Method: Retrospective, case series study of 1372 patients with Covid-19, from march 15 to may 23, 2020, with a follow-up of 3 months. The demographic and epidemiological characteristics were analyzed. Results: Of the 1372 patients, 19,9% were admitted to hospital and 25,18% of them were hospitalized in a critical unit. The median age was 40 years, there were more men than women and 68.5% was Chilean. 80,8% had FONASA as a health insurance. A lethality of 2% was observed. {"title":"Clasificación molecular del cáncer colorrectal, su impacto pronóstico y terapéutico: un paso crucial hacia la medicina personalizada","authors":"J. González M., G. A total of 25% of patients debut in metastatic stages, and 50% of patients with resectable disease subsequently have disseminated disease, thus their systemic therapy constitutes an important challenge. Although there are advances in their treatment, patients have a variable and unpredictable response due to their molecular heterogeneity, making it necessary to identify the specific mutations that leads oncogenesis in each patient. The CRC classification into 4 molecular subtypes or consensus molecular subtypes (CMS) has had wide clinical acceptance. CMS1 (immune), affects young patients, is rapidly progressive and refractory to conventional therapies, and could benefit from aggressive therapy and immunotherapy. CMS2 (canonical) is characterized by mutation in specific pathways linked to cellular metabolism. The CMS3 (metabolic), where its specific mutation is KRAS. Finally, CMS4 (mesenchymal) is associated with metastasis, worse prognosis and mutation in pathways of fibrogenesis and epithelial-mesenchymal transition. Specific treatments have been proposed for each of these types with the purpose of reaching a personalized medicine. The objective of this is study is to determinate clinical and epidemiological characteristics of patients with coronavirus en emergency department. The HCUCH Emergency Service treated a total of 6959 patients between March 13th and May 31th, of whom 1.278 were positive and had confirmed with coronavirus. The male sex was the most prevalent (59.7%). The most frequent symptoms in both groups were cough, myalgia and fever. In the group of ambulatory patients, headache stood out in 50% and in hospitalized patients, dyspnea with 67%. The mortality rate in hospitalized patients was 15.6%. Of these, 66.6% were older than 65 years. Regarding diagnosis of hospital discharge, 87.5% correspond to pneumonia. There is a higher prevalence of coronavirus disease in male patients. The most frequent comorbidities in hospitalized patients were HT and DM2. The highest rate of hospitalization and case fatality in people over 65 years of age. El quehacer docente asistencial fue interrumpido y la presión asistencial de la pandemia obligó a transformar la infraestructura, aumentando muy significativamente las camas críticas (UCI e Intermedio), redistribuyendo personal clínico y suspendiendo la actividad quirúrgica electiva. {"title":"Herpes zóster diseminado: caso clínico y revisión bibliográfica","authors":"Nicolás Moya G., Emilia Neves J.","doi":"10.5354/2735-7996.2020.69839","DOIUrl":"https://doi.org/10.5354/2735-7996.2020.69839","url":null,"abstract":"Herpes zoster classical clinical presentation is the acute onset of multiple vesicles over an erythematous base, disposed over one or two dermatomes with up to 20 vesicles located outside the main dermatome. Disseminated herpes zoster is an atypical and rare form of presentation of herpes zoster, which manifests with lesions beyond the described territory. It occurs mainly in patients with some type of cellular immunosuppression. The diagnosis is made with the medical history and physical examination, however, it should be confirmed with laboratory tests. Treatment must be initiated early to avoid serious complications, such as bacterial infection of the lesions, post-herpetic neuralgia, or even central nervous system involvement. The drug of choice is intravenous acyclovir that must be maintained until the cessation of the appearance of new lesions, and then switch to its oral presentation for another 5-7 days. Disseminated herpes zoster mortality rounds 5-15%. There are varicella-zoster virus vaccines, that have been shown to reduce the incidence of herpes zoster relapses, however its utility to disseminated herpes zoster is uncertain and further studies are required.
