX, zalecenia kliniczne, jak również sposób prowadzenia terapii na podstawie światowych wytycznych. Dodatkowo, omówiono problematykę zarówno leczenia za pomocą MTX, jak i leczenia ratującego przy pomocy leukoworyny i glukarpidazy. Leczenie typu HDMTX powinno być prowadzone pod szczególnym nadzorem, zgodnie z przyjętym protokołem klinicznym. Tylko odpowiednia współpraca lekarza, personelu pielęgniarskiego, diagnostów laboratoryjnych, farmaceutów klinicznych oraz, co najważniejsze, pacjenta powinna pozwolić na skuteczne leczenie, z odpowiednim zredukowaniem toksycznych działań niepożądanych do minimum.","PeriodicalId":295830,"journal":{"name":"Prospects in Pharmaceutical Sciences","volume":"36 16","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-11-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135038339","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The present study aimed to investigate the biological activities of Agrocybe praecox (Pers.) Fayod, commonly referred to as the Spring Fieldcap mushroom. The ethanol extract of the mushroom was obtained with the Soxhlet apparatus. Subsequently, the quantification of total antioxidant, total oxidant, and oxidative stress index of the mushroom extract was conducted utilizing Rel Assay kits. The total phenolic content was determined using the Folin-Ciocalteu reagent. The agar dilution method was employed to assess the antimicrobial activity of the mushroom extract. The antiproliferative activity of the compound was assessed using A549 lung cancer cells. Elemental levels of the fungus were measured using the wet digestion method. The findings of the study revealed that the levels of the elements fell within the range reported in the existing literature. In addition, it was determined that the total antioxidant value was 2.97±0.08, the total oxidant value was 7.63±0.16, the oxidative stress index was 0.26±0.01, and the total phenolic content was 49.7±3.0 mg/g. It was found to be effective against microorganisms at concentrations between 50-200 µg/mL. Furthermore, it was ascertained that this mushroom exhibited antiproliferative activity, with the degree of activity being contingent upon the concentration of the extract. Consequently, it was determined that Agrocybe praecox exhibits potential as an antioxidant, antimicrobial, and anticancer agent.","PeriodicalId":295830,"journal":{"name":"Prospects in Pharmaceutical Sciences","volume":" 37","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-11-09","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135293248","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
