{"title":"Characteristics of Emergency Department Visits at King Abdulaziz University Hospital: A One Year Analysis","authors":"J. Abualenain","doi":"10.4197/MED.25-1.1","DOIUrl":"https://doi.org/10.4197/MED.25-1.1","url":null,"abstract":"This study seeks to report the frequencies of the Emergency Department visits at King Abdulaziz University Hospital, as well as an analysis of its operational parameters. This is a retrospective analysis of all emergency visits over one year. In 2017, 69,522 patients were triaged, 62.55% adults, 20.12% pediatrics, and 17.33% OB/GYN. Ineligible patients (lower acuity stable non-Saudi patients who hold health insurance through their sponsors) were 12.84% of triaged patients, they were advised to go to another hospital. Females were 57.11%, and 40.92% were between 24 and 43 years, 18.49% younger than 14 years, and 9.22% older than 64 years. Saudi patients were 64.76%. Only 0.40% arrived by ambulance. Full management was provided for 73.83% of registered patients, whereas 26.17% left the waiting room after triage and initial evaluation by a physician without being treated due to bed unavailability; 97.22% were lower acuity and stable. Admission rates were 23.44%. The overall emergency department mortality rate was 0.52%, and death on arrival rate was 0.25%. Most registered patients (86.18%) were lower acuity and stable. All cases in which the patient needed resuscitation were treated immediately. Our study aimed to determine the average age of the initiation of toilet training among Saudi children and the factors that may affect this training. This study was carried out by distributing questionnaires to mothers who attended various clinics in our institute. A sample included 1000 healthy Saudi children. The mean age of the initiation of toilet training in healthy children was 25.6 ± 9.4 months while the median was 24 months, the mean duration of training was 6.3 ± 9.1 months. Most children (92%) were trained in the summer months. Mother’s educational level, number of siblings, and socioeconomic status were significant factors that affected the age of initiation. However, mother’s occupation, caregiver, and the order and gender of the child did not have a significant effect on the initiation of toilet training. The median age of toilet training among our children matched other studies. This poses a considerable clinical dilemma especially that the prevalence of morbid obesity continues to escalate in the United States of America, and as a result the number of gastric bypass procedures performed each year has similarly increased dramatically. The author presented a case of a 45-year-old male with refractory hypertension, secondary to mechanical complications after a Roux-en-Y gastric bypass procedure due to anatomical alteration contributing to pill retention and change in medications' pharmacokinetics. Investigations for secondary causes of hypertension were made, patient’s blood pressure had become exceedingly diffi cult to control after his Roux-en-Y gastric bypass procedure and had only been controlled with IV medications. To prevent such a mistake, a clinical and pathological correlation is recommended. In this retrospective study, we are documenting the clinical and pathological features of this disease through the last 15 years (2001-2016) in King Abdulaziz University Hospital; and comparing results with that of international findings. From a total number of 1805 lymph nodes sent for histopathological examination during that period, only 9 cases had been diagnosed as Kikuchi–Fujimoto disease. Three (33%) cases were 20 years old and under; five (56%) cases were in the ages between 20 to 40 years old and only one (11%) was above 40 years old. Eight (89%) cases were females. Regarding the outcome, all cases showed complete recovery and there was no recurrence. DOI: 10.4197/Med. 25.1.2 Abstract The western region of Saudi Arabia is known as a multinational region with diff erent ethnic groups of people. This study aims to evaluate the ABO and rhesus blood groups among study subjects in the Western area of Saudi Arabia and their comparison with other regions of the Kingdom. It is a retrospective study. 35,388 participants were included. ABO blood grouping was done using tube method. Agglutination in any tube or hemolysis was considered as positive. Blood grouping was done by a preliminary fi nger prick, and was repeated again and a serum sample was obtained at the time of donation. Statistical Package for Social Sciences software was used for the data analysis. Results showed that the most common blood group is O, (50.1%) followed by A (29.7%), B (16%) and less frequent is AB (4.1%). Rhesus positive were 91.3%, while rhesus negative were 8.63%. These results demonstrate that the most common blood group in the Western province is O. Blood group A was noticed to be less frequent. We present here a 42-year-old female complaining of a painless lump in the right breast for the previous two months. Physical examination revealed a palpable mass. Lumpectomy was performed and histopathologic examination revealed metaplastic breast carcinoma with foci of chondrosarcoma. The patient received adjuvant radiation therapy and chemotherapy and is currently following up as an outpatient. The clinical and radiographic results indicated amelogenesis imperfecta. Because of uncooperative behavior, treatment was accomplished under general anesthesia. The use of composite resin and stainless steel crowns produced eff ective and satisfying improvement of the patient’s aesthetic looks, in addition to prevention of further sensitivity and wear so enhancing his functional needs. The lastingness of the thorough management of this patient needed powerful stress on careful oral hygiene. There is a major role of multidisciplinary team to the eff ective management of patients with hypomaturation type of amelogenesis imperfecta. There are several variations of the modified breast lift, including circumareolar and circumvertical techniques, each of which has its own unique characteristics and approaches. The aim of this retrospective study was to evaluate and compare the uses of circumareolar (n = 55, 27.20%) versus circumvertical (n = 147, 72.80%) augmentation-mastopexy in 202 women over a 10 years period. Results showed that 28 patients (50.90%) with circumareolar augmentation-mastopexy need revised surgery due to undesired breast shape (n = 24, 43.60%), stretched areola (n = 17, 30.90%) and bad scar (n =9, 16.40%); while 32 patients (22.40%) with circumvertical augmentation-mastopexy need revised surgery due to secondary ptosis (n = 29, 19.70%), large areola (n =3, 2.00%) and unpleasant vertical scar (n = 13, 8.80%). Twenty-four men ≥19 years-old with premature ejaculation for ≥ 6 months and intravaginal ejaculation latency time ≤ 2 minutes underwent transurethral intraprostatic injections of onabotulinumtoxinA (100 U). Primary endpoint was change of intravaginal ejaculation latency time at 3-months. Secondary endpoints included changes in premature ejaculation profile and patient-reported global impression of change (PGI). Mean baseline ejaculation latency time has significantly increased at 1-, 3- and 6-months, respectively. In premature ejaculation profile “perceived control over ejaculation”, significant improvement was reported at 3-months, while non-significant changes were reported at 1- and 6-months. Patients reported non-significant changes of “personal distress related to ejaculation” and “interpersonal difficulty related to ejaculation”. Only 8.3%, 12.5% and 12.5% of men reported “better” at 1-, 3- and 6-months, respectively, while all other patients reported “no change” or “slightly better” in patient-reported global impression of change. No serious adverse effects were observed. Improvements of intravaginal ejaculation latency time were not clinically meaningful, as most men reported “no change” or “slightly better” in patient-reported global impression of change.
