Constantinescu","doi":"10.4197/med.22-3.2","DOIUrl":"https://doi.org/10.4197/med.22-3.2","url":null,"abstract":"The purpose of this study is to assess the loco-regional control and overall survival in head and neck cancer patients, as well as evaluate the clinical benefit of intensity-modulated radiotherapy implemented in 2011 at our Hospital. Data of 117 patients between 2007 and 2014 was reviewed retrospectively. Cumulative survival and disease control rates were calculated by Kaplan-Meier product-limit actuarial method. Loco-regional control and survival rates for intensity modulated and three-dimensional conformal radiotherapy were compared by a logistic regression test. After a median follow-up of 12 months, 53 (51%) patients who underwent radical radiotherapy were free of disease, 43 (42%) with disease, and seven (7%) unknown. During this time, 31 (26%) patients died from the disease. Using actuarial estimates for the two-year follow-up, this study found that significant gains in survival were obtained by switching treatment modalities. {"title":"Maternal and Fetal Outcome in a Patient with Buccal Embryonal Rhabdomyosarcoma","authors":"W. Fageeh, Ossama H. We report a 19-year-old female with rhabdomyosarcoma who was classified with stage IIA disease, based on the clinical grouping system developed by the Intergroup Rhabdomyosarcoma Study Group in 1972. The patient was treated with surgery and chemotherapy. The histopathological and radiological assessment showed no residual tumor in adjacent tissues to contradict the clinical classification. The patient was scheduled for radiotherapy but left against medical advice and returned pregnant six months later with recurrence. She chose not to terminate the pregnancy, restricting the treatment options to chemotherapy. At 32 weeks of gestation, she underwent a caesarean section due to fetal distress. The patient deteriorated and passed away shortly after. The baby, however, is healthy. Complications due to alloimmunization of red blood cells antigen remain a major risk as a post transfusion effect. The objective of this study is to determine the frequency of red cell alloimmunization in Jeddah, Saudi Arabia. A retrospective cross-section study of sickle cell disease patients at King Abdulaziz University Hospital between 2012 and 2013 was performed. Demographic characteristics and transfusion history was recorded. Blood samples were analyzed for alloimmunization using immunohematological technique. A total of 234 sickle cell patients were analyzed, of which 30 (12.8%) showed alloantibodies. A total of 43 alloantibodies were found out of which 28 belonged to Rh group, eight belonged to Kell while three belonged to MNS group. Demographic and transfusion characteristics were analyzed between alloimmunized and nonalloimmunized sickle cell disease patients. The rate of alloimmunization in Jeddah, Saudi Arabia was 12.8%. There was significant difference observed between alloantibodies detection between transfused patients compared to non-transfused patients. DOI: 10.4197/Med. 22.2.1 Abstract Several studies have suggested that there is a high discrepancy between clinical and radiographic knee osteoarthritis. The objectives of this study were to examine association between radiographic classifi cation and clinical manifestations of knee osteoarthritis, and to determine if the assessment of individual radiographic features was superior to the general radiographic scale in establishing such a relationship. A total of 125 patients with knee osteoarthritis were enrolled in this study. Radiographic features were assessed with the Kellgren-Lawrence grade scale for general radiographic grading, and a line-drawing atlas for detailed radiographic analysis. The severity of knee pain, stiff ness, and disability were measured using the Western Ontario and McMaster Universities Osteoarthritis Index. Patients' age and pain duration were found to correlate signifi cantly with knee pain, stiff ness, and disability. No association between general radiographic grading scale and clinical manifestations was found. However in detailed radiographic analysis, osteophyte site at the patellofemoral joint was found to correlate with knee stiff ness. In conclusion, radiographic scores were not found to be closely associated with the clinical features of knee osteoarthritis. DOI: 10.4197/Med. 22.2.5 Abstract This prospective study evaluates the role of robotic surgery in the management of large renal stones. Robotic renal stone surgery is safe and eff ective, and may be considered among the treatment options for large renal calculi in adults, especially for patients with concomitant ureteropelvic junction obstruction. The benefi ts brought by robotic systems include improved 3-D visualization, greater dexterity, precision, and increased range of motion. Thus, robotic pyelolithotomy has been recommended for the treatment of renal calculi with concurrent upper tract abnormalities. The effi cacy and safety of robotic pyelolithotomy as a treatment of large renal pelvic stones and partial staghorn stones is evident in our study, with a substantially high stone clearance rate. We were able to complete the robotic procedures safely and successfully in all but one of our 38 patients, who was converted to open surgery due to bleeding and severe adhesions. A complete medical history was obtained and all participants underwent a complete review of clinical data including electrocardiogram, echocardiogram, blood pressure measurement, and thyroid examination. The following laboratory examinations were performed: international normalized ratio; thyroid stimulating hormone; triiodothyronine; thyroxine; total cholesterol; triglyceride; low-density and high-density lipoprotein. A history of hospitalizations, stroke and in-hospital mortality were recorded. We recruited 84 women; the mean age was 61.8 years and the mean body mass index was 28.45 kg/m2. The mean hemoglobin level of the patients was 12.2 g/dL and the mean thyroid stimulating hormone level was 3.75 mIU/L. The target international normalized ratio was achieved in 58% of treated patients; 70% of the under target International Normalized Ration patients had been hospitalized more than once and 19% had suffered a stroke. {"title":"Coronary Artery Bypass Graft for Left Main Coronary Artery Disease","authors":"H. Jabbad","doi":"10.4197/med.22-2.34","DOIUrl":"https://doi.org/10.4197/med.22-2.34","url":null,"abstract":"Because left main coronary artery disease carries a high risk of morbidity and mortality, this retrospective study will review the data and results of surgical management of left main coronary artery disease, in King Abdulaziz University Hospital. 448 patients underwent coronary bypass graft into two groups, left main group (50) patients and non-left main group (398) patients. Preoperative data, risk factors and cardiac catheterization findings were compared in between the two groups in addition to perioperative morbidity and mortality. Patients in the left main group were younger in age with significantly lower ejection fraction and more risk factors (hypertension, dyslipidemia, and smoking). In our study the left main group patients had higher mortality than non-left main patients [4 patients = 8%, 6 patients = 1.8%]; the most common cause of perioperative mortality in the left main group was low cardiac output state, and the most common complications were perioperative myocardial infarction and prolonged ventilation. In the last decade, laparoscopy has become increasingly popular and has successfully replaced several open surgical procedures. It has been applied to many aspects of gynecological surgery and has become the procedure of choice for gynecological surgery. A review of the recent literature continues to support the utilization of laparoscopy for the management of genital tract malformation.
